The authors offer an overview on the clinical trials and drug development effort regarding curcumin and state a number of conclusions and opinions they draw based on their personal interpretation of previous research. The article drew plenty of attention as curcumin products have become an important supplement for many people in the U.S. and around the world. As for myself I often reach for curcumin supplements for pain and inflammation and find the botanical to be very beneficial. As we so often see the media reacted to the study with only a superficial understanding of this scientific article which has led to confusion and misinformation. I contacted Ajay Goel PhD for some clarification. He is professor and the director of Translational Genomics and Oncology and the director of the Center for Gastrointestinal Research at the Baylor Scott & White Research Institute Baylor University Medical Center in Dallas TX. He is also one of the top scientists in the world investigating botanical interventions. Regarding claims Goel noted The authors state This miniperspective will not attempt to address the potential therapeutic effects of even more complex turmeric extracts or preparations thereof but instead focuses on the reported utility of the chemical structure of the major constituent of these extracts curcumin. Goel said that many people are confused by this and have taken the claims more generally to mean that all curcumin is ineffective. Goel also noted that some of the statements in the article are indeed true Curcumin is hard to absorb Curcumin is not very stable Curcumin is not very water soluble There are a multitude of curcumin raw materials and they can differ significantly. I That is why it is important to use a consistent high quality enhanced absorption curcumin preparation that has been proven in multiple human clinical trials Goel explained. Absorption safety and efficacy should be demonstrated in peer-reviewed journals. Goel also pointed out that some of the authors statements are questionable In-vitro results (meaning tests outside a living system) are not borne out in in-vivo (meaning living systems) results. No double-blinded placebo-controlled clinical trial of curcumin has been successful. This is puzzling indeed Goel said. The authors do not indicate if they mean synthetically isolated curcumin as a curcuminoid or full-spectrum natural curcumin. If they are referring to curcumin as a family of compounds which most people do it is not true. In the electronic database of the National Institutes of Health (NIH) available at Pub.Med.gov there are well over 9 000 published studies on curcumin and if you just look at human double-blind placebo controlled trials there are more than 80--the vast majority of which use enhanced absorption curcumin. One of the stated reasons for the article Goel said was to explore the likelihood of curcumin as a drug candidate with the initial conclusion being that curcumin is not a good candidate as there is not a straightforward way to create an analog for a specific therapeutic effect. It is my opinion that curcumin cannot be reduced to a few chemical structures that behave in a singular fashion he concluded. There is a vast array of physical targets that are touched by the natural compounds in curcumin that cannot be duplicated by a synthetic drug. My own scientific research and the research of other esteemed scientists around the world has confirmed that curcumin is a powerful and clinically documented intervention that has provided health benefits to hundreds of thousands of people around the world. With that I have no doubt curcumin s momentum will continue. Specifically researchers found that diets high in fiber trigger microorganisms in the gut to produce short chain fatty acids (SCFAs) which induce neutrophil apoptosis and the resolution of inflammation. These findings have important implications for the treatment of gout and possibly for the treatment of arthritis. By understanding the way foods interact with living organisms we may be able to create diets that help people with the disease as well as their health overall said Mauro M. Common Antioxidant May Guard Against Liver Disease common antioxidant found in human breast milk and foods such as kiwi fruit can protect against nonalcoholic fatty liver disease (NAFLD) in the offspring of obese mice according to researchers at the University of Colorado Anschutz Medical Campus. Pyrroloquinoline quinone or PQQ is a natural antioxidant found in soil and many foods and enriched in human breast milk said the study s lead author Karen Jonscher PhD an associate professor of anesthesiology and a physicist at CU Anschutz. When given to obese mouse mothers during pregnancy and lactation we found it protected their offspring from developing symptoms of liver fat and damage that leads to NAFLD in early adulthood. The study published online in the Journal of the Federation of American Societies for Experimental Biology is the first to demonstrate that PQQ can protect offspring of obese mothers from acceleration of obesity-induced liver disease. NAFLD is the most common liver disease in the world affecting 20 to 30 percent of all adults in the U.S. and more than 60 percent of those who are obese. It heightens the risk of cardiovascular disease type 2 diabetes and liver cancer. Scientists have found that mice fed a A high fat Western-style diet give birth to offspring with a higher chance of getting the disease. We know that infants born to mothers with obesity have a greater chance of developing NAFLD over their lifetime and in fact onethird of obese children under 18 may have undiagnosed fatty liver disease that when discovered is more likely to be advanced at the time of diagnosis Jonscher said. The goal of our study which we carried out using a mouse model of obese pregnancy was to determine whether a novel antioxidant given to mothers during pregnancy and breastfeeding could prevent the development of NAFLD in the offspring. Jonscher and her colleagues fed adult mice healthy diets or Western-style diets heavy on fat sugar and cholesterol. They gave a subset of both groups PQQ in their drinking water. Their offspring were kept on the diets for 20 weeks. Those fed a Western diet gained more weight than those on a healthy diet. PQQ did not change the weight gain but it did reduce the fat in the livers even before the mice were born. The antioxidant also reduced inflammation in the livers of mice fed the Western diet. The researchers found that PQQ protected adult mice from fatty liver even when it was stopped after three weeks when the mice quit breastfeeding. Jonscher said she believes the antioxidant may work by impacting pathways critical to the early onset of diseases associated with maternal obesity high fat diets and inflammation. She added that it could possibly be used as a prenatal or lactation supplement to protect children of obese mothers from developing liver and cardiovascular disease in adulthood but cautioned that pregnant women should always consult their doctor before taking any supplement. No. 10--Should you advertise at all This is the first question you have to ask yourself and let me tell you without a moment s hesitation--the answer is a resounding YES Advertising when done correctly is very effective and a necessity to your bottom-line. Stores can fall into a false sense of security about advertising when things are going well thinking that s the time they don t need to advertise. Nothing could be further from the truth. At one point in the history of my health food store we were so happy with how sales were going that we decided we didn t need to advertise. In the meantime a competitor began to advertise heavily and they beat the pants off us Look to spend 5 to 7 percent of your sales on advertising. Even if you feel you M don t have the funds to advertise rethink that approach and work your budget to include advertising. It will benefit you in the long run. No. 9--Test test test. There s a famous advertising quote often attributed to Henry Ford which states that he knew half of his advertising budget was wasted he just didn t know which half Perhaps you ve found that to be true as well. That s why it pays to test your advertising. Try different methods to see what message works better. See which medium works best. (More on that in tip 3.) Spread your advertising budget around and experiment. You can t build a church with one brick you have to use many bricks. Use the various forms of advertising to tell your story brick by brick. Keep experimenting and testing. Even if something is good it can always be better. No. 8--The message. While you are testing your ads pay attention to the content and message you are spreading. Give people a reason to come into your store. Just stating that you are Bill s Health Food Store for example is not compelling. We were extremely successful with a highly effective pain-relieving product. Pain is a huge issue and initially people didn t think of going to a health food store to get something to deal with pain. We talked about this product s ability to relieve pain on a variety of mediums including TV radio print and billboards. It was an overnight success. We gave them a distinct reason to check out our health food store. Invite people to come in for a sample. Advertise a product that you know offers quick benefits. Give them a reason to come and see you. No. 7--Couponing( ) While consumers seem to like coupons I don t think retailers agree. They were very ineffective in our store. It s a time-waster to take coupons in and manage the whole process--it s simply not cost-effective. I m in favor of retailers controlling their own destiny and discounting. Just remember that what goes online stays online forever so be a smart store communicator Be wise with your words and pictures. Social media can be a blessing or a curse something that has become very evident with the advent of fake news. People can also spread fake news about supplements or their experience in taking supplements so be careful. No. 5--Blogging. Closely tied to social media is the ability to have your own blog. It s free and a great place to show your personality and educate customers about healthy living. If you enjoy writing or have someone on your staff that does it s a wonderful way to engage the reader about your store and store philosophy. No. 4--Traps. Advertising is a business filled with many wonderful people with wonderful ideas. That being said you have to be a wise consumer. I would not recommend signing contracts. Don t put all your eggs in one basket. Do tests of small campaigns--two to three months at a time. Switch your ads and switch the medium you are using if you find it s not working or producing results. You may receive very good advice from an advertising professional but be aware that they have their own interest in mind in their recommendations. Working with an outside consultant especially if you don t have much advertising experience can be a wise way to go. Just don t sign long-term commitments. Remain fluid and able to make changes and decisions of the outcomes you observe. Stay in control of your choices without being tied to a long-term contract. No. 3--The Best Medium. You have so many media choices between TV radio newspaper and billboards it can be hard 26 VITAMIN RETAILER to know exactly where to land. If I had to pick just one medium I would choose TV. It can be reasonably inexpensive and it utilizes multiple senses to communicate. People are hearing and seeing at the same time. Gluten refers to proteins found in wheat barley rye and any hybridized strain created from at least one of those cereals. So what is the controversy around oats Oats are considered a high-risk ingredient in glutenfree products because of almost universal contamination between barley and oats in the growing fields and the additional risk of contamination during harvest and processing. The mechanical separation of these crosscontaminated grains is also difficult since the barley oat and wheat seeds are so similar in size. Despite this the U.S. Food and Drug Administration (FDA) rule on gluten-free claims still excludes oats from the definition of gluten and does not mandate any other preventative measures for a finished food product other than it must contain less than 20 ppm of gluten to bear a gluten-free label. Health Canada on the other hand prohibits the addition of regular commodity oats in all gluten-free products sold in Canada. However it will allow gluten-free oats providing there is sufficient evidence that they have been subjected to special processes to ensure they are safe and that the final product does not contain more than 20 ppm of gluten. To protect North American consumers from the inadvertent contamination of gluten via oats the GlutenFree Certification Program (GFCP) has also adopted the Canadian glutenfree regulatory requirements as a basis for recognizing and sanctioning the best practices which manufacturers need to implement and validate to achieve 20 ppm of gluten in a final product. The GFCP requires that all incoming ingredients entering a manufacturing facility with a gluten-free management system in place be verifiably gluten-free. As oats are considered a high-risk gluten contaminant facilities should clearly set their specifications such that glutenfree oats have been grown transported stored prepared and managed in a manner that avoids crosscontamination by wheat barley rye or their hybridized strains to consistently achieve the desired food safety outcome of 20 ppm for gluten. Suppliers of gluten-free oats should have documented standard operating procedures (SOPs) in place which can be assessed by the facility to verify that the supplier conforms to those specifications. These approaches start with a higher risk profile so the challenges are in gaining assurance with a higher risk product through additional sampling and testing. The purity protocol specifies how oats can be grown and handled to prevent any cross-contact with gluten-containing grains. This includes dedicated growers dedicated storage facilities and controlled transportation including a trace-back and verification process back to the grower. The prevention approach requires additional diligence however and has been proven to produce a higher level of safety in gluten-free oats. The GFCP recognizes intervention on-farm as an efficient and effective way to start clean for the appropriate management of gluten during manufacturing to stay clean. Sourcing gluten-free oats from a supplier who can demonstrate a purity protocol has reduced risk advantages as documented due diligence at the supplier level can be plugged directly into a gluten-free management system as evidence at the manufacturing level. Less due diligence at the supplier level results in a greater risk and thus burden on the manufacturer to provide proof of conformity (i.e. visual inspection testing rejection of the ingredient or finished products before release to the public etc.) Regardless any manufacturer using gluten-free oats under the GFCP is subjected to a rigorous risk evaluation of their gluten management system to demonstrate that ability to manage incoming gluten-free oats consistently meets 20 ppm of gluten food as the standard for a safety outcome. One thing is certain how to consistently achieve gluten-free oats has forced industry governments and consumer groups to investigate closer. The GFCP accomplishes this strictly on an individual case by case basis using science to seek out the facts and provide documented proof to ensure that the requirements of both the GFCP and the regulators are met. For more than three decades I have raced more than 300 endurance events including more than 150 triathlons of Olympic distance or longer 103 marathons and numerous other events including ultramarathons and rough water swims from Alcatraz to the mainland. Through it all I have experienced the benefits of a wide range of innovative sports nutrition products. So when I hear of a compelling new sports nutrition ingredient I am always very curious. In Indian and Asian cultures turmeric and curcumin have a long history of use as a traditional herbal medicine. Western medicine is beginning to study the potential of turmeric in treating diseases such as arthritis cancer and diabetes. However some of the most fascinating curcumin research also dovetails with what I experience so much of muscle soreness. A new double-blind randomized placebo controlled study published in the European Journal of WWW.VITAMINRETAILER.COM MARCH 2017 Applied Physiology in March 2015 indicates curcumin can help reduce pain associated with delayed onset muscle soreness (DOMS) and aid muscle recovery in physically active people. The study examined whether curcumin had an impact on single-leg jump performance and DOMS following unaccustomed heavy exercise. Researchers gave 17 male volunteers 2.5 g of oral curcumin (or a placebo) twice daily two days before to three days after strenuous single-leg press exercise separated by 14 days of rest. During this trial period the researchers monitored post-exercise limb pain muscle swelling and other indicators of muscle damage and inflammation. Data was collected at baseline immediately after exercises one day after exercises and two days after exercises. The results indicated that aside from inducing moderate to large reductions in pain caused by single-leg squats gluteal stretches and squat jumps the curcumin supplements also caused small reductions in creatine kinase activity (increased amounts of creatine kinase in the bloodstream is associated with muscle damage). The researchers even found that curcumin improved the volunteers single-leg jump performances due to its anti-inflammatory benefits. Further research is needed but the results are compelling nonetheless. W bacteriocin-like inhibitory substances (BLIS) were shown to be significantly less likely to acquire S. pyogenes. Throat Health Supplementation With Streptococcus Salivarius K12 Strain (BLIS K12) In a controlled 90-day study 6 researchers tested the Streptococcus salivarius K12 strain (hereafter referred to as BLIS K12) for its potential effect in preventing streptococcal pharyngitis and or tonsillitis in 40 adults with a diagnosis of recurrent oral streptococcal pharyngitis. Twenty of these subjects took BLIS K12 daily in a tablet formulated to slowly dissolve in the oral cavity while the other 20 subjects served as untreated controls. In addition a sixmonth follow-up was included (without supplementation) to evaluate the potential for any long-term protective role that BLIS K12 might offer. Results showed that those using the BLIS K12 had about an 80 percent reduction in their episodes of streptococcal pharyngeal infection compared to the prior Bacterial Activity in the Oral Cavity The bacterium Streptococcus pyogenes is the cause of some throat infections including pharyngitis or strep throat. In human research 3-5 subjects possessing naturally higher levels of certain strains of the oral bacterium Streptococcus salivarius which provide 74 VITAMIN RETAILER year s incidence. The 90-day treatment was also associated with an approximately 60 percent reduction in the incidence of reported pharyngitis in the sixmonth period following use of the product. The product was well tolerated and there were no treatment-related side effects. In addition to adults children also benefit from BLIS K12 supplementation. In fact five studies in children have shown benefits for throat health. Eighty-two children including 65 with and 17 without a recent diagnosis of recurrent oral streptococcal infection were enrolled in a similar controlled 90-day study.7 Of those with recurrent infection 45 were treated with BLIS K12 daily in a slowly dissolving tablet and the remaining 20 served as an untreated control group. The 17 children without a recent infection were used as an additional control group. The result was that the 41 children using BLIS K12 showed about a 90 percent reduction in their episodes of streptococcal pharyngeal. In the six-month follow-up the incidence of pharyngeal infections was reduced by about 65 percent. Again the product was well tolerated with no side effects. In a placebo-controlled study 8 24 children with a history of frequent sore throats from tonsillitis were recruited into the study and treated with one billion CFU of BLIS K12 daily or a placebo. Results showed that the children receiving BLIS K12 experienced substantially fewer sore throats (0.11 per month) than they did before entering the study (0.33 per month) and also in comparison to the smaller placebo group (0.19 per month). Another controlled study9 was conducted with 61 children who had a diagnosis of recurrent oral streptococcal disorders. Thirty-one of them were treated daily for 90 days with a slowly dissolving BLIS K12 tablet. The remaining 30 children served as the untreated control group. Twenty children (10 per group) were also assessed for viral infection. Results showed that the 30 children who completed the trial with BLIS K12 experienced a significant 90 percent reduction in their episodes of streptococcal pharyngeal infection as compared to the prior year s episodes. No difference was observed in the control group. The BLIS K12 group also showed a significant 80 percent decrease in the incidence of oral viral infections. Again there was no difference in the control group. In addition children treated with BLIS K12 needed less antibiotic therapy less medication for reducing fevers and missed fewer days of school because of related illness. Seventy-six of these children then undertook a 90-day program requiring once-a-day dosing a slow-dissolving BLIS K12 lozenge. No probiotic supplement was given to the remaining 54 children who served as controls. Results showed that even nine months after the use of BLIS K12 had been stopped the probability of new GABHS infections was significantly lower (P 0.001) when compared to the period before dosing commenced. When compared to the untreated children those taking BLIS K12 appear to have had significantly fewer GABHS infections both during the 90-day period of prophylaxis and during the following nine months (P 0.001). A very recent published study11 focused on a further evaluation of the role of BLIS K12 in the control of pediatric streptococcal disease--and whether its use could also provide protection against various nonstreptococcal infections. A total of 48 children with a recent history of recurrent pharyngeal streptococcal disease received BLIS K12 daily for 90 days while 76 children with a very low recent occurrence of oral streptococcal disease served as the control group. Results showed that those children receiving the BLIS KL12 experienced a 90 percent reduction of streptococcal pharyngeal disease compared to the prior year. They also experienced a statistically significant reduction in tracheitis (bacterial infection of the trachea) viral pharyngitis rhinitis flu laryngitis acute otitis media enteritis and stomatitis (inflammation of the mucous membrane of the mouth e.g. canker sores) compared with untreated children. exactly what was shown in the next study. In a different study13 of 82 children with recurrent oral streptococcal infection the researchers also examined the effects of BLIS K12 supplementation on otitis media (middle ear infection). The results were that after 90 days children using BLIS K12 had about a 40 percent reduction in acute otitis media). In the six-month follow-up during which there was no further supplementation the incidence of ear infections was reduced by about 65 percent. In another study 14 children who were prone to recurrent otitis media were supplemented with one billion CFU daily of BLIS K12 or placebo to examine the effect on the rate of infection recurrence. The children receiving BLIS K12 had far fewer ear infections (0.22 per month) than they did prior to entering the study (0.50 per month) and also in comparison to the placebo group (0.55 occurrences per month). The results of this study indicate that the occurrence of otitis media has been reduced for children in the active treatment group. Halitosis Treatment Supplementation With BLIS K12 Halitosis commonly referred to as bad breath is a noticeably unpleasant odor on the exhaled breath. Test tube research15 has shown that BLIS K12 is effective against several bacteria involved in causing halitosis. This led researchers to speculate if supplementation with BLIS K12 would be beneficial against halitosis in people as well-- and it turned out that it indeed did. In a two-week placebo-controlled study 16 BLIS K12 or placebo was given to 23 subjects with halitosis following treatment with an antimicrobial mouthwash. The subjects undertook a threeday regimen of mouth rinsing with the antimicrobial mouthwash followed at intervals by the use of a lozenge containing either BLIS K12 or placebo. Assessment of the subjects volatile sulphur compound (i.e. rotten egg smell) levels one week after treatment showed that 85 percent of the BLIS K12-treated group and 30 percent of the placebo group (mouthwash only) had substantial reductions. Testing showed that BLIS K12 suppressed the growth of black-pigmented bacteria in saliva samples and also in various strains of bacteria implicated in halitosis. I use traditional Chinese veterinary medicine theory to heal disease with whole foods. Because I am so focused on the pet food industry I have become very familiar with the shortfalls in nutrition for the majority of the dogs and cats in America. One of those shortfalls is the overuse of grains and vegetable proteins in pet food as they are a much cheaper source of protein than meat. However the carbohydrateladen diets contribute to dental disease (and many other problems like chronic inflammatory diseases including obesity diabetes arthritis and endocrine system disorders). Only 20 percent of pet owners brush their pet s teeth yet 80 percent of pets have dental disease by age 3. Their pets commonly take the place of the children that are now out on their own. Baby Boomers may have a bit more discretionary income and they don t mind spending it to keep their pet family members healthy. Millennials are putting off marriage and families a bit longer and their pets commonly become their children. Millennials are looking to eat better and live cleaner lives partly because they care more about the planet and what they will be leaving as their legacy. Because they want to eat organic whole foods they want to feed their pets the same way. They are looking for labels that say organic natural holistic and healthy. They shy away from dyes chemicals and preservatives. They are also the Internet generation and they know how to do the research to find out if products really walk the walk or just talk the talk.
