That even includes recent changes here at VRM Media. Although I am sad to say goodbye to Vitamin Retailer s former editor Daniel Breeman who has moved on to new opportunities I am very excited to step up to the task of editor-in-chief of VR magazine after most recently serving as the managing editor I can assure our readers that I am firm in my belief of the important role dietary supplements play in health and well-being and it is my goal to continue to provide the best news information and services to be an invaluable resource to retailers and other industry members. In regard to change in the industry I m referring to a recent wave of actions intended to help consumers but for some seemed to be based on purely political motives. One of course being the New York State Attorney General Eric Schneiderman issuing cease-anddesist letters to four major retailers-- calling on them to pull their own brands of herbal supplements from their shelves. While this has proven to been a true challenge to the supplement industry I am heartened to learn of the recent joint statement between Schneiderman and Daniel Fabricant PhD CEO of the Natural Products Association We share a common objective of ensuring herbal products bought by consumers are authentic pure and sold in full compliance with consumer protection laws. Anyone who buys an herbal supplement should be able to do so with full knowledge of what is in the product and have complete confi- 1994 ea t ng 20 Y CelebraEixcellence f o rs 2014 I dence that every precaution was taken to ensure its authenticity and purity. While no single test or technology alone can provide complete confidence to consumers we support the application of DNA barcoding technology as part of a multi-faceted approach to assuring authenticity and identifying substitution. This includes testing herbal botanical ingredients prior to extraction in circumstances where DNA barcode is available for the relevant species. Together we look forward to jointly working with major manufacturers and retailers of herbal supplements to promote a model for product safety authenticity and transparency in this industry. We view this dialogue as a positive step in resolving our differences and we are hopeful that we can work in collaboration to enhance confidence and safe access for consumers. This month s Back Talk column features another industry association leader Karen Howard CEO and executive director of the recently formed Organic & Natural Health Association who also visited with the New York attorney general office to voice the association s concerns. You can read her Q&A article with VR on page 64. Overall I m hoping these conversations lead to better understanding realistic expectations and a more transparent industry. A B B A S Q U TA B hile CBD (cannabidiol) has been a recent relevant subject of interest to all in our industry we must begin the journey of understanding this position by looking a bit differently at the most important botanical that exists in nature--cannabis When it comes to the subject about the cannabis plant (also known as hemp) it has not been until recently that we have fully realized the botanical travesty that has been perpetrated on us. Cannabis use dates back thousands of years ago for a variety of purposes including food fiber and for its medicinal value. In addition cannabis and its associated active compounds called cannabinoids (aka. phytocannabinoids) have been of medicinal significance for thousands of years for its pain relieving and analgesic effects. Today there are more than 80 phytocannabinoids that have been identified with many having significant therapeutic value in a wide range of clinical conditions--from neuroprotection to treating cancer. Yet over the past few decades the U.S. government s continued exaggerated campaign about the harmfulness of cannabis and placing it as a Schedule 1 drug had put a halt to conducting valid research on this important plant. While its scheduling erroneously places it along with serious drugs such as heroin it also categorizes cannabis hemp as a plant which has no medical use or benefit. This clearly flies in the face of the following facts 1) Cannabis has a long history of safe use for thousands of years in China India and other countries 2) Cannabis was previously listed in the British and U.S. Pharmacopeia in the late 1800s as one of the most widely prescribed remedies of the time 3) Thousands of valid scientific clinical studies on the therapeutic benefits of the plant and or its active cannabinoids have been published over the past few years alone (thanks to countries that did not stifle research) 4) The existence of medicinal drugs based on cannabinoids here in the U.S. and finally 5) The U.S. Government was issued a patent in 2003 for the NeuroProtective & Antioxidant Effects Of Cannabinoids W Understanding that in the 20th century cannabis has been used more for its euphoric effects than as a medicine I will not diminish the issue of abuse in street cannabis and the its high THC content (tetrahydrocannabinol is the psychoactive cannabinoid). Nevertheless cannabis use as a medicinal remedy before the 20th century is well documented. Its current manipulation to contain high levels of THC does not represent the phytocannabinoid profile that should normally exist in the plant and negates the importance of the other non-psychoactive cannabinoids that play a pivotal role in the maintenance of overall health in the body such as CBD New Discovery The ECS While humans have used cannabis and hemp for centuries it has only been the last few decades that scientific understanding of how the active cannabinoids work in the body has evolved. To understand the importance of how these plant cannabinoids stabilize the body and help to maintain good health we must introduce one of the most important medical discoveries that was made in the early 1990s--the existence of an endocannabinoid system (ECS) in our body. Interestingly while we all have learned about important physiological systems such as the circulatory system digestive system cardiovascular system neurological system etc. the ECS is very unique in that it is interconnected with almost every bodily function and acts as a regulator for homeostasis. The endocannabinoid system is a network of receptors known as CB1 (primarily in the brain) and CB2 (primarily in the immune system and peripheral tissue) and are also scattered throughout many other tissues and organ systems. Essentially both types of cannabinoid receptors are found throughout the entire body but are distributed differently. When activated they serve important diverse purposes affecting motor function pain inflammatory response memory immune response digestion bone growth blood pressure and helps protect brain cells just to name a few. Once the ECS system was discovered the immediate scientific response was directed at what compounds were produced in the body to bind and activate these receptors It did not take too long after the ECS finding where the first endocannabinoids were discovered--yes cannabinoids produced in the body that can attach to and activate the CB1 and CB2 receptors. What we have come to now understand is that cannabinoids are important to stability and functionality in the body and that the endocannabinoid system can be found not only in humans but also in all animals as well. In addition earliest evidence of the development of an ECS system may be millions of years ago in the simple sea squirt. So this ancient signaling network we now understand and call the endocannabinoid system helps modulate inflammatory processes many aspects of functionality in our brains and numerous physiological and biochemical processes affecting stability throughout the body. Dysfunction or deficiency of the ECS seems to be involved in the cause of certain lifestyle diseases such as cancer atherosclerosis Crohn s disease dementia and arthritis among others. Because of its regulatory role in the body many feel that the ECS serves as an adaptogenic system to help keep the body in homeostasis balance and reverse stress related inflammatory processes that lead to pain and associated inflammatory diseases. In the group of approximately 80 naturally occurring phytocannabinoids found some of them have been shown to exert similar properties to the cannabinoids produced in the body. Certain phytocannabinoids bind directly to and activate the CB1 & CB2 receptors in the body while some such as CBD help to build up stores of our own cannabinoids by decreasing their breakdown. In any case only recently have we begun to understand the actions of these important plant compounds and their benefits to the body. Research on cannabinoid pharmacology and biology has begun to offer new insights into their medical value and clinical use. Unfortunately from a sociopolitical standpoint the phytocannabinoid THC has been the focus of attention recreationally as it is responsible for the euphoric properties. The present manipulation of the cannabis plant to contain abnormally high levels of THC for recreational purposes has overshadowed the importance of other components such as CBD as well as THC itself. In controlled doses THC is an effective phytocannabinoid that has neuro-protective anti-inflammatory anti-nausea effects and works in tandem with CBD. GW Pharmaceuticals has produced several varying THC CBD & CBD medications for treating multiple sclerosis cancer pain and epilepsy and they are going after cancer next. Due to its therapeutic effects and being non-psychoactive CBD has now been the focus of extensive research and highlighted in the media by CNN s Chief Medical Correspondent Dr. Sanjay Gupta. His three-part series on the medical value of CBD was truly an eyeopener for all of us and further validates the significance of this important phytocannabinoid. The series unraveled the compelling data on the therapeutic value of CBD in children treated for epileptic seizures. Unlike THC CBD is not psychoactive does not affect the brain like THC and actually can reduce the euphoric effects of THC. In several studies CBD alone has been shown to be very safe and well tolerated even at high doses and also exhibits anti-inflammatory neuro-protective and antioxidant effects. CBD as well as highlighting its potential therapeutic benefits for many conditions including neurological conditions (seizures multiple sclerosis etc.) inflammatory diseases cancer (prevention and treatment) depression anxiety PTSD and a variety of pain conditions. The question as to CBD s legality and how it comes to market is challenging at best. While we have been witnessing changes in state laws governing the legal status of medicinal cannabis it is not as clear for CBD. If you look at the legal definition of cannabis which includes its phytocannabinoids technically CBD is erroneously considered a Schedule 1 drug under federal law even though it has no psychoactivity and even though the U.S. government has a patent on its therapeutic value With a Schedule 1 drug status still hovering over medical cannabis we must now look at the hemp plant as a potential source of CBD and other important phytocannabinoids. Hemp (often called industrial hemp ) is the same plant species as medical cannabis (Cannabis sativa) but has very low levels of THC in comparison. In addition they are genetically different and bred differently for different purposes. While hemp contains protein an ideal ratio of omega-3 omega-6 fatty acids phytosterols fiber vitamins minerals etc. it also contains cannabinoids such as CBD in its leaves and smaller amounts in its stalk and seeds. With medical cannabis unavailable to extract CBD due to its drug status we have an opportunity to extract important phytocannabinoids from industrial hemp. Due to the specious laws that have even kept hemp illegal to grow in the U.S. (with the exception of two states that can grow for research purposes) it is federally legal to import it into the country providing it contains less than 0.3 percent THC. Today utilizing unique extraction and distillation techniques imported naturally occurring phytocannabinoid rich hemp oil from stalk and seeds is available under strict tariff codes and FDA (U.S. Food and Drug Administration) regulations. While this is an important step in providing the industry and consumers a natural healthy source of beneficial phytocannabinoids there still remains the issue that CBD is not yet a dietary supplement. Companies are in process to apply for GRAS (generally recognized as safe) status as the safety data for CBD is solid. There is much to be written on this subject to help educate clinicians retailers and consumers on the importance of CBD and other phytocannabinoids as being the most clinically relevant plant compounds we have seen in a very long time. Because of their biological effects the existence of an ECS system in our body that feeds off of cannabinoids the discovery of ECS deficiency states requiring cannabinoid supplementation and the roles CBD has in a variety of disease conditions these important plant compounds will surely dominate medicine and nutrition over the next decade and more. The retailer also pulled all products with Acacia rigidula unless testing confirms that the supplements do not contain BMPEA. Physician Pieter Cohen of Harvard Medical School identified BMPEA as an amphetamine-like substance that was being sold in a number of weight-loss and sports supplements. This is a very important step that goes beyond the steps the FDA has taken to date said Cohen. I hope that the FDA moves to make this their policy as well since there is no legitimate role for Acacia rigidula in supplements and Acacia rigidula has been used as code for synthetic stimulants. NPA CEO and Executive Director Daniel Fabricant PhD former director of Division of Dietary Supplement Programs at the FDA noted that in the case of BMPEA the health effects of this compound are unknown at present. This directly contradicts the claim that BMPEA acts like an amphetamine the health effects of which are widely known and characterized. While BMPEA may N interact with the same receptor targets as amphetamine it won t have the same potency or side effects of amphetamine he said. FDA stated in the warning letters that products declaring BMPEA as a dietary ingredient are misbranded because the substance doesn t meet the definition of a dietary ingredient under the Federal Food Drug & Cosmetic Act (FD&C). NPA supports the agency using all of its resources to protect consumers when it finds an issue with a misbranded or adulterated product in the marketplace Fabricant said. According to the agency TriMethyl Xtreme Distributed by Nevadabased Extreme Products Group claims to contain anabolic steroids not permitted to be sold in a dietary supplement making it an adulterated product. In these cases the full weight of the federal Food Drug & Cosmetic Act permits FDA to issue recalls seizures injunctions and to seek civil or criminal enforcement. At the very least more robust and more frequent inspections of manufacturers under the existing good manufacturing practices (GMPs) regulations are required. Mister added that for those who relate this problem to the actions taken by the New York State attorney general (NYAG) against four major retailers and certain herbal supplements these situations are not comparable. The NYAG used a test that has been roundly criticized as being inappropriate for testing herbal extracts making the foundation of his case without merit he stated. Further in the NYAG s settlement with one of the retailers the agreement indicates that those products in question were (at the time they were removed)--and still are--safe pure properly labeled and in full compliance with all regulatory requirements--which begs the question why did the NYAG demand they be removed in the first place By contrast the matter of intentionally spiking products with illegal anabolic steroids does deserve attention by law enforcement. Since not much has changed since then with regard to any new natural sugar alternative entering the market I decided to make stevia the focus of this year s sugar alternatives article with an examination of stevia s benefits along with being an effective sweetening agent. The fact is in-vitro animal and human studies have variously demonstrated that stevia s pharmacological activities and therapeutic benefits include antitumor and anticancer anti-inflammatory antihyperglycemic antihypertensive antidiarrheal immunomodulatory diuretic and enzyme inhibitory actions.1 In this article I will review research on the use of stevia for its antioxidant properties and its effects on glucose tolerance insulin sensitivity satiety and blood pressure. L Stevia background Although I provided this background information on stevia in last year s article it bears repeating here. Native to subtropical and tropical regions from western North America to South America stevia is a genus of herbs and shrubs in the sunflower family. Stevia rebaudiana commonly known as stevia is grown for the sweetness of its leaves. Stevia s sweetness occurs more slowly and lasts longer duration than sugar. 44 VITAMIN RETAILER The limitation of stevia is that its extracts generally have a bitter aftertaste sometimes described as licorice like. The compounds in stevia providing its sweet taste are called steviol glycosides and provide up to 300 times the sweetness of sugar. The major steviol glycosides are stevioside and rebaudioside A (aka RebA).2 It is important to note that whole-leaf stevia or crude stevia extracts have not had GRAS (generally recognized as safe) approval as a food additive. Rather Sweet Green Fields Blue California McNeil Nutritionals Cargill and Whole Earth Sweetner Merisant have all received GRAS approval for concentrated rebaudioside A products derived from stevia.3 Besides its functioning as a sugar alternative stevia may offer other health benefits. Human research indicates that stevioside (750-1 500 mg day) may be effective in lowering blood pressure in hypertensive patients.4 Other clinical research suggests that stevioside 1 000 mg day) might reduce postprandial glucose levels by 18 percent in people with type 2 diabetes.5 However since the GRAS approved stevia compound is rebaudioside A it is not clear that commercially available products will have the same effects. Antioxidant Properties Glucose Tolerance and Insulin Sensitivity A study6 examining the effect of harvest time and site found that that ste- via contains a high level of phenols and high antioxidant activity. As expected stevia was found to be effective for inhibiting free radicals. The authors concluded that The strong antioxidant properties make very interesting the possibility of using stevia extracts to improve functional food properties. In another study 7 the effects of stevia on streptozotocin (a naturally occurring chemical that is particularly toxic to the insulin-producing beta cells of the pancreas in mammals)induced diabetic rats was examined. The rats were divided into different groups including groups fed whole stevia leaves powder or extracted stevia polyphenols. The results showed that there was a reduction of blood glucose and an improvement in insulin levels in the stevia whole leaves powder and extracted polyphenols fed rats compared to control diabetic group. Furthermore stevia whole leaves powder and extracted polyphenols also reduced the free radical activity (as measured by MDA concentration in liver) and improved antioxidant status through antioxidant enzymes. Glucose tolerance and insulin sensitivity were improved. In addition stevia leaves and extracted polyphenol alleviated kidney damage caused by streptozotocin. In one study 8 the effect of aqueous stevia extracts on a glucose tolerance test was investigated in 16 normal volunteers. Aqueous extracts of 5 grams of leaves were given at six-hour intervals for three days. Glucose tolerance tests were performed before and after each stevia administration. Results showed that stevia extract improved glucose tolerance decreasing plasma glucose levels during the test and after overnight fasting in all volunteers. In another study 9 12 people with type 2 diabetes received 1 000 mg day of stevioside a constituent of stevia or a placebo with a test meal. Later those that received the stevioside were given the placebo instead with the test meal and vice-versa. In either case blood samples were drawn at 30 minutes before and for 240 minutes after ingestion of the test meal. Results showed that compared to control stevioside reduced post-meal glucose levels by 18 percent (P .013). In addition stevioside also increased the insulinogenic index by approximately 40 percent by compared to control (P .001). The authors concluded Stevioside may be advantageous in the treatment of type 2 diabetes. and satiety levels did not differ between groups. Furthermore stevia preloads significantly reduced postmeal glucose levels compared to sucrose preloads (p .01) and postmeal insulin levels compared to both aspartame and sucrose preloads (p .05). This research demonstrates that stevia can indeed be used to maintain satiety while dieting and even help modulate glucose levels in the process. and positive effects on glucose tolerance insulin sensitivity satiety and blood pressure. References Blood pressure In addition to the aforementioned benefits of stevia this alternative sweetener has also been shown to help reduce blood pressure in people with hypertension. In a multicenter randomized double-blind placebocontrolled trial11 in 168 men and women (aged 20-75) with mild hypertension the effects of 500 mg stevioside powder or placebo three times daily for two years were examined. Blood pressure was measured at monthly clinic visits patients were also encouraged to monitor blood pressure at home using an automated device. After two years the stevioside group had significant decreases in average systolic blood pressure (from 150 to 140 mmHg) and diastolic blood pressure (from 95 to 89 mmHg) compared to starting blood pressure (P 0.05) and compared with placebo (P 0.05). Based on patients records of self-monitored blood pressure these effects were noted beginning approximately one week after the start of treatment and persisted throughout the study. Another multicenter randomized double-blind placebo-controlled study12 was undertaken in 106 hypertensive men and women (aged 29-75). Subjects received 250 mg stevioside or placebo three times daily. Their blood pressure was measured monthly for one year. The results were that after three months the systolic and diastolic blood pressure of the stevioside group decreased significantly (systolic 166.0 to 152.6mmHg diastolic 104.7 to 90.3 mmHg P 0.05) and the effect persisted during the whole year. Satiety Given its benefits for glucose tolerance one might speculate that stevia would also have benefit as part of a weight-loss program. However some critics suggest that using stevia rather than sugar as a sweetener would simply cause the dieter to eat more calories in compensation. To test this hypothesis a study10 was conducted in which 19 healthy lean (BMI 20.0-24.9) and 12 obese (BMI 30.0-39.9) individuals (18-50 years old) received preloads containing stevia (290 calories) aspartame (290 calories) or sucrose (493 calories) before the lunch and dinner meals. Hunger and satiety levels were reported before and after meals and every hour throughout the afternoon. Participants provided blood samples immediately before and 20 minutes after the lunch preload. Results showed that despite the caloric difference in preloads (290 calories vs. 493 calories) participants did not compensate by eating more at their lunch and dinner meals when they consumed stevia and aspartame versus sucrose. During menopause as well as the time just prior (perimenopause) anxiety in women is especially common due to the hormonal changes taking place. Lower estrogen levels in particular are thought to trigger feelings of anxiety in many women. Defining Anxiety Anxiety refers to an unpleasant emotional state ranging from mild unease to intense fear. Anxiety differs from fear in that while fear is a rational response to a real danger anxiety usually lacks a clear or realistic cause. Though some anxiety is normal and even healthy higher levels of anxiety are not only uncomfortable but can lead to significant problems. Anxiety is often accompanied by a variety of symptoms. The most common symptoms relate to the chest such as heart palpitations (awareness of a more forceful or faster heart beat) throbbing or stabbing pains a feeling of tightness and inability to take in enough air and a tendency to sigh or hyperventilate. Tension in the muscles of the back and neck often leads to headaches back pains and muscle spasms. Other symptoms can include excessive sweating dryness of the mouth dizziness digestive disturbances and the constant need to urinate or defecate. And along with anxiety of course comes the inability to relax which may lead to difficulty in getting to sleep and constant waking through the night. 58 VITAMIN RETAILER Generalized anxiety disorder (GAD) is characterized by a persistent state of exaggerated worry and fear (at least six months) often when there is little or nothing to provoke it. Severe anxiety will often produce what are known as panic attacks --intense feelings of fear. Panic attacks may occur independent from anxiety but are most often associated with generalized anxiety or agoraphobia. Agoraphobia is defined as an intense fear of being alone or being in public places. As a result most people with agoraphobia become housebound. Natural Approaches to Anxiety As women in their 40s and 50s are going through menopause declining estrogen levels often produce feelings of anxiety. This link is the result of the effect that lower estrogen levels have on brain s regulation of moods and emotion. Changes in estrogen levels lead to alterations in important neurotransmitters (brain chemicals that transmit nerve impulses)-- including serotonin norepinephrine dopamine and melatonin--that play a huge role in how we feel. About one in four women will experience more noticeable anxiety or depression during perimenopause and menopause. Before offering some suggestions with specific natural products to address anxiety there are important dietary recommendations that can help during this change of life. Perhaps the most important is simply to increase the consumption of plant foods especially those high in phytoestrogens while reducing the consumption of animal foods. Phytoestrogens are plant-derived substances that are able to weakly bind to the estrogen receptors in mammals and have a very weak estrogen-like effect in some tissues and a weak anti-estrogen effect in other tissues. Soy foods and flax seeds contain a high content of phytoestrogens and have received a lot of attention as phytoestrogen sources but many other foods such as apples carrots fennel celery parsley and other legumes also contain phytoestrogens though in smaller amounts. A high dietary intake of phytoestrogens is thought to explain why hot flashes and other menopausal symptoms including anxiety appear to occur less frequently in cultures consuming a predominantly plant-based diet. In addition such a diet is promising for disease prevention with some research showing a lower incidence of breast cancer and heart disease in women consuming highphytoestrogen diets. Fish Oils Fish oils concentrated for the omega-3 fatty acids EPA and DHA have been shown to have positive effects for patients with many different types of psychological disorders associated with stress including anxiety depression bipolar disorder (manic depression) borderline personality disorder and attention deficit disorder with hyperactivity (ADHD). Results from trials in menopausal women showed particularly interesting results. For example in a 2009 study conducted in Quebec Canada 120 women going through menopause were WWW.VITAMINRETAILER.COM JUNE 2015 given either a fish oil supplement providing 1 200 mg of EPA DHA or placebo for eight weeks. The baseline level of hot flashes was an average of 2.8 per day. After eight weeks the hot flash frequency decreased by 55 percent in the EPA DHA group but only 25 percent decrease in the placebo group. Fish oil supplementation was also associated with an improvement in quality of life scores as well as mood. months of use). Women in the St. John s wort group reported improvements in psychological symptoms linked to menopause significantly better quality of life scores fewer sleep problems as well as a feeling of sexual well-being. Maca Black Cohosh In the last 30 years black cohosh (Cimicifuga racemosa) has emerged as the most studied of the herbal alternatives to hormone replacement therapy for menopause symptoms. The collective findings in black cohosh studies and long-term clinical anecdotal evidence on black cohosh indicate that it is most effective for menopause symptoms of hot flashes day time or night time mood swings sleep disorders and body aches. It also appears to be helpful in reducing anxiety as well as symptoms of anxiety such as heart palpitations (see Table 1). Some recent studies have used black cohosh extract in combination with other botanical extracts such St. John s wort red clover soy and chaste berry. Of these St. John s wort deserves some additional attention. Another botanical medicine that can be of great benefit in relieving anxiety during menopause is maca (Lepidium meyenii)--an historic herbal remedy from Peru often used to enhance male sexuality but which also has some very interesting effects on women. Research on menopausal women using a maca indicates that unlike hormone replacement therapy (HRT) and phytoestrogenic botanicals maca can increase the body s production of estrogen--versus simply adding estrogen replacement to the body--and lower levels of cortisol. What makes this especially interesting is that from other research conducted on the composition of various powdered preparations of maca root it appears that the herb does not contain plant estrogen or hormones. It has been suggested that maca s therapeutic actions rely on plant sterols stimulating the hypothalamus pituitary adrenal and ovarian glands and therefore also affecting the thyroid and pineal glands thus improving sleep mood fertility energy and hot flashes. As such maca tends to treat menopausal symptoms as a whole it doesn t treat any one specific symptom of menopause (such as hot flashes) alone. In one double-blind four-month study of early postmenopausal women patients were given either placebo or two 500 mg capsules of maca twice daily for a total of 2 g day. After two months maca stimulated estrogen production and suppressed cortisol. It also had a small effect on increasing bone density and alleviated numerous menopausal symptoms including hot flashes insomnia depression nervousness and diminished concentration. In regard to these herbal approaches to menopausal symptoms here is a little clarity Choose one of the following for general symptom relief o Black cohosh extract The dosage of the extract used in the majority of clinical studies has been 2 mg of 27-deoxyacteine twice daily. o Maca Gelatinized maca extract 1 000 mg twice daily or dosage equivalent to 3 500 dried powdered maca root daily. If symptoms of anxiety or depression are significant (can be used with either of the two approaches above) o St. John s wort extract standardized to 0.3 percent hypericin 900 to 1 800 mg daily Final Comments Increased lactic acid levels may be an underlying factor in many cases of anxiety. She has worked to strategically advance the mission and vision of organizations through effective advocacy and strong collaboration. She has held a variety of executive positions including serving as professional staff for a Congressional committee and has policy expertise in the diverse areas of integrative and complementary medicine managed care health care technology and mental health. Prior to joining Organic & Natural Health Association (O&N) she served as president of the National Animal Supplement Council (NASC) where she managed the NASC Quality Seal Program a voluntary compliance program for manufactures and distributors of pet supplements requiring adherence to the association s GMPs (good manufacturing practices) participation in a national adverse event reporting system and a bi-annual audit. Howard also served as executive director for both the American Association of Naturopathic Physicians (AANP) and the Association of Accredited Naturopathic Medical Schools. Here she discusses a recent meeting with the New York attorney general s office and her thoughts on improving supplement safety and quality practices. Howard We went to New York to share our perspective on how best to keep bad products off the shelves and demonstrate our willingness to improve the regulatory process in ways that benefit consumers. We represent the companies committed to the highest quality standards available and the consumers desire for products they can trust. Our members value business and marketing practices that are unambiguous that requires traceability and accountability for all products including herbal supplements. We discussed the entirety of the manufacturing process with the AG s office and shared our perspective on how consumers who buy herbal supplements will be better served by requiring raw ingredient suppliers to adhere to the same GMP requirements as manufacturers. For us testing is one of many tools that work to ensure ingredient identity quality and safety. A tool that is only as good as the test itself and the moment and the batch the test was applied. We wanted the AG to hear our perspective on the power of adhering to the dietary supplement GMPs the standards quality companies are currently using and how the quality suffers from not requiring consistent application of these rules. Howard The purpose of the petition is to apply the FDA s current cGMPs (code 21 CFR Part 111) to raw material manufacturers thus ensuring supply chain integrity is managed at the earliest possible point. This will require manufacturers of raw ingredients to have a strong working relationship with farmers and that appropriate cGMPs are instituted at the point where raw materials are manufactured. Doing so will provide flexibility welcomed by branded ingredient manufactures that are already following cGMPs but are forced to compete with lower quality ingredient manufactures that have minimal if any cGMPs. VR What issues did you discuss during your meeting with the New York attorney general s office affecting the natural products industry Howard We opened our discussion with an offer to collaborate and serve as an industry resource to the Attorneys General (AG). There is always room for improvement in any industry and the supplement industry is no exception to that rule. Organic & Natural is committed to ensuring consumer access to quality products. We went to open a dialogue with the AG offices with the primary goal of making sure consumers have confidence in the authenticity and purity of the products they are purchasing. Did we disagree with the NY AG in certain areas Yes. And it was clear from our meeting the AG agrees with our position that this issue is not solely about testing. Supply chain integrity took center stage in our discussion. VR How important is supply chain integrity Howard and we stand on the principle that FDA and FTC (Federal Trade Commission) have the authority to remove these substandard and often dangerous products off the shelves. True they do already have that authority. The Organic & Natural Health Association is willing to take this one step further and petition FDA to impose existing cGMPs on raw ingredient suppliers. Without taking this step the integrity of the supply chain will continue to result in poor products poor health care outcomes and worse attributions by industry critics. Will supply chain integrity eliminate all criticism and fix all our problems with DSHEA No. And it is essential to making dramatic strides in protecting consumers by improving the quality of products on the shelves.
