The guidelines recommend a healthy eating pattern with limited sugar and saturated fat less salt and more vegetables and whole grains. But should we depend on the government for the most-up-to-date sound advice on healthy nutrition I contacted Jonny Bowden PhD CNS an author of 14 health books including his latest Smart Fat co-written with Dr. Steven Masley for his thoughts on the matter. Bowden has a PhD in nutrition is a member of the prestigious American Society for Nutrition and is a renowned expert on weight loss and wellness. It appeared I struck a nerve. The biggest problem to me is why we pay attention to them Bowden said. What people have to understand is that the USDA has two mandates 1. Deliver accurate science-based information to people and 2. Promote the agricultural interest of the U.S. Those two objectives are often at cross purposes. As for reducing sugar Bowden said considering the power of the sugar lobby he was surprised the recommendation to cut back was accomplished. I guess it s a step in the right direction he said noting that he believes all added sugars should be cut. The news that it s okay to eat more foods with cholesterol such as eggs is about 20 years behind the times he said adding that all saturated fats are still being demonized--the very topic Bowden tackles in his new book. J tional needs primarily through foods but also recognize dietary supplements as useful in providing one or more nutrients that otherwise may be consumed in less than recommended amounts or that are of particular concern for specific population groups. However given the number of under-consumed nutrients in our population dietary supplements are needed to fill in the nutrient gaps. The guidelines identified potassium dietary fiber choline magnesium calcium and vitamins A D E and C as shortfall nutrients that is nutrients that are consumed by many individuals in amounts below the Estimated Average Requirement or Adequate Intake levels. More specifically the guidelines identified calcium potassium dietary fiber and vitamin D as nutrients of public health concern because low intakes are associated with health concerns and suggested supplementing with vitamin D especially when sunshine exposure is limited due to climate or the use of sunscreen. On the show Hollewijn interviewed people involved in health healing and healthy lifestyles for three hours every Saturday. In April 2007 Gesundheit ... with Jacobus took to the health store industry and opened Gesundheit Nutrition Center in Bozeman MT. Gesundheit (which means good health in German) Nutrition Center had a consumer base pretty quickly as people recognized Jacobus and the talk shows continued. Sometimes the topics are about specific products or protocols that people might find at the store in the event that he has the educator for a specific brand on his show which often brings people in said his daughter Rebecca Hollewijn manager at Gesundheit Nutrition Center. But other times the topics have nothing to do with an associated product and it is nice to have that separation because the concept of the radio show was never an advertising platform but is truly about educating people helping them achieve better health and helping them to take their health into their own hands. Gesundheit has 10 employees who bring their own distinct knowledge and interests to health and healing. The store also has a clinical licensed nutritionist on staff who has her own practice and works three days a week with the store. Her knowledge is a huge asset and we allow customers to make appointments to see her to discuss health concerns in greater detail Rebecca Hollewijn said. GESUNDHEIT NUTRITION CENTER (BOZEMAN MT) you will see that sort of visual cohesion. The retailer sees about 95 customers each day and the number is steadily climbing since Gesundheit opened. In addition the average sale has seen an increase. Digestive products (enzymes probiotics etc.) account for about 9.9 percent of sales vitamins 8.8 percent herbs account for 8.1 percent and nutritional oils at 7.8 percent. The store separates the departments quite a bit so sales percentages are small. Other departments that store has include amino acids antioxidants athletics books brain sleep bulk cardio children and more. We have vastly more to offer that is condition-specific and or more geared to healing and maintaining health she continued. Other competitors though we don t really see them as competition because we all fill slightly different niches are the Community Food Co-op which has a wellness department with a primary focus on grocery and deli and Nutshell Naturals which focuses on supplements and a juice bar. While there is some overlap in the brands that the stores carry Hollewijn said there is a very cooperative approach between the stores. We regularly call them if we don t have something that a customer is looking for and the same is true for them. At the heart of it all is not a competitive spirit but a desire to help the customers who walk through the doors and if we can t do that then we re happy to refer them to other resources in the area she noted. J In-store Comfort The store features an octagonal seating area that was at the core of their spatial planning. The seating area wasn t the only place where the owners made careful decisions. When we were figuring out how to lay out the store it came down to a lot of problem-solving in terms of how much space each section needed what was logical in terms of the proximity of certain sections to others and what made sense visually Rebecca Hollewijn said. The shelves that are immediately visible to consumers contain primarily meal replacement powders (such as Vega s VegaOne and RAW Meal from Garden of Life) and protein powders. The reason for this Rebecca Hollewijn said is two fold. First the gondolas that are at the front of the store are larger than the two at the back in terms of their depth so the larger canisters look better and fill the shelves better than would a bunch of mismatched smaller bottles also because we have them arranged by brand there s a certain visual uniformity to the sections that I think is pleasing to the eye. Given the importance of this issue to the industry we believe additional time is needed to allow sufficient stakeholder response said Dan Fabricant PhD executive director and CEO of NPA. Defining natural is a major undertaking and NPA feels that no harm will result from FDA extending the comment period due to the interest significance and complexities surrounding the topic. NPA strongly supports and welcomes this effort by the FDA to define what can be labeled natural because millions of Americans are buying products they think might be natural but are really not said Fabricant. PBS Segment on Supplements Skewed and Misleading Says AHPA segment aired on January 19 on PBS Frontline presented dietary supplements as inherently unsafe by misrepresenting the rigorous safety provisions established for these products under the Federal Food Drug and Cosmetics Act the strict liability statute under which supplements are regulated. The segment titled Supplements and Safety relied on anecdotal reports of noncompliant products without informing viewers that these products are subject to aggressive enforcement by the U.S. Food and Drug Administration (FDA) and in fact implying the opposite American Herbal Products Association (AHPA) said. The program went on to represent a fraction of the products on the market which the FDA has the authority to remove as representative of the entire industry. A The regulated supplement industry is completely committed to selling safe products and to complying with the federal laws that ensure product safety and has a long history of adherence to current rules and advocating for strong enforcement of the rules said AHPA President Michael McGuffin. The content and tone of the Frontline segment presents a skewed portrayal of the supplement marketplace by ignoring the majority of supplement companies that ensure their products are safe and are properly manufactured and labeled and who often go beyond what is required by federal law in order to meet the needs of their most demanding customers. Contrary to Frontline s portrayal of dietary supplements these products are regulated by a host of laws and federal agencies all designed to ensure supplements are safe produced in a quality manner do not contain contaminants or impurities and are accurately labeled AHPA said. FDA Updates Webpages to Reduce Dietary Supplement Confusion T he U.S. Food and Drug Administration (FDA) recently updated language on a few pages of its website to more accurately describe illegal drug-spiked products and to remove references to them as dietary supplements. This comes after the American Herbal Products Association (AHPA) requested the changes AHPA said. The FDA usually uses terms such as marketed as dietary supplements or masquerading as dietary supplements when describing products found by the agency to contain undeclared drugs drug analogs or other illegal 18 VITAMIN RETAILER ingredients. This is in line with a March 2011 public statement by FDA s Deputy Director for Policy and Analysis Mike Levy who said These products are masquerading as dietary supplements-- they may look like dietary supplements but they are not legal dietary supplements. The recent changes made by FDA were to two webpages that were inconsistent with this standard language. For example the title on a Center for Drug Evaluation and Research (CDER) webpage was changed from Tainted Supplements CDER to Tainted Products Marketed as Dietary Supplements CDER. The page Medication Health Fraud was updated to replace the term Tainted Dietary Supplements with the more accurate Tainted Products Marketed as Dietary Supplements. This contributes to the misinterpretations of the new science by the consumer media covering those studies he continued. As a result consumers are misinformed about the safety of these products and misled to believe they are less safe than they actually are. Kingston spoke at the December 9 2015 Dietary Supplement Caucus briefing. Kingston gave examples of studies that have been misrepresented by the consumer media including the October 2015 study in the New England Journal of Medicine that projected more than 23 000 annual dietary supplementrelated emergency room visits. The study had many shortfalls including the R fact that not all emergency room visits are created equal in terms of level of severity or seriousness according to Kingston who observed that in almost 90 percent of the heart-related ER visits the patients were discharged from the ER and sent home. He also questioned the accuracy of the report because consumer health products that do not fall under the regulatory definition of dietary supplements were included in the researchers projections for dietary supplements and this was overlooked in the media coverage. If you re going to raise questions regarding the safety of an entire class of products it s important to know what actually is a dietary supplement and what s not said Kingston. The article gave examples of products contained in the analysis which included various drugs dimethyl sulfoxide topical herbal products and snake oil all of which were listed as miscellaneous complementary health products but are products and substances outside of the class of FDA (U.S. Food and Drug Administration) regulated dietary supplements. Kingston also pointed out that more than 20 percent of ER visits cited in the article involved unsupervised ingestion by children and although a concern supplement ingestions by children often require no treatment and result in no adverse effects despite the child being evaluated in an emergency department. He further noted that among older adults pill-induced swallowing problems led to more than one-third of the ER visits which is something that the industry can help evaluate and address. Another 10 percent of the overall visits were from excessive doses. Even more important Kingston said these ER visits must be considered in the context noting that more than 166 million Americans use dietary supplements so that s a pretty large denominator to consider when evaluating these numbers. Campbell Soup Disclosing Presence of GMOs in Its Products ampbell Soup Company (Camden NJ) announced its support for the enactment of federal legislation to establish a single mandatory labeling standard for foods derived from genetically modified organisms (GMOs). According to the company s press release Campbell believes it is necessary for the federal government to provide a national standard for labeling requirements to better inform consumers about this issue. The company said it will advocate for federal legislation that would require all foods and beverages regulated by the U.S. Food and Drug Administration C (FDA) and the U.S. Department of Agriculture (USDA) to be clearly and simply labeled for GMOs. Campbell is also supportive of a national standard for non-GMO claims made on food packaging. As a result of its decision to support mandatory national GMO labeling the company said it will withdraw from all efforts led by coalitions and groups opposing such measures. The company continues to oppose a patchwork of state-by-state labeling laws which it believes are incomplete impractical and create unnecessary confusion for consumers. Low levels of vitamin D in the blood are tied to an increased risk of developing MS. People who have MS and low levels of vitamin D are more likely to have greater disability and more disease activity. For the study 40 people with relapsing-remitting MS received either 10 400 international units or 800 international units of vitamin D3 supplements per day for six months. Patients with severe vitamin D deficiency were not included in the study. The current recommended daily allowance of vitamin D3 is 600 international units. Blood tests at the start of the study and again at three and six months measured the amount of vitamin D in the blood and the response in the immune system s T cells which play a key role in MS. While researchers are still determining the optimal level of vitamin D in the blood for people with MS a suggested range of 40 to 60 nanograms per milliliter (ng ml) has been proposed as a target. Participants taking the high dose of vitamin D reached levels within the proposed target whereas the group taking the low dose did not reach the target. Side effects from the vitamin supplements were minor and were not different between the people taking the high dose and the people taking the low dose. One person in each group relapsed. The people taking the high dose had a reduction in the percentage of inflammatory T cells related to MS severity specifically IL-17 CD4 and CD161 CD4 cells. When the increase in vitamin D levels in the blood over base line levels was greater than 18 ng ml every additional 5 ng ml increase in vitamin D led to a 1 percent decrease in the percentage of IL- 17 CD4 T cells in the blood. The people taking the low dose did not have any noticeable changes in the percentages of their T cell subsets. We hope that these changes in inflammatory T cell responses translate to a reduced severity of disease said study author Peter Calabresi MD director of the Johns Hopkins Multiple Sclerosis Center and professor neurology at the Johns Hopkins University School of Medicine. Inflammation Can Fan the Flames of Depression hronic inflammation in the bloodstream can fan the flames of depression much like throwing gasoline on a fire according to a new paper from researchers at Rice and Ohio State Universities. The study reviewed 200 existing papers on depression and inflammation. The authors found that in addition to being linked to numerous physical health issues including cancer and diabetes systemic inflammation is linked to mental health issues such as depression. Among patients suffering from clinical depression concentrations of two inflammatory markers CRP and IL-6 were elevated by up to 50 percent. According to Christopher Fagundes an assistant professor of psychology and co-author of the paper chronic inflammation is most common in individuals who have experienced stress in their lives including lower socio-economic status or those who experienced abuse or neglect as children. Other con- C tributing factors are a high-fat diet and high body mass index. Previous research shows that individuals who have socio-economic issues or had problems in their early lives are already at higher risk for mental issues because of these stresses in their lives Fagundes said. As a result they often experience a higher occurrence of chronic inflammation which we have linked to depression. He said that it is normal for humans to have an inflammatory response-- such as redness--to an area of the body that is injured. This is your immune system working to kill that pathogen which is a good thing Fagundes said. However many individuals exhibit persistent systemic inflammation which we re finding is really the root of all physical and mental diseases. Stress as well as poor diet and bad health behaviors enhances inflammation. Fagundes noted that a strong support system early in life is critical in helping individuals learn to deal with stress later in life. The study also found that depression caused by chronic inflammation is resistant to traditional therapy methods but can be treated with activities such as yoga meditation and exercise. GLA is a conditionally essential omega-6 fatty acid found in some plant seed oils such as borage evening primrose and hemp oils while EPA DHA are omega-3 fatty acids commonly found in fish krill and algal oils. Here I will address the value of these fatty acids and potential advantages of their joint use. G The Value of These Fatty Acids GLA s value is that in the body it can be converted into a prostaglandin known as PGE1 which has antiinflammatory antithrombotic (i.e. reducing blood clot formation) antiproliferative (i.e. inhibits growth of tumor cells) and lipid-lowering properties while also enhancing smooth muscle relaxation and vasodilation.1 The benefits of EPA DHA are too numerous to be discussed here in any detail but include reducing the risk of atherosclerosis increasing good HDL cholesterol decreasing high blood pressure reducing inflammation in such disorders as rheumatoid arthritis asthma colitis Crohn s disease and lupus--and reducing the symptoms of other disorders including angina migraine headaches psoriasis and tinnitus.2 While each of these fatty acids offer a range of benefits some overlapping there is good reason to consider concurrent supplementation with GLA and EPA DHA--especially when considering the problematic pathway of GLA. The Problematic Pathway of GLA The problem is that GLA s valuable properties may not end up being expressed for two reasons both of which have to do with metabolic conversion processes. First while GLA is not common in our food supply the body is capable of converting linoleic acid (LA)--one of the most commonly occurring essential fatty acids--into GLA. However the enzyme (delta-6desaturase) that converts LA into GLA is hindered by numerous factors including aging nutrient deficiency trans-fatty acids hydrogenated oils smoking to and excessive alcohol consumption and gastritis.3 In fact research has shown that individuals with gastritis had significantly lower GLA levels than individuals without gastritis.4 The second problem is that GLA has to be converted into dihomogamma-linolenic acid (DGLA) before it can then be converted into PGE1 and provide all of its valuable properties. The issue is that DGLA can then go down two different pathways. One of those is being converted to PGE1 which is good. The other is that DGLA can be converted into arachidonic acid (AA) which in turn can be converted into PGE2--an inflammatory VITAMIN RETAILER 61 FEBRUARY 2016 WWW.VITAMINRETAILER.COM SUPPLEMENTSCIENCE prostaglandin that has the opposite effect of PGE1. Another possibility is that it can be converted to both PGE1 and PGE2 in which case it will have beneficial and detrimental effects at the same time. tive means to help treat asthma. In a randomized prospective doubleblind placebo-controlled parallel group trial 8 patients with mild-to-moderate asthma were treated with 0.75 g GLA 0.5 g EPA (low dose) 1.13 g GLA 0.75 g EPA (high dose) or placebo daily. The results were that leukotriene synthesis decreased (p 0.05) and selfreported asthma status and bronchodilator use improved in subjects consuming low- and high-dose treatments between week 2 and week 4 (p 0.01). Similar studies in asthma patients also demonstrated that low and high dose treatments with these fatty acids reduced leukotriene synthesis9 and improved patient perceived quality of life and asthma management as evidenced by reduced asthma symptoms.10 acne received either omega-3 fatty acids as 2 g EPA and DHA borage oil containing 400 mg GLA or a control (placebo). The results were that omega-3 fatty acids and borage oil supplementation both significantly decreased inflammatory and non-inflammatory acne lesions. No severe adverse effect was reported. This study showed for the first time that omega-3 fatty acids and GLA were effective adjuvant treatments for acne patients. Solution to the Two Problems The first problem is easily solved by using a borage or evening primrose oil supplement which already provides GLA without the metabolic requirement of first converting it from LA. There is also a way to solve the second problem and mitigate the ultimate conversion of GLA to AA and subsequently to PGE2 use an omega-3 fatty acid rich fish oils at the same time as taking a GLA supplement. In a clinical study 5 42 healthy adults took 1.5 g day of GLA which increased plasma levels of AA--not a good thing. However 250 mg or 1 g of EPA included along with the GLA prevented the increase in plasma AA concentrations. In another clinical study 6 adult subjects took 3.0 g day of GLA and EPA. The results were that EPA levels were significantly increased as were DGLA levels--but AA levels did not increase. The researchers concluded This study revealed that a GLA and EPA supplement combination may be utilized to reduce the synthesis of pro-inflammatory AA metabolites and importantly not induce potentially harmful increases in serum AA levels. Similar results were seen in other research.7 In addition to mitigating the potential formation of inflammatory PGE2 there are other specific situations which may benefit from the concurrent use of GLA and EPA DHA. These include asthma metabolic syndrome diabetes acne cognitive development cystic fibrosis high triglycerides high blood pressure and osteoporosis. Cognitive Development In a randomized placebocontrolled trial 13 409 children (aged 3-13) were given a supplement with 750 mg DHA plus EPA and 60 mg GLA school day or a placebo for 20 school weeks. The results were that children using the supplement showed improvements in non-verbal cognitive development compared with the placebo with strongest effects in 7-12 year olds (p 0.008). The placebo group showed significant within-group improvements after switching to treatment (p 0.001). Metabolic Syndrome Diabetes Fifty-nine subjects with early-stage type-2 diabetes or metabolic syndrome (prediabetes) participated in an eight-week randomized singleblind parallel intervention study11 where they received either corn oil (CO) a GLA-rich borage oil with echium oil (BO) or an EPA DHA-rich fish oil (FO). The results were as follows Participants in the CO group showed no improvements whereas supplementation with BO significantly lowered total and LDL cholesterol levels while supplementation with FO reduced serum triglycerides hemoglobin A1c (a marker for long-term glucose control) and increased HDL-cholesterol. Researchers concluded that short-term dietary supplementation with BO and FO improved biomarkers associated with type-2 diabetes metabolic syndrome. Cystic Fibrosis Chronic inflammation plays a major role in lung deterioration in cystic fibrosis (CF) patients and anti-inflammatory strategies have beneficial effects. To study14 the changes seen after a one-year course of low-dose dietary supplements with a mixture of fatty acids in 17 adult patients with CF in chronic inflammation pulmonary status (lung function respiratory exacerbations and antibiotic consumption) quality of life and anthropometric parameters subjects received 324 mg of EPA 216 mg of DHA 480 mg of LA and 258 mg of GLA daily. Results were that at the end of the treatment period TNF alpha levels and other inflammatory markers fell significantly and spirometry (a breath measure test of lung function) improved significantly. Annual respiratory exacerbations and days of antibiotic treatment also fell significantly. Furthermore fat-free mass and hand grip dynamometry improved significantly. The result were that triglyceride levels were significantly lower on day 28 than on day 0 in the 4 g EPA DHA group the 4 g EPA DHA plus 1 g GLA group and the 4 g EPA DHA plus 2 g GLA group. LDL cholesterol decreased significantly (by 11.3 percent) in the 4 g EPA DHA plus 2 g GLA group. The researchers concluded that 4 g EPA DHA and 2 g GLA favorably altered blood lipid and fatty acid profiles in healthy women and was estimated to have a 43 percent reduction in the 10-year risk of myocardial infarction. Conclusion The concurrent use of GLA with EPA DHA is well advised to help reduce the potential conversion of GLA to AA and ultimately to the inflammatory prostaglandin PGE2. Encase the mattress in an allergen-proof plastic wash sheets blankets pillowcases and mattress pads every week in hot water with additive- and fragrance-free detergent and consider using bedding material made from Ventflex a special hypoallergenic synthetic. The next step in reducing the allergic threshold focuses on elimination of food allergies or sensitivities which can be a big factor in some people with asthma or eczema. An adverse reaction to a food may be immediate or delayed. Double-blind food challenges have shown that immediate-onset sensitivities are usually due to (in order of frequency) eggs fish shellfish nuts and peanuts. Enzymatically-modified isoquercitrin (EMIQ) is manufactured through a natural enzymatic modification that attaches glucose chains (glycosides) to the quercetin molecule to enhance bioavailability. Blood levels of quercetin are more than 40 times greater with EMIQ compared to an equal amount of quercetin. Hence 50 mg of EMIQ would be equal to approximately 1 000 mg of quercetin. Most clinical studies have used a dosage of 200 mg daily of EMIQ. That level would be equal to a whooping 8 000 mg of quercetin. In Japan research has shown EMIQ is useful in helping reduce the major symptoms of hay fever. In these double-blind placebo controlled studies subjects suffering from allergy to Japanese cedar pollen took two capsules daily of 100 mg EMIQ or a placebo for eight weeks during the pollen season. Throughout the course of the study EMIQ group experienced significantly less red itchy eyes. EMIQ appears best for hay fever symptoms especially the eyes. Wellmune is a special beta-glucan preparation from baker s yeast that has been shown to be effective in several double-blind studies in boosting immunity in preventing colds and the flu. It has also been shown to help in allergies. One placebocontrolled doubleblind study compared the effects of daily supplementation for four weeks with 250 mg Wellmune with a placebo in ragweed allergy sufferers. Compared with placebo Wellmune reduced total allergy symptoms (2 percent) symptom severity (52 percent) and symptom rating (37 percent). The Wellmune group also reported decreased sleep problems (53 percent) reduced nasal symptoms (59 percent) eye symptoms (57 percent) non-nasal symptoms (50 percent) and improved quality of life (56 percent). These results indicate Wellmune is good to support all allergic symptoms due to pollen allergy. Natural Products to Support Allergy Sufferers In addition to general dietary support there are a number of available natural products that can help deal with allergies. Digestive enzyme preparations were shown in studies performed in the 1930s and 40s as well as in a more recent study to be quite effective in dealing with food allergies. In the more recent study compared with no enzymes administration of digestive enzymes markedly reduced the severity of food-induced symptoms in all 10 patients including symptoms of allergic sinusitis (6) skin reactions (5) or asthma (2). Other protein digesting enzymes may be of benefit as well. The proteolytic (protein digesting) enzymes are most effective. Sources include pancreatin fungal preparations bromelain among others. Follow label instructions as for dosage. Best for food allergies and general allergy support. Flavonoid-rich extracts or individual flavonoids can be quite helpful. In particular two double-blind studies have shown apple polyphenols (AP) to reduce hay fever symptoms. The first study was conducted on patients with allergy to cedar pollen. Make sure to find your calorie allotment based on your sex and weight and stay under that daily. Be a Vegan Until Lunch--Cutting calories can lead to hunger.
