At 90-yearsyoung my grandmother lives by herself in a three-story house and is completely self-sufficient. She still cooks cleans does her own laundry and dotes on all of us when we visit her. And when she tells people her age they can t believe it they assume that she is only in her early 70s. So what is her secret My grandmother remains active by exercising daily taking walks around her neighborhood and eating a healthy balanced diet. While my entire family hopes to inherit her genes and live long healthy lives just like her--the reality for many Americans is that that isn t the case. Research shows that Baby Boomers in the United States are more likely to have a disability later in life than the previous generation. For instance a 2013 study published in JAMA Internal Medicine titled The Status of Baby Boomers Health in the United States The Healthiest Generation found that while Baby Boomers may be living longer than people approximately 20 years older they aren t necessarily healthier. The study showed that even though they are less likely to smoke or have emphysema they are more likely to be obese have diabetes or high blood pressure than the previous generation at similar ages. In addition influenza and pneumonia also continue to contribute to deaths among older adults despite the availability of effective vaccines. In this month s issue highlight The Baby Boomer s Diet Dilemma (see page 24) Ron Antriasian vice president of sales and business development for Life Extension noted that a senior s limited income may have an effect on their food choices which is directly correlated to their health. In many cases seniors on a fixed income may choose to cut back on costs they can control including fresh fruits vegetables and organic foods in favor of cheaper less healthy alternatives he explained. Diabesity may affect as many as 100 million Americans and nearly a billion people around the world but compare the public health response to previous epidemics--polio for example. It doesn t come close. Perhaps this is because it s a silent insidious epidemic developing over years with debilitating symptoms that seriously impact a person s quality of life. Yes we have taken some measures such as urging people at risk to improve their diet and exercise habits. This approach places the emphasis on the lifestyle choices of the individual but new research along with the skyrocketing rates of diabetes suggests that we re missing some key pieces of the puzzle. As a nation we adhere religiously to the notion of calories in and calories out. Eat less exercise more and everything will be fine. This is not entirely wrong--but it s clearly an oversimplification. We cannot pre- tend that metabolism functions in isolation sequestered from environmental influences and the delicate balance of our biological systems. So it s not a question of following the same strategies--except more vigorously. We need to look beyond the well-worn tropes that have dominated our approach to these conditions. Fortunately there s a growing body of research to help us better understand the complex factors behind metabolic syndrome and type 2 diabetes. Two factors emerging as key culprits environmental toxins and poor quality sleep. The Toxic Load While what we eat and how much certainly affect our weight and susceptibility to diabetes and metabolic syndrome this oversimplified equation ignores the body s ability to process these calories. Again there is a growing body of evidence that overexposure to environmental toxins can impair our intricate metabolic mechanisms. Numerous studies demonstrate that many of the chemical compounds pervasive today have an adverse impact on metabolism. A study published in The Lancet found a correlation between persistent organic pollutants (POPs) in blood and insulin resistance.1 Another study described the different ways toxins provoke insulin resistance such as mitochondrial injury oxidative stress inflammation and debilitated thyroid metabolism.2 Research published in JAMA showed BPA found in plastics canned foods and even cash register receipts increases risk of diabetes.3 Toxins have been shown to interfere with an entire class of nuclear receptors (called PPARs) causing insulin resistance and other harm.4 Another study found that weight gain and fat storage in rats exposed to chemical toxins was completely independent of calories and exercise.5 There are dozens of studies with similar findings and they paint a toxic picture 8 NATURAL PRACTITIONER WWW.NATURALPRACTITIONERMAG.COM AUGUST 2015 Go to www.naturalpractitionermag.com kyowa for info about this advertiser PRACTITIONER CORNER environmental pollutants appear to scramble our metabolic signals impairing glucose management and weight control mechanisms. Clearly genes and genetic expression play a role as well but as so many have suggested Genetics loads the gun environment pulls the trigger. While it s upsetting to see that common chemicals are having such a profound impact on metabolism--and other areas of health--the fact that research is elucidating some of these complex mechanisms means we may be zeroing in on effective therapeutic targets. ingredients and supplements which are shown to reduce levels of toxins in the body. But it s important to do it right so as not to overwhelm your system or deplete essential nutrients. I rarely recommend extreme measures such as rapid detox programs fasting or colonics. Rather an emphasis on nutrient-dense whole foods and select botanicals and nutrients offers a gentle yet effective route to eliminating toxins from the body over time. Our bodies are designed with an elaborate system of detoxification mechanism incorporating many organ systems and biochemical pathways including the skin lungs liver and kidneys. The daily intake of dietary phytochemicals found in common foods herbs and nutrients provides ongoing support for the optimal functioning of our inherent detox capacities. Cruciferous vegetables such as broccoli cabbage kale and bok choy are well-known detoxifiers and also help promote healthy hormone metabolism. Other effective detoxifiers include green tea garlic milk thistle dandelion leaf and root onions and turmeric. One clinical study showed that broccoli sprouts helped the body detoxify a number of airborne pollutants particularly benzene. A half cup a day enhanced excretion of benzene acrolein and other toxins. There are also a variety of vitamins minerals and other nutrients that support detoxification such as L-methylfolate zinc selenium N-acetyl-cysteine glutathione and vitamin C. Alginates derived from kelp are also effective detoxifiers shown to remove heavy metals radioactive isotopes and pesticides from the digestive tract. Alginates also support healthy glucose metabolism. Another clinically proven detoxifier is modified citrus pectin (MCP). Made from the pith of orange peels MCP has a welldeserved reputation for safely binding and removing toxins such as lead mercury arsenic and others while not affecting essential minerals. MCP also binds and blocks galectin-3 an inflammatory protein that s been linked to cancer fibrosis heart disease and other conditions. Detoxification Given the quantity of toxins we face in our everyday lives detoxification plays an important role in maintaining long-term health on a number of levels. Like industrial pollutants sleep deprivation has become a common feature of modern life. It s well known that poor sleep can lead to a host of health problems including problems with immunity cellular health digestion and cognitive wellbeing--including the ability to flush toxins from the brain. Now we can add metabolism to the list. This is not really news. There have been studies as far back as 1969 showing that sleep deprivation even for just a few days decreases insulin sensitivity and increases glucose levels. One study found that people who slept only four hours each night for six nights reduced their glucose tolerance by 40 percent prematurely aging their metabolism. The issue reversed after normal sleep was restored.6 Another study found similar results even with less severe sleep deprivation--5.5 hours per night over 14 nights.7 Other studies have shown that loss of sleep contributes to increases in certain growth hormones associated with increased glucose and cortisol.8-10 Lack of sleep has also been shown to increase the release of inflammatory cytokines which can also increase insulin resistance as well as causing other problems.11 lular function neurological health and offer other important benefits. Routine plays a critical role in good sleep and also helps balance circadian rhythms which in turn can benefit metabolic function. It s best to go to bed at the same time each night and embrace relaxation routines before bedtime. That means avoiding televisions smart phones and computers at least two hours before bed as well as other electronic devices that emit blue light since this disrupts melatonin production. Melatonin naturally increases in a dark environment so make sure your bedroom is free of glowing electronics and external light sources such as streetlights. There are many herbs and nutrients that can also support relaxation and good sleep. One extract emerging as a multi-purpose ingredient is honokiol derived from magnolia bark. In a report published in the Journal of the Academy of Nutrition and Dietetics they describe how this tool can be used to more accurately evaluate dietary consumption of all types of fluids. They found that higher HBI scores were associated with more favorable lipid profiles decreased risk of hypertension and among men better C-reactive protein (CRP) levels. Water consumption is associated with numerous health benefits and beverage recommendations exist but few have evaluated overall beverage intake quality. Beverage intake guidelines have been suggested and although the 2010 U.S. Dietary Guidelines recommend drinking water instead of sugary drinks no tools existed that measure overall beverage intake quality. A Healthy Beverage Index (HBI) similar to the Healthy Eating Index could be used to evaluate overall beverage intake quality and to determine if improvements in beverage intake patterns are associated with improvements in health explained Kiyah J. A great deal of attention has been directed at sugarsweetened beverage (SSB) intake and a broader focus beyond just SSBs is needed. Dr. Duffey and co-investigator Brenda M. Davy PhD developed the HBI a 10-item scoring index that captures total energy from beverages total fluid requirements and recommended limits for beverage subgroups such as low-fat milk fruit juice and alcohol. They weighted some components of the HBI more heavily because of their recognized contributions to good health such as water contributing at least 20 percent of total fluid intake and others less heavily for example consuming no R more than 8-oz. of fruit juice. Using dietary and health data from more than 16 000 adults who participated in the nationally representative National Health and Nutrition Examination Surveys (2005-2010) Drs. Duffey and Davy calculated HBIs and correlated those with cardiometabolic risk factors such as obesity overweight hypertension high fasting insulin high fasting glucose high low-density lipoprotein (LDL) cholesterol and high CRP. The HBI score ranges from 0 to 100 with a higher score indicating better adherence to beverage guidelines and a healthier beverage intake pattern in both men and women. The average HBI score was 63 16 out of 100 for the sampled population. Their analysis considered age sex race ethnicity level of education completed marital status household size total daily energy intake and physical activity as possible confounding factors. Drs. Duffey and Davy found that people with better HBI scores had more favorable cardiometabolic outcomes. Among normal weight males a 10-point higher HBI score was associated with an average of 36 percent lower odds of having a high waist circumference and 7 percent lower odds of having high CRP levels. For overweight obese males each 10-point increment higher in HBI score was associated with 4 percent lower odds of having high fasting insulin and high LDL cholesterol levels and 3 percent lower odds of having high total cholesterol. The odds of having high CRP were also lower with each 10-point higher in HBI score in this group. Irrespective of weight status each 10-point higher HBI score was associated with 4 percent lower odds of having hypertension. Among all females regardless of weight status each 10-point higher HBI score was associated with an average 4 percent lower odds of having high fasting insulin levels an average 3 percent lower odds of high LDL cholesterol an average 5 percent lower odds of having low high-density lipoprotein cholesterol and an average 3 percent lower odds of having hypertension. For the typical person there are three factors that lower the HBI score. SSB consumption of for example soda or a vanilla latte deducted 15 points. More than 10 percent of daily energy requirements coming from beverages reduced the score by another 20 points. Finally failure to meet total fluid requirements lost another three points for the typical person. Davy emphasized that There is flexibility in the system and there are some beverage categories that an individual could choose not to consume at all for example diet sodas or alcohol yet they would still receive the maximum number of points for that category. The authors would like this technique to be developed into a rapid assessment tool that might be used online to provide patients doctors and dietetics practitioners with accurate consumption information that could be used to encourage better eating behaviors. It s projected by 2050 that Americans aged 65 and older will amount close to 89 million people which is double the number of elderly adults in 2010. The reason individuals are living longer into their 70s and 80s even with the post-World War II Baby Boom. From January 1 2011 and on for the next 20 years approximately 10 000 Americans will blow out their 65th birthday candles making the last Boomers 65 in 2030. By then the aging population will be significantly changed. Close to 72 million people will be considered elderly that s one of every five Americans. The rising number of elderly throughout the country has stimulated advances in medical treatment prolonging the average life expectancy. Chronic diseases such as tuberculosis and syphilis were once a problem. Fast-forward 30 years heart disease cancer Alzheimer s disease and diabetes pose as the current threats. Due to this addressing the aging population s nutritional needs has become more demanding. Even when a person s situation is at its best that individual may be marred by a weakened immune system. That health decline is simply bought on by age. Not consuming enough calories and protein along with essential micronutrients further exposes the elderly population to a reduced immune system. That in turn produces a higher risk for acute illness and infection. Moving forward adopting healthy behaviors at an early age can prevent the drastic health decline down the road. For example studies have indicated that individuals who do not smoke and participate in regular activity during their youth are less likely to develop heart disease cancer and other chronic illnesses. The journal Clinical Intervention Aging published a review in 2010 indicating that 16 percent of those over the age of 65 are malnourished. That number is expected to increase in the next 30 years. Malnutrition is defined by the journal as a state in which a deficiency excess or imbalance of energy protein and other nutrients causes adverse effects on body form function and clinical outcome. There are evident mechanisms of weight loss--wasting cachexia and sarcopenia--that are caused by malnutrition. The first of the three is wasting a primary indication that the body is not receiving proper food intake. This is mostly caused by physiological changes that the aging process presents. Cachexia is when there is a loss of fatfree mass or body cell mass--triggered by catabolism and resulting in changes of body composition. When this occurs there is an acute immune response where cytokines get released effecting hormone production and a spike in resting energy expenditure. An increase in gluconeogenesis is to be expected as well as a negative nitrogen balance to be present. Eventually this instigates the loss of muscle mass also termed sarcopenia. The Current Opinion in Clinical Nutrition and Metabolic Care published a review indicating that 3 to 8 percent of lean muscle mass is lost per decade after 30--hypothesizing to impact 30 percent of individuals over the age of 60 and 50 percent over the age of 80. I decided to take part in The Future of Food Summit because my passion is public health and my major research interests are centered on the role of lifestyle and nutritional factors including vitamin D omega-3s and cocoa flavanols in reducing risks of cardiovascular disease diabetes cancer and cognitive decline. In essence the nexus of food and health is at the core of what I do every day and the impetus behind my participation in the Summit. As a principal investigator of a number of trials that are the result of public-pri- The link between food health nutrition and wellness is profound and inextricable. Poor nutrition inhibits the health and well-being of individuals and populations. Further even moderate degrees of nutrient deficiency can have devastating consequences for individuals across generations and in turn for populations across the globe. The burden of nutritional inadequacy extends beyond personal health and risk of chronic disease to include loss of national economic growth productivity and societal prosperity. Malnutrition is an acute public health issue made more challenging in a society that is aging faster than ever before. By 2050 21 percent of the worldwide population two billion people will be over 60 years old. In the United States eight in 10 seniors (60-plus) will be living with one or more chronic health challenges. As elderly populations grow and length of life increases the impact of nutrition on health and wellbeing should be a global priority and public health focus. What was the panel s opinion regarding food sleep and brain development The panel I participated in at the Future of Food Summit offered stimulating discussion. Scientists have linked sleep disruption to accelerated aging increased risk of obesity immune dysfunctions and cardiovascular dysfunction. Yet we still lack answers to many questions about mild concussions Alzheimer s disease ADHD (attention deficit hyperactivity disorder) and Parkinson s disease. Dina moderated our panel and engaged each of the panelists in intensive discussion including Dr. David Dinges from the Perelman School of Medicine at University of Pennsylvania Dr. Philip Rubin from Yale School of Medicine and senior advisor to the President who works on the White House BRAIN Initiative (aimed at revolutionizing our understanding of the human mind and uncovering new ways to treat prevent and cure brain disorders) and myself. We explored how food and sleep impact the development and functioning of the brain and we discussed how the BRAIN initiative is helping answer pressing questions such as How do you have a healthy brain for life Why does the brain make certain decisions about food and what are the factors that influence and control this It was a compelling look into the future of brain health and how preventive measures may reduce health risks. Prevention holds the key to achieving optimal health and extending health span in addition to lifespan. Although it s never too late to adopt healthier behaviors and practices beginning early in life holds the greatest promise. DASH diet and the Mediterranean diet incorporate a whole-food type of diet and components such as olive oil nuts and fish--may reduce risk of both heart disease and stroke. Our research group has an interest in rigorously testing promising dietary patterns and nutritional interventions for reducing risks of heart disease stroke diabetes and cancer. The DASH and Mediterranean patterns of eating are linked to improvements in cholesterol levels blood pressure and blood glucose regulation so it makes sense that the heart and the brain may benefit. In addition to this research examining the role of cocoa flavanols in reducing risk of cardiovascular disease and cognitive decline is a promising and exciting research direction. Our new COSMOS trial builds on a decade of scientific evidence indicating that cocoa flavanols improve blood vessel function metabolic health and cognition. There have been previous small-scale trials involving cocoa flavanols with fewer than 100 individuals but the next logical step is to move forward with a large-scale randomized trial that can assess effects on clinical outcomes such as heart attacks and strokes. Thus we re pleased to be launching the COSMOS trial which will enroll 18 000 men and women nationwide. As one of the lead investigators on several health studies including the Q A What have you learned about food and brain health Q A Based on scientific research what dietary changes could people make for better health Having a healthful diet and regular physical activity are fundamental to good health. It s preferable to obtain the nutrients we need from food whenever possible as opposed to relying on supplements. We discussed how an optimal diet includes plenty of vegetables fruits legumes whole foods and minimal intake of processed foods--taking supplements will never be a replacement for a balanced diet. That said some individuals may benefit from moderate supplementation if they have greater requirements or if their diets are lacking in certain areas. For example moderate supplementation with calcium and vitamin D may be helpful for bone health folic acid supplementation is important for women of child-bearing age and some individuals may need supplementation with B12 or other vitamins minerals. Regarding physical activity it s so important that I ve often taken out my prescription pad to write specific recommendations for brisk walking or other types of exercise for my patients. AUGUST 2015 WWW.NATURALPRACTITIONERMAG.COM NATURAL PRACTITIONER 65 NATURAL HEALTH STUDIES Vitamin D Supplementation Shows Promise for Crohn s Disease N ew research published in the journal United European Gastroenterology (UEG) suggests that supplementation with vitamin D may impact on the intestinal barrier dysfunction associated with Crohn s disease (CD) and could have a role in the treatment of the condition. The study is by Professor Maria O Sullivan and Tara Raftery Department of Clinical Medicine Trinity Centre for Health Sciences St. James s Hospital Dublin Ireland. Incidence of CD varies across Europe with up to 10 cases per 100 000 population per year. Generally case rates are higher in northern and Western Europe than Southern and Eastern Europe. There is emerging data that vitamin D supplementation may prolong remission in CD however the clinical efficacy and underlying mechanisms remain unclear. In this new research the authors aimed to determine changes in gut barrier function (as determined by intestinal permeability and antimicrobial peptide concentrations) as well as disease markers in CD in response to vitamin D supplementation. In a double-blind randomized placebocontrolled study the authors assigned 27 CD patients in remission to 2 000 IU day vitamin D supplementation or placebo for three months. They found that patients treated with the supplementation were more likely to maintain their intestinal permeability whereas this deteriorated in the placebo group. Increased intestinal permeability is considered a measure of gut leakiness which is shown to predict and precede clinical relapse in CD. In addition patients with the highest blood levels of vitamin D had signs of reduced inflammation (measured by C-reactive protein and antimicrobial peptides) and these patients also reported better quality of life. This is the first reporting of effects of vitamin D supplementation on intestinal permeability and antimicrobial peptide measures in a CD cohort The authors concluded. Whilst the data requires further confirmation it broadly supports evidence from previous experimental studies that suggest a role for vitamin D in maintaining intestinal barrier integrity. While the data is promising the authors highlighted that in order to understand its translation into treatment for CD further larger randomized controlled trials will be required. (Source United European Gastroenterology-- June 2015) Is Dietary Supplementation Appropriate for Children With ASD hildren with autism spectrum disorder (ASD) are often picky eaters which can lead parents to suspect that their children might not be getting adequate amounts of vitamins and minerals. This sometimes leads parents of children with ASD to try nutritional supplements and dietary regimens such as gluten-free and casein-free (GFCF) diets without professional supervision. In the largest study of its kind published in the Journal of the Academy of Nutrition and Dietetics researchers report that these well-intentioned efforts can result in both insufficient nutrients and excessive nutrients. Despite supplementation children with ASD still were deficient in calcium for example while some were consuming excessive amounts of vitamin A and other nutrients. A total of 368 children aged 2-11 were recruited from five AS ATN sites at Cincinnati Children s Hospital University of Arkansas University of Colorado University of Pittsburgh and University of Rochester. All had been diagnosed with autistic disorder Asperger disorder or pervasive developmental disorder as defined by the DSM-IV. Three-day food records were completed C for the children by their caregivers. A registered dietitian nutritionist trained the caregivers to record the amount of all foods beverages and nutritional supplements consumed including brand names and recipes used for food preparation. In the case of nutritional supplements photographs of the labels were taken to insure that ingredients were accurately recorded. Registered dietitian nutritionists verified these records and immediately called families if clarification was needed. By examining these detailed eating records investigators found that the children were consuming similar amounts of micronutrients as children without ASD. They also had the same deficits in vitamins D E calcium potassium and choline as the general pediatric population. Although ASD children are given supplements more often (56 vs. 31-37 percent of the general population) even after supplementation 40-55 percent were lacking in calcium and 30-40 percent were lacking in vitamin D. Children on the GFCF diet consumed more magnesium and vitamin E. This may be due to the substitution of soy and nutbased products. Children on this diet were more adequately supplemented with vitamin D. Calcium supplementation was equally inadequate in those on and off the diet. Despite different eating behaviors children with ASD received much of their needed micronutrients from food consumption. This might be due to the high levels of fortification in the modern food supply where vitamins and minerals are often added. This fortification may also be responsible for the overconsumption of certain nutrients by children with ASD. For the supplement users in this study many exceeded the tolerable upper limit for safe intake levels of vitamin A folic acid and zinc. (Source Journal of the Academy of Nutrition and Dietetics-- June 2015) 66 NATURAL PRACTITIONER WWW.NATURALPRACTITIONERMAG.COM AUGUST 2015 High Fat Sugar Intake May Relate to Loss of Cognitive Function study at Oregon State University (OSU) indicates that both a high-fat and a high-sugar diet compared to a normal diet cause changes in gut bacteria that appear related to a significant loss of cognitive flexibility or the power to adapt and adjust to changing situations. This effect was most serious on the highsugar diet which also showed an impairment of early learning for both long-term and short-term memory. The findings are consistent with some other studies about the impact of fat and sugar on cognitive function and behavior and suggest that some of these problems may be linked to alteration of the microbiome--a complex mixture in the digestive system of about 100 trillion microorganisms. The research was done with laboratory mice that consumed different diets and then faced a variety of tests such as water maze testing to monitor changes in their mental and physical function and associated impacts on various types of bacteria. The findings were published in the journal Neuroscience in work supported by the Microbiology Foundation and the National Science Foundation. It s increasingly clear that our gut bacteria or microbiota can communicate with the human brain said Kathy Magnusson a professor in the OSU College of Veterinary Medicine and principal investigator with A the Linus Pauling Institute. Bacteria can release compounds that act as neurotransmitters stimulate sensory nerves or the immune system and affect a wide range of biological functions she said. We re not sure just what messages are being sent but we are tracking down the pathways and the effects. Mice have proven to be a particularly good model for studies relevant to humans Magnusson said on such topics as aging spatial memory obesity and other issues. In this research after just four weeks on a high-fat or a high-sugar diet the performance of mice on various tests of mental and physical function began to drop compared to animals on a normal diet. One of the most pronounced changes was in what researchers call cognitive flexibility. The impairment of cognitive flexibility in this study was pretty strong Magnusson said. Think about driving home on a route that s very familiar to you something you re used to doing. Then one day that road is closed and you suddenly have to find a new way home. A person with high levels of cognitive flexibility would immediately adapt to the change determine the next best route home and remember to use the same route the following morning all with little problem. With impaired flexibility it might be a long slow and stressful way home. This study was done with young animals Magnusson said which ordinarily would have a healthier biological system that s better able to resist pathological influences from their microbiota. The findings might be even more pronounced with older animals or humans with compromised intestinal systems she said. What s often referred to as the Western diet or foods that are high in fat sugars and simple carbohydrates has been linked to a range of chronic illnesses in the United States including the obesity epidemic and an increased incidence of Alzheimer s disease. We ve known for a while that too much fat and sugar are not good for you Magnusson said. This work suggests that fat and sugar are altering your healthy bacterial systems and that s one of the reasons those foods aren t good for you. It s not just the food that could be influencing your brain but an interaction between the food and microbial changes. (Source Neuroscience-- August 2015) Vitamin D Supplements May Benefit Children With Kidney Disease M aintaining normal vitamin D levels helps preserve kidney function in children with chronic kidney disease (CKD) according to a study in the Journal of the American Society of Nephrology (JASN). The findings indicate that vitamin D may be a useful addition to treatments for safeguarding kidney health in children with mild to moderate CKD. Blocking the renin-angiotensin-aldosterone system (RAAS) which is a complex hormone system that regulates blood pressure and fluid balance is a cornerstone of therapy in CKD. Unfortunately patients may experience a limited response to drugs such as angiotensin-converting enzyme inhibitors (ACEIs) that target the RAAS over time and intensifying treatment with these drugs can lead to side effects. Patients may experience benefits from adding other therapies that can augment RAAS blockade and clinical trials in adults with CKD have shown that vitamin D may have such an effect. To determine whether vitamin D levels influence CKD progression in children Rukshana Shroff MD PhD (Great Ormond Street Hospital for Children London U.K.) and her colleagues analyzed long-term data on 167 children from a clinical study called the Effect of Strict Blood Pressure Control and ACE Inhibition on Progression of CKD in Pediatric Patients (ESCAPE) trial. Children with lower vitamin D levels had higher levels of blood markers related to kidney dysfunction as well as greater kidney function loss over time. Five-year kidney survival was 75 percent in patients with vitamin D levels 50 nMol L at the start of the study and 50 percent in those with lower levels. Nutritional vitamin D supplements like cholecalciferol have a wide therapeutic window. Thus clinical trials with cholecalciferol a safe and effective vitamin D supplement with minimal need for monitoring are recommended in children with CKD said Dr. Shroff. (Source Journal of the American Society of Nephrology-- June 2015) AUGUST 2015 WWW.NATURALPRACTITIONERMAG.COM NATURAL PRACTITIONER 67 NATURAL HEALTH STUDIES Chocolate for Your Heart ating up to 100 g of chocolate every day is linked to lowered heart disease and stroke risk finds research published online in the journal Heart. There doesn t seem to be any evidence for cutting out chocolate to lower the risk of cardiovascular disease conclude the researchers. They based their findings on almost 21 000 adults taking part in the EPIC-Norfolk study which is tracking the impact of diet on the long term health of 25 000 men and women in Norfolk England using food frequency and lifestyle questionnaires. The researchers also carried out a systematic review of the available international published evidence on the links between chocolate and cardiovascular disease involving almost 158 000 people--including the EPIC study participants. The EPIC-Norfolk participants (9 214 men and 11 737 women) were monitored for an average of almost 12 years during which time 3 013 (14 percent) people experienced either an episode of fatal or nonfatal coronary heart disease or stroke. Around one in five (20 percent) participants said they did not eat any chocolate but among the others daily consumption averaged 7 g with some eating up to 100 g. Higher levels of consumption were associated with younger age and lower weight (BMI) waist hip ratio systolic blood pressure inflammatory proteins diabetes and more regular physical activity--all of which add up to a favorable cardiovascular disease E risk profile. Eating more chocolate was also associated with higher energy intake and a diet containing more fat and carbs and less protein and alcohol. The calculations showed that compared with those who ate no chocolate higher intake was linked to an 11 percent lower risk of cardiovascular disease and a 25 percent lower risk of associated death. It was also associated with a 9 percent lower risk of hospital admission or death as a result of coronary heart disease after taking account of dietary factors. And among the 16 000 people whose inflammatory protein (CRP) level had been measured those eating the most chocolate seemed to have an 18 percent lower risk than those who ate the least. The highest chocolate intake was similarly associated with a 23 percent lower risk of stroke even after taking account of other potential risk factors. Of nine relevant studies included in the systematic review five studies each assessed coronary heart disease and stroke outcome and they found a significantly lower risk of both conditions associated with regular chocolate consumption. And it was linked to a 25 percent lower risk of any episode of cardiovascular disease and a 45 percent lower risk of associated death. This is an observational study so no definitive conclusions about cause and effect can be drawn. And the researchers point out that food frequency questionnaires do involve a certain amount of recall bias and underestimation of items eaten. Reverse causation--whereby those with a higher cardiovascular disease risk profile eat less chocolate and foods containing it than those who are healthier--may also help to explain the results they said. Cumulative evidence suggests that higher chocolate intake is associated with a lower risk of future cardiovascular events the researchers added. They also pointed out that as milk chocolate which is considered to be less healthy than dark chocolate was more frequently eaten by the EPIC-Norfolk participants the beneficial health effects may extend to this type of chocolate too. This may indicate that not only flavonoids but also other compounds possibly related to milk constituents such as calcium and fatty acids may provide an explanation for the observed association they suggested. There does not appear to be any evidence to say that chocolate should be avoided in those who are concerned about cardiovascular risk. (Source Heart-- June 2015) 68 NATURAL PRACTITIONER WWW.NATURALPRACTITIONERMAG.COM AUGUST 2015 Increased Anxiety Associated With Sitting Down L ow energy activities that involve sitting down are associated with an increased risk of anxiety according to research published in the open access journal BMC Public Health. Further understanding of these behaviors and how they may be linked to anxiety could help in developing strategies to deal with this mental health problem. Many studies have shown that sedentary behavior is associated with physical health problems like obesity heart disease type 2 diabetes and osteoporosis. However there has been little research into the link between sedentary behavior and mental health. This is the first systematic review to examine the relationship between anxiety and sedentary behavior. Anecdotally--we are seeing an increase in anxiety symptoms in our modern society which seems to parallel the increase in sedentary behavior. Thus we were interested to see whether these two factors were in fact linked said Megan Teychenne lead researcher and lecturer at Deakin University s Centre for Physical Activity and Nutrition Research (C-PAN) in Australia. Also since research has shown posi- tive associations between sedentary behavior and depressive symptoms this was another foundation for further investigating the link between sedentary behavior and anxiety symptoms. C-PAN researchers analyzed the results of nine studies that specifically examined the association between sedentary behavior and anxiety. The studies varied in what they classified as sedentary behavior from television viewing computer use to total sitting time which included sitting while watching television sitting while on transport and work-related sitting. Two of the studies included children adolescents while the remaining seven included adults. It was found in five of the nine studies that an increase in sedentary behavior was associated with an increased risk of anxiety. In four of the studies it was found that total sitting time was associated with increased risk of anxiety. The evidence about screen time (TV and computer use) was less strong but one study did find that 36 percent of high school students that had more than two hours of screen time were more like to experience anxiety compared to those who had less than two hours. The C-PAN team suggests the link between sedentary behavior and anxiety could be due to disturbances in sleep patterns social withdrawal theory and poor metabolic health. Social withdrawal theory proposes that prolonged sedentary behavior such as television viewing can lead to withdrawal from social relationships which has been linked to increased anxiety. As most of the studies included in this systematicreview were cross-sectional the researchers say more follow-up work studies are required to confirm whether or not anxiety is caused by sedentary behavior. It is important that we understand the behavioral factors that may be linked to anxiety--in order to be able to develop evidencebased strategies in preventing managing this illness Teychenne said. Our research showed that evidence is available to suggest a positive association between sitting time and anxiety symptoms--however the direction of this relationship still needs to be determined through longitudinal and interventional studies.
