Another aspect is one that is too easily put on hold--exercise. Maybe it s because we ve heard that exercise is only a small part of weight loss that we don t give it the attention it deserves. It s easier to cut 1 000 calories from a bloated diet than to burn off 1 000 calories through exercise so why bother Last month the Cleveland Clinic which has been ranked the No. 1 hospital in the country for cardiology and cardiac surgery for 21 years in a row by US News & World Report stressed the role of exercise for heart health--even for those who have heart disease. To keep this important health information top-of-mind here is a discussion to start with your customers Exercise is one of the keys to preventing heart disease the No. 1 killer of both men and women in the United States but yet a Cleveland Clinic survey shows only 20 percent of Americans know how much exercise is recommended for a healthy heart. The survey also showed 40 percent of Americans are exercising less than that recommended amount--2.5 hours a week of moderate aerobic exercise. Most commonly people cited work obligations (41 percent) as the biggest factor preventing consumers from exercising more followed by being too tired (37 percent) and obligations with family and friends (28 percent). The survey found that men are considerably less likely than women to let things get in the way of their exercise routine. The survey also revealed large misconceptions when it comes to exercise and heart conditions. Only one-third of Americans knew that someone with heart disease needs to exercise the same amount as someone without heart disease. Additional survey findings include Weight the biggest motivating factor. According to those who exercise weekly 51 percent say that losing or maintaining weight is the No. 1 motivator. Only 32 percent of Americans say they exercise to benefit their hearts. Calories Americans don t know how much to burn. Despite most Americans exercising for weight purposes only 31 percent of those surveyed know they need to burn or cut out 500 calories from their diet to lose one pound a week. Cardiac rehabilitation benefits are known though it s underutilized. Most Americans (82 percent) realize that cardiac rehabilitation can reduce mortality rates from heart disease by about half. However typically less than a third of patients prescribed cardiac rehab complete it. Cardiac rehab is a medically supervised program designed to improve health by identifying and reducing risk factors that lead to cardiovascular disease. Testing Who needs it While it s true that those with heart disease should talk to their doctor about starting an exercise program most Americans don t understand which patients actually need stress tests. Eighty-one percent of those surveyed incorrectly thought that someone with high cholesterol needs an exercise stress test before starting an exercise program. Heart disease kills about one in four Americans but many of these deaths could be prevented by simple lifestyle changes like exercising and improving diet Nissen said. Americans know exercise is important but most don t realize just how far a little exercise can go--potentially reducing the risk of dying from heart disease by as much as 40 to 50 percent. It s worth making time for it. The leaves of the moringa tree are more nutrient dense than kale and provide a complete plant protein similar to quinoa. Moringa seems to be an emergence of an old trend from a different continent said Amber Elliot buyer manager at Elliott s Natural Foods Sacramento CA. Curtis agreed and noted that though it is new to the Western world moringa has been used for centuries by other cultures and countries. Curtis volunteered in the Peace Corps in Niger in West Africa. C East Brunswick NJ 08816 JanetP VRMmedia.com NPA Files Suit Against Board Members n February 4 in a Member Update email from the Natural Products Association (NPA) Executive Director and CEO Daniel Fabricant addressed membership informing them that action had been taken against the NPA. The email also announced that the NPA filed claims in the U.S. District Court for the District of Columbia against 10 current and former members of the Board of Directors of the NPA. NPA s court filing describes a pattern of conduct by these individuals that sought to undermine NPA s ability to do its work for their personal whims and nothing more. While no one likes litigation NPA needed to take this action for the association to protect itself Fabricant said in the email. In addition recent allegations made against NPA by the same former employee are false and they will ultimately fail as NPA has asserted in a separate court filing. NPA will continue to vigorously defend itself. In December Brent Weickert NPA s former CFO filed a lawsuit against the NPA and Fabricant. Represented by Kathleen M. Hartman Esq. of Callahan Thompson Sherman & Caudill LLP Irvine CA the suit listed complaints including wrongful termination in violation of public policy retaliation in violation of FEHA (Fair Employment and Housing Act) intentional infliction of emotional distress failure to pay vested vacation time failure to prevent discrimination harassment and retaliation in violation of FEHA religious and sexual harassment-hostile work environment in O violation of FEHA and more. Exhibits listed by the NPA included emails between Weickert and other members of the board credit card statements social media feeds of Weickert emails between Fabricant and Weickert and more. Weickert was employed by NPA from September 2 1997 until October 15 2015 as the senior vice president chief financial officer working out of an office in Paso Robles CA according to the summons. He alleged that Fabricant created a hostile work environment at NPA and misused funds. According to CEOUpdate.com Weickert wrote a letter to Board President Roxanne Green in May 2015. The letter advised NPA about multiple issues including that Fabricant s drinking had become excessive and that his abusive behavior had driven away multiple female employees. Green said in a statement We will vigorously defend the case and are confident we will prevail. We have the utmost confidence in Dr. Fabricant and his ability to continue leading the organization which he has helped to expand modernize and strengthen the natural products industry. Marc Ullman Esq. of counsel to Rivkin Radler LLP stated Now that I ve had the chance to look at the initial complaint and the counter-complaint I feel as though it is a lot of sour grapes from some board members who are used to running the ship when NPA had some weak leadership. I experienced firsthand the incompetence. NPA had become rudderless because of two very weak executives before him [Fabricant]. CEOUpdate.com reported on a motion to dismiss filed in January that NPA countered the filed charges noting that Weickert was an at-will employee who was properly terminated for several reasons Faced with a budget shortfall the executive committee determined it would be cheaper to outsource his job functions. Also Weickert working from his home in California was not necessarily efficient or prudent from a control and reporting standpoint the article detailed. The motion stated that Weickert never witnessed the harassment of female employees personally and therefore cannot seek damages for a hostile work environment. Even in the face of the structural challenges that existed prior to my arrival NPA is stronger than it has ever been Fabricant said in the member email. NPA has re-established itself as the most effective advocate for defending your business. Look no further than the re-introduction of the FSA HSA Bill our work on the recent Frontline story our numerous grassroots victories from last year and the plethora of comments we have submitted over the past 19 months. Other organizations may focus on hosting an event or show or holding a meeting to stroke egos. But at the end of the day when the industry s back is against the wall--whether in Congress state legislatures regulatory agencies attorneys general or in court--it has always been and will continue to be NPA that saves the day. AHPA Submits Comments to Improve Dietary Supplement Label Database T he American Herbal Products Association (AHPA) submitted recommendations to improve the federal government s Dietary Supplement Label Database (DSLD) to better inform supplement consumers. The DSLD is a joint project of the Office of Dietary Supplements (ODS) and the National Library of Medicine (NLM) of the National Institutes of Health (NIH) and provides images of supplement product labels and information about the listed products in numerous fields. AHPA continues to encourage members that market dietary supplement products under their own brands to submit labels for inclusion in the Dietary Supplement Label Database 14 VITAMIN RETAILER because so many AHPA members have a strong interest in the database functioning effectively to provide users with accurate labels and other information about dietary supplements said AHPA President Michael McGuffin. To better inform consumers AHPA recommends improvements be made to the accuracy of database entries and that the database be redesigned to provide only currently marketed labels. AHPA recommended several changes to improve numerous database functions including organization and content label sources foreign products and labels submitted by supplement marketers. Some of the recommendations included archiving labels for products that are no longer available for purchase in the U.S. removing any labels identified by the brand owner as erroneous DSLD only display actual product labels and to remove product obtained by other sources refrain from listing products not labeled for marketing in the U.S. and that the disclaimer on the DSLD website be modified to make it consistent with FDA s (U.S. Food and Drug Administration) Online Label Repository. This past fall Marketplace commissioned lab testing for samples of fish oil vitamin C and protein powder supplements to see if consumers are actually getting what they pay for. Based on those test results Marketplace reported that Emergen-C and two protein powders GNC Lean Shake 25 and Cytosport Muscle Milk did not match their label claims. However after re-testing the samples it was A found that the lab results and analysis provided to Marketplace were incorrect and that there is no evidence of problems with those products. The original lab tests were performed by an independent lab in Michigan which is ISO-17025 accredited registered with the U.S. Food and Drug Administration and used by the supplement industry. According to the CBC News story the lab was recommended by Neil Thanedar co-founder and CEO of LabDoor a company that has products tested and makes those results public to help guide consumers about vitamins and supplements. Thanedar agreed to analyze the test results for Marketplace and the supplement companies strong disputed the results. While the NPA (Natural Products Association) was the only association interviewed to defend the industry I was taken aback by some things stated by Dr. Paul Offit pediatrician and director of the Vaccine Education Center at the Children s Hospital of Philadelphia (CHoP). He mentioned that supplements are so awful that they can even harm you and increase your risk for heart disease and cancer. His vitriol continued as he said they have removed supplements from their own formulary. If you don t know what a formulary is it is a list of medications broken down by category that a hospital carries to treat their patient population. The list is continually revised and updated to ensure patients receive optimal health care with the best standards of practice available. Before a medication is added or deleted from the hospital formulary teams of doctors pharmacists and nurses evaluate these products to ensure that they are safe work well and are cost-effective. And hospitals don t add things to their formulary unless there is good evidence behind them. Hospitals and physicians graduated from their old practice of applying experience and judgement to applying evidencebased medicine and clinical trials on ingredients since the late 1980s. The issue I have with Offit s comments is whether he is being open and completely transparent about exactly how many supplements are on CHoP s formulary. I don t think the answer is zero. While a hospital s formulary is a closely held secret most hospitals will carry supplements because they are efficacious and doctors use some off-label as adjuvants with medications. Even Offit admitted that 35 supplements met his hospital s criteria (more on that point later). When survey respondents were queried about the factors leading them to carry the dietary supplements on their formularies 95 percent affirmed their selection by their pharmacy and therapeutics (P&T) committee process 94 percent chose safety data and 93 percent indicated clinical evidence of efficacy as a determining factor. multivitamins omega-3s probiotics and folic acid to name a few. A survey of National Association of Children s Hospitals and Related Institutions (NACHRI) hospitals in 2008 found that most formularies (99 percent) in pediatric hospitals include the basics of vitamins and minerals. While only 2 percent of NACHRI hospitals surveyed in the study carried herbal supplements nearly 40 percent carried other dietary supplements including fish oil probiotics melatonin chondroitin and glucosamine. When survey respondents were queried about the factors leading them to carry the dietary supplements on their formularies 95 percent affirmed their selection by their pharmacy and therapeutics (P&T) committee process 94 percent chose safety data and 93 percent indicated clinical evidence of efficacy as a determining factor. The other factors included availability of a standardized product (84 percent) physician demand (77 percent) cost (62 percent) proof of product quality (58 percent) patient demand (50 percent) and whether third-party reimbursement would cover the dietary supplement (25 percent). In fact dietary supplement use in hospital formularies is recognized by powerful organizations such as the American Society of Health-System Pharmacists (ASHP) and the Joint Commission. ASHP s Statement on the Use of Dietary Supplements is a valuable source of guidance regarding the review of dietary supplements for formulary inclusion. Dietary supplements must undergo a formulary review process at least as rigorous as that for prescription and nonprescription drugs. In other words the inclusion of any product in a health system s formulary dietary supplement or other is based on comparative data concerning efficacy safety cost and the potential for drug-dietary supplement interactions. The Joint Commission also looks at dietary supplements as drugs in their guidance for hospitals despite the fact that they are not regulated as drugs but rather a special category of food with far greater regulatory rigors than conventional foods. Joint Commission like ASHP evaluates supplements under the evidentiary rigors of drugs. In their Comprehensive Accreditation Manual for Hospitals Joint Commission defines a medication as any prescription medication sample medication herbal remedies vitamins nutraceuticals over-thecounter drugs vaccines diagnostic and contrast agents ... used on or administered to a person to diagnose treat or prevent disease or other abnormal conditions. Therefore the Joint Commission expects hospitals to analyze dietary supplement use in accordance with their claimed use and thorough documentation with evidence for that intended use. Hospital Standards Let s look at the standard of care in hospitals. Evidence also suggests that H2 blockers and protonpump inhibitors (PPIs) two of the most prescribed drugs on the planet can disrupt the microbial flora and lead to severe adverse effects lower down the gastrointestinal tract. In short they enable bacteria ingested with our food to survive the normally harsh pH environment of the stomach and allow pathogenic bacteria to colonize the intestinal tract. Probiotic supplements containing bifido and other beneficial bacteria are often prescribed in both inpatient and outpatient settings to help counter growth of bad bacteria. Supplements are neither a substitute for a good diet nor an alternative for medications. They are a food intended to supplement the diet to meet the nutritional needs and requirements of consumers. But let s face it most of us don t eat enough broccoli or consume enough fish to derive a sufficient benefit from eating healthy foods alone. Many of us myself included take a multivitamin or other supplement safely each day. Just like all health-related decisions anyone considering taking a dietary supplement should first consult with their health care provider. It is important that people know what they are putting into their bodies and there are laws for labeling requirements in place to ensure 30 VITAMIN RETAILER consumers get the information they need. One reason supplements are safe is because they are regulated by the FDA. FDA has the tools to enforce against unsafe products and they have. FDA can and should pull any product off the shelf it deems to be unsafe and the agency has done just that in some cases. Not only can the FDA pull a product that has already gone to market but the agency can also block (and has blocked) the movement to market of products it determines might be unsafe for the general public. The FDA may also enter into consent decree agreements with firms to stop the sale of products. The FDA has and continues to enjoin manufacturers shutting them down or seizing products that do not meet the current good manufacturing practices (cGMPs) the required quality standard all dietary supplement manufacturers must meet. These products are well made by responsible manufacturers and have and should play an important role in improving America s health nutrition and wellness. Are supplements regulated similarly to drugs The dietary supplement cGMPs were developed from FDA s drug GMPs so in some respects dietary supplements are regulated like drugs. Dietary supplements are also the only commodity of food for which mandatory serious adverse event reporting is a requirement. When you compare serious adverse events reported to the FDA for supplements prescription drugs and conventional foods supplements are by far safer than drugs and even conventional foods. So you can either listen to the beat of that old drum that supplements are an unsafe unregulated commodity or you can combat the tired rhetoric with the fact that 180 million Americans safely use dietary supplements on a daily basis and they are regulated by FDA. How many supplements does your local hospital carry on formulary Better yet how many supplements does Dr. Offit s hospital carry at CHoP The truth is that dietary supplements play an important role in the management of a variety of diseases as evidenced by their inclusion in numerous hospital formularies and the decision to include these products is not made lightly but is supported by scientific literature and a thorough safety review. The findings were published in the online issue of PLOS One. These results suggest that much of the burden of leukemia worldwide is due to the epidemic of vitamin D deficiency we are experiencing in winter in populations distant from the equator said Cedric Garland DrPH adjunct professor in the Department of Family Medicine and Public Health and member of Moores Cancer Center at UC San Diego Health. People who live in areas with low solar ultraviolet B exposure tend to have low levels of vitamin D metabolites in their blood Garland said. These low levels place them at high risk of certain cancers including leukemia. Few foods are natural sources of vita- E min D which is more abundantly produced when ultraviolet radiation from sunlight strikes the skin and triggers synthesis. According to the American Cancer Society 54 270 cases and 24 450 deaths from leukemia occur in the United States alone each year. There is no known way to prevent most types of leukemia though some types may be prevented by avoiding high doses of ionizing radiation exposure to the chemical benzene smoking and certain types of chemotherapy. The UC San Diego study analyzed age-adjusted incidence rates of leukemia in 172 countries from GLOBOCAN an international agency for research on cancer that is part of the World Health Organization comparing that information with cloud cover data from the International Satellite Cloud Climatology Project. The study follows similar investigations by Garland and colleagues of other cancers including breast colon pancreas bladder and multiple myeloma. In each study they found that reduced UVB radiation exposure and lower vitamin D levels were associated with higher risks of cancer. Leukemia rates were highest in countries relatively closer to the poles such as Australia New Zealand Chile Ireland Canada and the United States. They were lowest in countries closer to the equator such as Bolivia Samoa Madagascar and Nigeria. These studies do not necessarily provide final evidence said Garland but they have been helpful in the past in identifying associations that have helped minimize cancer risk. B12 Deficiency a Concern for Long-term Care high proportion of older adults entering long-term care homes in Ontario Canada are B12 deficient with more developing deficiencies over the course of their first year in residence according to research from the University of Waterloo. There is a connection between B12 deficiency and several serious health conditions. Researchers found that almost 14 percent of study participants were B12 deficient at the time of admission to a long-term care home while another 38 percent had only slightly better levels. Over the course of one year an additional 4 percent developed B12 deficiencies. However those receiving supplements had better B12 levels. The study conducted in partnership with the Schlegel-University of Waterloo Research Institute for Aging (RIA) and published in Applied Physiology Nutrition and Metabolism suggests a simple yearly blood test could be the key to elimi- A nating B12 deficiencies among elderly living in long-term care homes. In Ontario there is currently no systematic screening system to identify B12 deficiencies among long-term care residents--at the time of admission or afterward. The negative effects of a B12 deficiency are considerable said Heather Keller a professor of kinesiology at Waterloo and Schlegel Research Chair in Nutrition and Aging. This is of particular importance in the context of our aging population with more Canadians requiring long-term care. Untreated B12 deficiency will eventually lead to anemia and neurologic complications such as unsteady gait and paralysis. Low levels of the vitamin have been found to be associated with depression and dementia increased confusion lethargy and even osteoporosis. In spending time in long-term care homes you often see depression and loneliness. This is why we need to do everything in our power to enhance quality of life and quality of care in this setting said Kaylen Pfisterer lead author on the paper and assistant research coordinator at the RIA. Screening for B12 deficiency is a first step to targeting B12 treatment to those who may benefit most. A blood test would allow health care practitioners to screen for a B12 deficiency among new residents and if performed annually monitor vitamin levels throughout the resident s time in long-term care improving overall health and well-being. Then you finally tracked someone down and he or she couldn t answer your question. Did you buy from that store When I first started in the business more than 45 years ago consumers didn t have many options for purchasing dietary supplements. A local drugstore might stock a few multivitamins but for more than that consumers would have to find a health food store. But that s changed. You can find supplements on countless websites. Many big box stores also sell a variety of supplements. What sets successful health food stores apart is their ability to provide knowledgeable customer service that builds lasting relationships with H customers. And retailers can t do that effectively if they don t invest in educating their employees. Go Team Go Your team is your most valuable asset. Of course you need to hire good people but it doesn t end there. People starting out in retail don t automatically have good customer service and sales skills. Even those with years of experience may have inadequate training. Plus in our industry it s essential that your employees have wide-ranging knowledge on health and wellness as well as understand what they can and can t say about the dietary supplements you sell from a legal standpoint. That s a lot to learn--and a lot to teach. How do you even know where to start If you re lucky and you treat your employees right they might stick with you for a long time. But turnover is inevitable. Or maybe your happy prob- lem is that you re growing and you need to hire even more people. If you just wing it when it comes to training those new employees you re doing your business a disservice. You ll miss something important or create confusion among your staff because everyone has been taught a little differently. It s important to develop a standard training plan for new employees and put it into practice each time you hire someone new. Outline what needs to be covered and in what order. Give time estimates for the training so you ll know what to budget when you re managing shift schedules. Determine how the training will happen. Will you facilitate all the training Will some experienced people on your staff take some parts Will you use outside resources for this initial training Perhaps you use videos to teach certain training topics inventory management. At the same time if your team can t intelligently answer questions about dietary supplements while also staying within the bounds of structure function language you re putting your business at risk both financially and legally. Soft skills development is important for everyone on your team including yourself. These skills are essential for engaging your customers and giving them the best service possible. Soft skills training can encompass a wide variety of topics such as interpersonal skills and time management but for a retail store the most important are customer service and selling skills. You ll want to do some of this training yourself. For example you ll instruct your team on how you want your customers greeted when they come in the store. Providing additional training can pay big dividends. Training does cost money. The best employees devote some of their own free time to self-development but you can t require employees to train on their own time. Legally you have to pay them for training time. I won t get into all the labor laws here but be sure you know what s required in your state. Regardless consider the money you invest in your employees as money well spent. At the same time the training itself doesn t have to cost you a lot of cash. There are many opportunities for free and low-cost training. It just takes a little research to find what s out there. All you need for a lot of training is a computer with an internet connection and speakers or headphones. Webbased product and industry training is common and runs the gamut from short product-focused training that various manufacturers provide to weeklong events offered by trade-based groups well-known industry thought leaders or other organizations. Some are offered at specific times and dates while other trainings can be accessed on demand. You ll find plenty of free options but also consider quality educational offerings that charge fees for enrollment especially for your high-performing team members. You can also find a lot of free softskills training resources on the internet. A quick search will locate lots of options from articles and blog posts that you can use as the basis for a short discussion at a team meeting to professional videos posted on YouTube. Make sure you evaluate after web-based training to find out which sessions were high impact and which might have been a bit ho-hum. This way you can continue to refine your list of the best web offerings that are pertinent to your organization. You might decide to send some employees to outside training. Various organizations offer workshops throughout the country. Some are connected to trade shows some are stand-alone conferences or events while others may be offered locally through a college or other organization. Costs for these can vary. Your local chamber of commerce or small business association may also offer workshops or lunch and learns. And don t forget to tap into the resources available at the local tech school or university. Many of these are free or available at low cost. One more thing It s okay if some of your employees know more than you do. In fact you should be happy to have a staff that s so well educated. They ll be able to answer customer questions without having to rely on you. Plus they ll feel confident in the job they re doing and be more engaged in serving your customers. They are natural catalysts that accelerate activity and change without being changed themselves. They can do this over and over again. Enzymes are critical to all biological processes and are required for the body to function. Without enzymes there is no life. There are a wide variety of enzymes playing an active role in virtually every system in the body but there is not a single enzyme that does it all. Enzymes are very specific acting on what is called a substrate. Not all types of enzymes are located in all parts of the body. Areas become specialized according to need so that the enzymes in your mouth that start to break the bonds in starches to reduce them to sugars are different from the enzymes in your eyes designed to protect you from irritation and infection. Enzymes only work on a specific substrate. E In a delightful chemistry education website (chem4kids.com) the authors refer to enzymes as assembly line robots. There are many types of robots ones that lift the car chassis some attach doors others install a transmission. The authors explain that enzymes work in that manner. They grab some particle in the body do something to it release it when their action is complete and move on to the next particle to do the same thing all over again. And like assembly line robots their actions are not interchangeable. The tire robot cannot install a transmission and vice versa. Because these enzymes are so very specific if one part breaks they might not work. Some enzymes work alone and some need ongoing assistance. Enzyme helpers are called coenzymes--small organic molecules that can bind to an enzyme. You may be able to name one coenzyme--coenzyme Q10 which is a highly effective dietary supplement. However you may be surprised to learn that riboflavin thiamine and folic acid which you recognize as B vitamins are also coenzymes. Supplemental Enzymes Enzymes can be taken as supplements for a variety of health concerns. Some of the most clinically studied are the digestive enzymes made in the pancreas and released in a specific ratio as pancreatin. Pancreatin contains protease (digests proteins) amylase (digests carbohydrates) and lipase (digests fats). Enzymes from pancreatin are from an animal source usually porcine and are more similar to what occurs in the human body. There are also plant-based enzymes in which the protease lipase and amylase are extracted from a fungal source. The down side is that they are less like the enzymes in the human body. Plant-based enzymes are good for digestion as they exert their activity longer. Enzymes have an optimal pH range and the digestive tract transitions from a very acidic stomach to a very alkaline small intestine. Since plantbased enzymes can function in a wider range of pH they can actively digest food during a broader time span. Using enzymes for digestion can help with leaky gut (which is associated with autoimmune diseases) and even autism and autism-spectrum disorders. There are a surprising number of conditions associated with poor digestion and those are just a few. Animal-based enzymes also improve digestion. However many practitioners believe their best application is for inflammation and malignancies. The theory is that since they are more like human enzymes they increase systemic enzymes that are used by the inflammation and immune systems of the body. White blood cells use enzymes as tools to destroy abnormal cells and the resolution of the inflammatory process includes the removal of debris and restoration of healthy circulation using enzymes. Clinical studies have shown that they reduce inflammatory pain and stiffness. Enzymes and Allergies Research suggests that allergies can be triggered by the presence of partially undigested proteins. When the intestines are exposed to undigested proteins histamine and other inflammatory compounds are released. Allergies especially allergic rhinitis (hay fever) and allergic asthma are extremely prevalent. Unfortunately conventional medical treatment for allergies comes with its own drawbacks and nutraceutical options have been limited and largely lacking in efficacy. However a promising new nutraceutical treatment for allergies has emerged from Europe and is based on--of all things--a quail egg powder called AllerGuard Express. This article will provide a comprehensive overview of the research on this new treatment--but first let s take a brief look at allergy statistics and current medical treatment. I million adults were diagnosed with hay fever in the past 12 months.2 Likewise one in 12 people (about 25 million or 8 percent of the U.S. population) has asthma--that s up from one in 14 people only a few years earlier.3 Comparatively one in 10 children have asthma.4 Allergic disease including asthma is the fifth leading chronic disease in the U.S. in people of all ages. It is the third most common chronic disease in children under 18 years old.5 Current Medical Treatment Currently there are two types of medical treatment for allergies medication and immunotherapy. With regard to medications the American College of Allergy Asthma & Immunology says Decongestants and antihistamines are the most common allergy medications. They help to reduce a stuffy nose runny nose sneezing and itching. Other medications work by preventing the release of the chemicals that cause allergic reactions. According to the American College of Allergy Asthma & Immunology Immunotherapy involves giving gradually increasing doses of the substance or allergen to which the person is allergic. The incremental increases of the allergen cause the immune system to become less sensitive to the substance probably by causing production of a blocking antibody which reduces the symptoms of allergy when the substances is encountered in the future. This treatment is delivered via weekly injections (a negative for some people) or in oral form and is expensive. Medical insurance may pay for the injections but seldom pays for oral treatment. Furthermore it can take quite a long time to get relief from allergic symptoms with immunotherapy--typically a year or more. daily of 21 mg AllerGuard Express or a placebo. The study took place over a 22-month period. The results showed that AllerGuard Express was significantly better than placebo. In fact there was a 77.7 percent (p 0.001) reduction in the number of attacks at the end of treatment an improvement in respiratory function and a 58.6 percent reduction in the use of rescue medications by the AllerGuard Express group. Likewise a significant reduction in consumption of the rescue medication (p 0.0001) was also seen and these results took place at the peak of the pollen season and were already significant 45 days after the start of treatment. By the third month 80 percent of the treated patients treated no longer experienced any symptoms or unpleasant symptoms. An additional eight-month multicenter study11 examined the effect of AllerGuard Express (21 mg day) or placebo in 95 patients 48 of whom were adults and 47 of whom where children all with pollen-induced allergic rhinitis. As with the prior two studies results showed a statistically significant reduction in the intensity of allergic rhinitis in the AllerGuard Express group compared to the placebo group (p 0.0001) with an improvement in all the symptoms and a significant improvement in the condition of the nasal mucosa compared to the placebo group (p 0.0001). Once again there was a significant reduction in consumption of the rescue medication (p 0.0001) and once again these results were obtained at the peak of the pollen season. The doctor assessed the overall efficacy as good or very good in 68.7 percent of the patients treated with quail egg powder (p 0.05). Unlike in seasonal rhinitis the symptoms of perennial allergic rhinitis will usually be present throughout the entire year. Consequently a 10-month multi-center study examined the effect of AllerGuard Express (21 mg day) or placebo in 124 adult patients with perennial allergic rhinitis. In this study both the doctor and the patient assessed the results. The doctor s patient s assessment showed that the effect of treatment was significantly greater with AllerGuard Express (63.4 percent 65.7 percent) compared to the placebo group (27.3 percent 25 percent) (p 0.003) and that efficacy was seen after 28 days of treatment in terms of both nasal congestion and conjunctivitis runny nose sneezing and nasal congestion. Altogether there were 739 allergic WWW.VITAMINRETAILER.COM MARCH 2016 The Story of Quail Eggs Now let s shift gears and take a look at allergy treatment using AllerGuard Express. It all started in the early 1970s when a French general practitioner noticed that farmers who raised quails presented fewer allergy symptoms than the general population in the same area. One quail farmer saw the gradual disappearance of existing asthma and constant allergy-related shortness of breath in his spouse and then that of his employees with the consumption of quail eggs. Another quail farmer who experimented successfully with this treatment on his own family and friends confirmed this observation. The physician then gave raw quail eggs to his allergy patients including both adults and children and observed a reduction in their symptoms.7 Subsequently investigations in several human clinical trials were carried out by a larger group of physicians under the direction of a highly respected French allergist physician Dr. G. Bruttmann. In these studies subjects suffering from outdoor and indoor allergens were given AllerGuard Express tablets or placebo. In addition each of the studies included a good size population and was double-blinded. The results of these studies indicated that consumption of AllerGuard Express led to relief of subjects symptoms with good tolerability of the administered product. Following is a review of those studies. AllerGuard Express In the Treatment of Allergic Rhinitis In a multi-center study 9 a group of 160 adults with pollen-induced allergic rhinitis were treated with one tablet daily of 21 mg AllerGuard Express or a placebo. The study took place over an eight-month period. The results showed a statistically significant reduction in the intensity of allergic rhinitis in the treatment group compared to the placebo group (p 0.0001) with an improvement in all the symptoms considered. Furthermore there was a significant improvement in the condition of the nasal mucosa compared to the placebo group (p 0.0001) and a significant reduction in consumption of the rescue medication (p 0.0001). Moreover these results were obtained at the peak of the pollen season. By the third month 70 percent of the treated patients no longer experienced any unpleasant symptoms. Another eightmonth multi-center study10 examined the effect of AllerGuard Express (21 mg day) or placebo in 180 adult patients with pollen-induced allergic rhinitis. As with the prior study results showed a statistically significant reduction in the intensity of allergic rhinitis in the quail egg group compared to the placebo group (p 0.0001) improvement in all the symptoms and a significant improvement in the condition of the nasal mucosa compared to the placebo group (p 0.0001). Allerguard Express In the Treatment of Dust Mite-induced Allergic Asthma in Children In a multi-center study 8 a group of 180 children with dust mite-induced allergic asthma were treated with one tablet 84 VITAMIN RETAILER patients in these five studies. In each study there was a significant improvement in allergy symptoms among those patients using AllerGuard Express over those using the placebo. Combining the results of all five studies 72 percent of quail egg users experienced significant relieve from allergy symptoms while only 20 percent of those in the placebo group experienced similar relief. They then immediately took two tablets (42 mg of AllerGuard Express) or placebo both in the form of chewable tablets and were evaluated over a two-hour period. The results were that participants who AllerGuard Express were able to breathe significantly better through their nose (18 percent improvement p 0.05 vs. placebo) starting as soon as 15 minutes after allergen exposure and consumption of the tablets and also reported improvements in how they felt ranging from 25-44 percent for stuffy nose runny nose itchy nose watery eyes and itchy eyes during the same 15 minute time period. Over the entire two-hour post-challenge period participants who took the AllerGuard Express reported they felt significantly better with regard to stuffy nose runny nose itchy nose watery eyes and itchy eyes compared to how they felt after taking the placebo and all without any adverse events. against environmental triggers. In individuals with a sensitized immune system allergen-specific IgE antibodies are produced in response to environmental allergens.13-14 Worldwide sensitization (IgE antibodies) to allergies in the environment is present in up to 40 percent of the population.15 Certain outdoor and indoor antigens such as pollen mold animal dander and house dust mites contain protease enzymes. When they are inhaled and come into direct contact with the nasal cavity endothelium these protease enzymes can injure tissues and induce a transient IgE-mediated allergic inflammatory response.16 In-vitro testing has shown that protein fractions contained in the quail egg including ovomucoids and ovoinhibitors act as serine protease inhibitors.17-18 By inhibiting proteases AllerGuard Express bioactives help reduce the symptoms of an allergic reaction. Conclusion There is no single magic pharmaceutical or nutraceutical that will eliminate allergies. M URRAY ND W hat if some of the largest clinical studies assessing the benefits (or risks) of selenium supplementation in cancer prevention have used the wrong form That looks to be the case as a new study from the University of Miami sheds light on the different effects of selenium-rich yeast versus another popular form selenomethionine. What the researchers have discovered is that there are proteins produced when making selenium-rich yeast that have anti-cancer properties beyond simply providing a source of selenium. The Role and Importance of Selenium The trace mineral selenium functions primarily as a component of the antioxidant enzyme glutathione peroxidase. This valuable enzyme works with vitamin E and plays a pivotal role 86 VITAMIN RETAILER in preventing free radical damage to cell membranes. Not surprisingly low levels of selenium in the body have been linked to a higher risk for cancer cardiovascular disease inflammatory diseases and other conditions associated with increased free radical damage including premature aging and cataract formation. Low selenium levels are also associated with low immune function. Even in people with normal blood selenium levels selenium supplementation (200 mcg day) boosts immune function. In one double-blind study it resulted in a 118 percent increase in the ability of white blood cells to kill tumor cells and 82.3 percent increase in the activity of a type of white blood cell known as a natural killer cell. These white blood cells are given this name because of their powerful ability to kill cancer cells and microorganisms.1 The Health Benefits of High Selenium Yeast Go Beyond the Mineral Maintaining proper selenium levels appear to be important in protecting against many health conditions but some of the studies show some advantages to selenium-rich yeast that indicate there may be other compounds responsible for the observed effects in addition to selenium. For example in one double-blind placebo-controlled trial 69 healthy men were given selenium from a seleniumrich yeast known as SelenoExcell (200 or 285 g day) or selenomethionine (200 g day) for nine months. While blood selenium levels increased by 93 percent 54 percent and 86 percent in the selenomethionine and low- and high-dose SelenoExcell groups respectively only the men receiving the SelenoExcell demonWWW.VITAMINRETAILER.COM MARCH 2016 strated a decrease in oxidative stress. Levels of standard markers of oxidative damage 8-OHdG and 8-isoPGF2 were decreased 34 percent and 28 percent respectively versus no change in the selenomethionine group.2 These results indicate there are additional factors in the high-selenium yeast contributing to the antioxidant effects. These additional factors could explain the difference in the results seen in prostate cancer prevention. while selenomethionine (200 mcg per day) was shown to increase the prostate selenium levels by 22 percent SelenoExcell produced a 34 percent increase at the 200 mcg day dosage and a 92 percent increase at the 400 mcg per day dosage. It is thought that increased selenium uptake in the prostate after supplementation with SelenoExcell as compared to selenomethionine may a key factor in the different results seen in the NPC and SELECT studies but there are other considerations as well. In addition to acting as an antioxidant selenium-rich yeast may also offer protection against cancer by immune system effects detoxification of antagonistic metals inactivation of nuclear transcription factors that stimulate cancer growth and a host of effects on DNA and cellular replication. Recently researchers at the University of Miami evaluated the A Closer Look at Selenium and Prostate Cancer The results from studies looking at selenium in prostate cancer prevention have brought conflicting results. These may be related to the form of selenium being used and the ability of selenium to be incorporated into prostate cells. In the Nutritional Prevention of Cancer (NPC) study supplementation with SelenoExcell a selenium-enriched yeast was associated with a 52 percent decrease in prostate cancer incidence.3 In contrast in the SELECT study selenomethionine failed to show any protective effects alone or when combined with synthetic vitamin E.4 The difference in results may be entirely related to the form of selenium being used (see above in regards to antioxidant activity). One factor used to explain differing results is the effect of selenium supplementation on the concentration of selenium within prostate tissue. Selenomethionine has been shown to have only a modest effect in increasing prostate tissue selenium levels.5 In contrast results from a double-blind study in men with prostate cancer show high-selenium yeast is significantly more effective in increasing selenium levels in prostate tissue.6 In the study 53 men were randomized to receive placebo 200 mcg of SelenoExcell or 400 mcg of SelenoExcell for four to six weeks prior to surgery to remove their prostate gland. Results showed a clear dose effect in increasing the selenium content in prostatic tissue and provided a greater degree of selenium incorporation in prostate tissue compared to published results with selenomethionine. This action has profound effects as NF- B is a master regulator of pro-inflammatory reactions and gene expression. NF- B activation is a key factor in cancer cell growth and metastasis. By blocking NF- B the SGPs have an effect far beyond the action of simply providing a form of selenium. Comments If the science above is confusing let me try to restate what it is telling us. Basically it has long been assumed that selenium-rich yeast was beneficial because it provided a superior form of selenium. Once absorbed the selenium would be utilized as a valuable antioxidant as part of an antioxidant enzyme known as glutathione peroxidase. What the emerging science indicates is that the seleniumcontaining proteins that are produced in the process of making high-selenium yeast may turn out to be more important than the selenium itself.
