He is also in favor of modernizing the agency. Gottlieb who has a strong medical background but not as much on the food side was sworn in as the 23rd commissioner of FDA on May 11 (see news item on page 14). According to the FDA website he is a physician medical policy expert and public health advocate who previously served as the FDA s deputy commissioner for medical and scientific affairs and before that he was a senior advisor to the FDA commissioner. Gottlieb also worked on implementation of the Medicare drug benefit as a senior advisor to the administrator of the Centers for Medicare and Medicaid Services. In 2013 he was appointed by the Senate to serve on the Federal Health Information Technology Policy Committee which advises the Dept. of Health and Human Services on health care information technology. T New Focus According to Bloomberg News workers in the FDA s Office of Regulatory Affairs will begin focusing on overseeing specific product areas--such as pharmaceuticals or medical devices-- rather than activities in their U.S. geographic regions Gottlieb said in a memo. The staff of 4 000 investigates consumer complaints oversees imported goods and inspects domestic and for- eign facilities to ensure compliance with agency standards. The initiative was proposed four years ago with implementation set to start this month. The refocusing will make our field programs more modern and responsive to today s threats and challenges while making sure that we are taking a riskbased and science-based approach to our work Gottlieb said in the memo. We need to make sure that we are achieving the greatest degree of consumer protection with the resources that we have. The Natural Products Association (NPA) supports the FDA s plans to reorganize its inspection staff and applauded Gottlieb for his efforts to modernize the agency. The most effective way to oversee the supplement industry is to enforce the current law and aggressively pursue criminals that break it said Dan Fabricant PhD president and CEO of NPA. We applaud Dr. Gottlieb s commitment to modernizing the FDA and making it more responsive to the consumers and manufacturers of dietary supplements and natural products. The Council for Responsible Nutrition (CRN) is also in favor of the change of direction. Steve Mister CRN s president and CEO stated CRN commends FDA Commissioner Scott Gottlieb and the agency for announcing the implementation of its Program Alignment a major initiative that will help strengthen FDA s ability to protect public health. With this appointment the natural products industry has breathed a sigh of relief especially since new dietary ingredient (NDI) rules are currently on the line and also with the hope that the new alignment will strengthen enforcement of the laws we already have. C East Brunswick NJ 08816 JanetP VRMmedia.com FDA Takes Action Against U.S.-based Companies Said to be Selling Fraudulent Cancer Treatments n April 25 the U.S. Food and Drug Administration (FDA) posted warning letters addressed to 14 U.S.-based companies including South Carolinabased Everything Herbs and Californiabased Nature s Treasures among others said to be illegally selling more than 65 products that fraudulently claim to prevent diagnose treat or cure cancer. The products are marketed and sold without FDA approval most commonly on websites and social media platforms. Consumers should not use these or similar unproven products because they may be unsafe and could prevent a person from seeking an appropriate and potentially life-saving cancer diagnosis or treatment said Douglas W. Stearn director of the Office of Enforcement and O Import Operations in the FDA s Office of Regulatory Affairs. We encourage people to remain vigilant whether online or in a store and avoid purchasing products marketed to treat cancer without any proof they will work. Patients should consult a health care professional about proper prevention diagnosis and treatment of cancer. It is a violation of the Federal Food Drug and Cosmetic Act to market and sell products that claim to prevent diagnose treat mitigate or cure diseases without first demonstrating to the FDA that they are safe and effective for their labeled uses. The illegally sold products cited in the warning letters include a variety of product types such as pills topical creams ointments oils drops syrups teas and diagnostics (such as thermography devices). They include products marketed for use by humans or pets that make illegal unproven claims regarding preventing reversing or curing cancer killing inhibiting cancer cells or tumors or other similar anti-cancer claims. The FDA has requested responses from the 14 companies stating how the violations will be corrected. Jack Challem The Nutrition Reporter Dies at 66 n April 13 2017 Jack Challem bestselling author and beloved figure in the natural products industry passed way after a battle with pancreatic cancer at the age of 66. Born May 29 1950 he will be remembered for many JACK CHALLEM contributions including ushering in science-based nutrition reporting in the natural products industry. Armed with a B.A. and graduate work in sociology and psychology from Northeastern Illinois University some of his earliest science reporting work was for the Los Alamos National Laboratory near Santa Fe NM throughout the 1980s. According to the Nutrition Business Journal sales of supplements grew 6.2 percent in 2016 (compared to 5.9 percent in 2015 and 5.1 percent in 2014) to reach the 39 billion mark. U.S. natural and organic food channel saw sales grow 10.7 percent to 67.2 billion in 2015. The entire natural products industry including food beverages supplements and personal care products (i.e. cosmetics) grew nearly 10 percent to 180 billion in 2015. NPA Asks Congress to Support Access to Vitamins for Low-income Families T he Natural Products Association (NPA) recently commended the U.S. House for urging the Institute of Medicine (IOM) to investigate expanding access to vitamins for low-income mothers and children. The provision included in the House Agriculture Committee s FY 2017 appropriations bill report comes after a yearlong grassroots and advocacy effort spearheaded by NPA. If school cafeterias are allowed to serve things like French fries and pizza we think it is reasonable to find a way for mothers and fathers to purchase items that can support their children s health said Dan Fabricant PhD president and CEO of NPA. Expanding WIC (Women Infants and Children) to cover supplements would mark a big win for low-income families and we commend the House for taking this first step in pushing such a common-sense policy stance. NPA s grassroots network renewed its push this year to expand access to dietary supplements to all Americans ensuring this crucial provision to start the process to help low-income families was included in the 14 VITAMIN RETAILER House appropriations bill added Fabricant. As an industry we still have some work to do but we are truly thrilled at starting this important discussion to get it across the goal line we need everyone in the industry to write congress at www.saveoursupplements.org and tell them that we want to see expansion of WIC to include multivitamins. Since 1978 the WIC Supplemental Food Program has served low income at risk pregnant breastfeeding and postpartum women infants and children up to age 5. The WIC program provides federal grants to states for supplemental foods health care referrals and nutrition education. The program has grown significantly in its nearly 40 years of existence and it now serves approximately eight million participants. According to a study published in the Maternal and Child Health Journal children without access to proper nutrition are more likely to develop learning difficulties in the first two years of life. The inclusion of vitamins in the WIC program will help combat that issue specifically for lowincome women and children. Vitamins are proven to have many health benefits that are especially relevant to those individuals the WIC program intends to help. They can Prevent classic nutrient deficiency diseases (scurvy pellagra and iron deficiency anemia) Improve appetite and growth rates in low-income children Prevent neural tube birth defects Protect against heart disease and stroke and Build bone mass in the young. NPA will continue to work with Congress to define the parameters for the inclusion of multivitamins in the WIC program and in the Food Stamp SNAP program which will likely be addressed in the 2018 farm bill to ensure the maximum number of families access their benefits. Serving as a resource for regulators retailers and industry the OWL is an industry-wide self-regulatory initiative that will help create a rich and more complete picture of the marketplace and can be accessed by anyone using the internet. According to Steve Mister president and CEO Council for Responsible Nutrition (CRN) Last month during Expo West it was heartening to witness the high level of awareness about the Supplement OWL and exhilarating to be surrounded by the industry buzz. This has been a unique opportunity for our industry working together on a self-regulatory initiative for the greater good. We credit those thought leaders who recognized the importance of an industry-wide registry that would fill a gap for regulators and we commend those companies who wanted to be--and are-- part of the first wave of labels available in the product registry. Together we gained consensus around a dietary supplement registry we built it and now we re ready to make it grow. With the launch the Supplement OWL is now widely accessible for examination. All companies can test-drive the product registry and retailers can begin to use it to evaluate products on their shelves. CRN is confident that the number of companies participating in the Supplement OWL and the number of labels included will grow to the point where users will question why a company s label is not available in the Supplement OWL. Today is just the start but broader engagement that s the end game said Mister. If you have confidence in your product and in your label why wouldn t you want to house it in a free product registry that demonstrates you re willing to help the industry be more transparent and more accountable to our regulators to retailers and ultimately to consumers. If daylight is indeed the best disinfectant then the Supplement OWL helps to clean up the industry by putting products on display. Earlier this month the American Botanical Council the Consumer Healthcare Products Association and the Natural Products Association publicly announced their support for the Supplement OWL. It s the right thing to do especially at a time when some anticipate government will seek to reduce industry regulation. That s when we have our best opportunity to proactively find ways to raise the bar for this industry. That s what the Supplement OWL does. The Supplement OWL offers two tiers of information. Participation in Tier 1 includes an image of the product a complete product label and other fields of information obtained principally from the label itself. There is no charge to participate in Tier 1. Tier 2 allows companies to upload additional supporting information and documentation about their products and to choose who will have access to that information. Once a label is uploaded to Tier 1 companies can participate in Tier 2 for a nominal charge. Getting started with uploading labels involves a three-step process beginning with outreach to UL the global independent safety science company who is developing and administering the technology that drives the Supplement OWL to learn what product and label information will need to be submitted. Then companies can choose from four options to determine the best method to submit their labels 1) electronic transfer from an existing IT system 2) conversion from the ODS label database 3) manual data entry or 4) a third-party upload service. Homeopathic Manufacturers Explore Next-generation Quality Control ember companies of the American Association of Homeopathic Pharmacists (AAHP) gathered to learn about promising new technologies recently used by the U.S. Food and Drug Administration (FDA) to analyze homeopathic products. Developments in this area could lead to industry-wide manufacturing advancements as well as improvements to the homeopathic drugs that help consumers every day. Mark Land president of AAHP opened the discussion at the association s March 9 biannual meeting in Anaheim CA. In a broad overview Land stated that while most instruments currently used to assess homeopathic medicines can resolve to below one microgram--to less than one part per million--new technologies can resolve to parts per billion. Since homeopathic drug products go through a process of serial dilution to achieve certain potencies these hypersensitive technologies may allow manufacturers to get a deeper look into the makeup of their products than was ever before possible. These technological advancements mark an industry that is growing and getting better. Yet challenges can emerge as new data becomes available for example consistencies at the M parts-per-million level may not hold true at the parts-per-billion level. Dr. Neil Spingarn the meeting s keynote speaker who specializes in chemical analysis and laboratory testing services for pharmaceutical manufacturers is seeing this growing pain throughout the pharmaceutical industry. Routine analysis within our lifetime has evolved from parts per million to parts per billion and even parts per trillion if you know what you are looking for said Spingarn. The challenge is that we find more things as technology gets better but that should lead to exciting developments in homeopathic medicines. In light of new analytical capabilities Spingarn gave members in attendance tips to hone their processes and procedures around quality control. He touched upon potential mechanical issues in the dilution process such as the unpredictability of bubbles and foam accuracy of automated equipment and potential interactions with materials on the surface of succussion containers. Spingarn also instructed attendees on how to perform a step-by-step validation of the dilution process. Further discussion from the membership focused around validations for extreme dilutions the ability to detect memory effects labeling and logistical issues around scheduling and carrying out validation tests. Land and Spingarn both emphasized that in adhering to validation of substances in the parts-per-billion level the homeopathic industry would be reaching a high bar in quality control. In fact such high standards are generally reserved for contaminants not often applied to pharmaceutical products. The FDA s recent use of these hypersensitive technologies on homeopathic products was one of the first for the pharmaceutical industry. By setting a new and very high standard for product testing and quality validation the FDA has given the homeopathic industry an opportunity for growth said Land. Seven randomized trials with zinc acetate and zinc gluconate lozenges found that the duration of colds was shortened on average by 33 percent. Zinc lozenges appear to influence the common cold through the release of free zinc ions into the oro-pharyngeal region. However zinc ions can bind tightly to various chemical complexes in such a way that little or no free zinc ions are released. Previously zinc lozenges containing citric acid were shown to be ineffective in treating colds because citric acid binds zinc ions very tightly and no free zinc is released. In the meta-analysis Dr. Harri Hemil from the University of Helsinki Finland collected randomized trials on zinc acetate and zinc gluconate lozenges and compared their observed efficacies. Three trials had used zinc acetate lozenges and found that colds were shortened on average by 40 percent. T Four trials had used zinc gluconate lozenges and colds were shortened on average by 28 percent. The 12 percent difference between the average effects of the two kinds of lozenges was explained purely by random variation. Furthermore one of the zinc gluconate lozenge trials was an outlier inconsistent with all the other six zinc lozenge trials. If that outlier trial was excluded the difference between the three zinc acetate and the three zinc gluconate trials shrunk to just 2 percent i.e. a 40 versus 38 percent reduction in common cold duration. Thus properly composed zinc gluconate lozenges may be as effective as zinc acetate lozenges. Hemil also analyzed the dose response relationship between the elemental zinc dose and the observed efficacy in reducing common cold duration. There was no difference in the efficacy between five trials that used 80 to 92 mg of zinc per day and the two trials that used 192 and 207 mg of zinc per day. Thus zinc doses of more than 100 mg per day do not seem to provide any more benefit. According to Hemil there is no justi- fication for the popular phrase that there is no cure for the common cold because of the strong evidence that zinc lozenges can shorten common cold duration by more than 30 percent. However in future studies the optimal composition of zinc lozenges should be investigated. The optimum frequency of their administration also warrants further investigation. Nevertheless he also considers that the current evidence of efficacy for zinc lozenges is so strong that common cold patients should be encouraged to try them for treating their colds but the patients should ascertain that the lozenges do not contain citric acid or its salt citrate. New Measurement Technique Lowers Estimated Vitamin D RDA A fter re-measurement of vitamin D by improved technology the recommended dietary allowance (RDA) for vitamin D intake drops from 800 to 400 International Units (IU) per day new research reports. The results of the study were presented at ENDO 2017 the annual scientific meeting of the Endocrine Society in Orlando FL. The RDA is easily achievable with a supplement of 400 IU in winter when vitamin D levels are lowest in North America said principal investigator J. Christopher Gallagher MD professor and director of the Bone Metabolism Unit in the Division of Endocrinology of Creighton University School of Medicine. In estimating the RDA for vitamin D intake the laboratory method used for measuring serum 25-hydroxyvitamin D--25(OH)D--can affect the results he said. The estimated RDA based on the older immunoassay system was 800 IU daily whereas the newer liquid chromatography tandem-mass spectrometry (LC-MS MS) technique estimated that 400 IU daily would meet the RDA. In their earlier double-blind doseresponse clinical trial in the winter and spring of 2007 to 2008 Gallagher and his colleagues enrolled 163 healthy postmenopausal Caucasian women 57 through 90 years of age with vitamin D insufficiency and followed them for one year. The women were at least seven years postmenopausal and they had vitamin D insufficiency based on the World Health Organization cutoff (serum 25(OH)D 20 ng ml or lower). The participants were randomized to one of seven vitamin D3 doses 400 800 1600 2400 3200 4000 4800 IU day or placebo for one year and all the women were given calcium supplements to maintain a total calcium intake. After analyzing the samples and estimating the RDA using the older immunoassay the authors reported that 800 IU daily would meet the vitamin D intake requirement for 97.5 percent of the population. But now that liquid chromatography mass spectrometry (LC-MS MS) has become the gold standard for measuring 25(OH)D the researchers have reanalyzed the original samples using this new technology. Able to determine a more precise dose-response curve they have calculated the RDA for vitamin D to be 400 IU daily. Remember this RDA is for bone health only Gallagher cautioned. It may be different for other diseases. Although trials looking into cancer diabetes and other diseases are ongoing we do not have information about this yet. Whatever the reason perhaps you ve considered partnering with a private labeler to create a unique brand for your store. Carrying a private label creates brand recognition as well as an exclusive product that can be tailored to your customers unique needs and demands. It may sound like a daunting prospect to enter the manufacturing arena even as a silent partner focused more on the retail side of things but more and more businesses are embarking upon offering private label products. Burman noted that he has seen growth in products for inflammation such as turmeric. Burman happened to find his at an annual trade show and has enjoyed a mutually beneficial relationship for close to two decades. The hugest thing is making sure they know you are behind them and they need to be behind you so that you both grow and have mutual goals in thriving as natural product companies he said. Although there is no magic number and one may certainly suffice depending on your needs Kanter has chosen to work with three private labelers. They have different formulas or approaches or ways they put their products together so it was a way to give our customers choices he said. With very few exceptions we do not do the same product among the three brands. Another consideration is ensuring that you diversify your stock. For one thing Kanter observed not all products are available in private labels so you should certainly establish relationships with other companies. Your real goal is to sell other products. If you position them right you can be successful with other brands and your own private label--that it is the key he said. You want to be really careful that you assure the quality of your product is equal to the other products that you are selling he continued. And that should be reflected in your price if you underprice the products customers may question the quality. We operate from the fact that we deserve to earn a profit. Customer Service Education Customers invariably will have questions about your products everything from How is this product different from that product or What does this ingredient on the label mean or How often should I use this to a myriad of others. Part of transparency is making sure that the manufacturer provides and the retailer has access to and regularly reviews certification and audit reports. Testing is also essential. While many contract manufacturers have sophisticated laboratories at the end of the day it is the retailer s name on the product label. An independent testing protocol will offer you the greatest assurance said Trosin. Martens agreed saying Utilize third-party testing routinely. where they are sourcing their ingredients by providing a certificate of analysis. And quality should come before expense advised Martens. Consider whether the quality of the cheapest product is good enough. Labeling It s in their name--private labelers can work with you on designing your label. For example NOW can present the retailer with label design options either from stock or they can work together to customize something unique as well as to make sure that the label is compliant. knowledgeable retail staff that thoroughly understands the products and can answer questions confidently and accurately. Kanter said that when he was choosing a private labeler a major consideration was customer service-- would they train the staff either at the facility or at the store or via webinar etc. And because ideally this should be a mutually beneficial partnership some manufacturers may lend a hand with sales. Ask any private labelers you are considering what sort of marketing programs they have in place to help sell your products successfully advised Flight. Private Label Things to Know Once you have selected a private labeler manufacturer you will need to establish expectations on both sides of the fence. Some decisions that you will have to make are for starters what product(s) or line of product(s) do you wish to brand as your own What are the reordering guidelines including how frequently and how far in advance How should you design the label including font and colors A thousand and one other questions may pop into mind. If you ve chosen correctly your private labeler will help guide you through the process. Formulas You don t have to be a science geek to work with a company in developing formulas for your products--they are the experts. However be sure that the company reps do explain the ins and outs of the product(s) so you will know exactly what you are getting. Burman said that the company should ideally offer a variety of unique formulas that are hard to find and specific to natural retailers. Also make sure they don t sell the formulas to mass market companies he recommended. Zehrung added that his company has recently lowered their minimum order quantities which allows more retailers to participate. We also contract with several labs that give us a number of delivery methods we can offer as well as many packaging options he said. Orders for custom formulas however may take more time as they are subject to the availability of ingredients. Once you establish a relationship with a private labeler you may reap some additional rewards. For example Burman has been with the same vendor for more than 18 years and their loyalty has earned them a three piece per product minimum as well as free shipping for orders over 300. Advantages and Disadvantages Exclusivity. Name recognition. Visibility. Customer loyalty. Repeat business. Private label provides retailers the opportunity to brand their store s image a proven approach to attract and retain business said Flight. As customers become used to a particular store brand it reinforces their loyalty to the store she continued. And you become an exclusive supplier to your customer base whether you sell the products in store online or both. Similarly another major advantage is a steady stream of advertising without spending additional money. When a product is on a consumer s shelf at home the consumer is reminded of your store on a daily basis said Kanter. On the flip side you re also putting your good name and reputation on the line. If your private labeler disappoints in any way either by producing an inferior product or by not honoring any aspects of the agreement it is your store rather than the anonymous manufacturer that will bear the brunt. Zehrung also cautioned that retailers should be prepared to put up some money in the beginning particularly if their aim is to sell a high-quality product--and it is. You have to spend it to make it and the better your product the more it usually costs to make. The key is finding that balance between high end high quality and affordable price competitive in the category he said. Martens concurred adding Go for quality instead of simply considering low cost. Control is another benefit Zehrung pointed out. Working with your private labeler the retailer can have a voice when it comes to ingredients testing etc. These are all things the retailer can then use to advertise to their followers to instill faith in their growing product line he said. In fact Burman said that the company with whom he has dealt with for many years has consistently improved their formulas and has ... brought key trending products to our shelves. In addition there are consumers who choose to be avoiders of some foods or ingredients out of humanitarian environmental and or religious concerns. This confluence of reasons seems to have created a sizeable and growing market within the U.S. retail marketplace and it has been defined as that of Free From. This suggests there is a segment of consumers who do not differentiate between organic and free-from products. For companies to effectively use free-from claims to support sales manufacturers can look to align with factors consumers deem as being more important to leverage the added benefits in a meaningful way. The Free-from Opportunity Consumers definitely have an interest in free-from foods however there appears to be a series of trade-offs after price in their minds when using the claim to select food and beverages. By now most people on a gluten-free diet know that gluten refers to the proteins found in wheat barley and rye and avoid consuming these ingredients at all costs. However most people and even some experts would say that distilled liquors regardless of their starting ingredients (including those made with wheat barley or rye) are completely safe to drink because the gluten is removed during distillation. So what should one believe The United States Alcohol and Tobacco Tax and Trade Bureau (TTB) the regulatory body and competent authority responsible for the labeling of almost all alcoholic beverages only allows gluten-free claims on product labels if the alcohol is made without wheat barley rye crossbred varieties of these grains or ingredients derived from these grains. Products made with wheat barley or rye which have been processed to remove gluten are permitted to be labeled as processed (or treated or crafted) to remove gluten if one of the following statements is also included on the product label 1. Testing problems may occur because gluten cross-contamination can occurs before and or after fermentation or distillation making it difficult to know what type of test to use. The dust from gluten common tools equipment and employee hygiene and traffic all add to the potential for cross-contamination. The official position of the TTB states that there is no scientifically valid method for accurately measuring the gluten content of fermented beverages. Therefore it is not possible to absolutely verify gluten protein content in final distillate. The Allergen Control Group (ACG) owners of the Gluten-Free Certification Program (GFCP) has reached a consensus to start clean stay clean in order to reduce the potential for failure and add an extra level of protection to celiac consumers. To do this the GFCP sets out that gluten should be managed as a chemical food safety hazard. That means no intentional addition of gluten as an ingredient is allowed. This article will examine both types of remedies. F Flower Essences Background The British doctor bacteriologist and homeopath Edward Bach discovered flower essences back in 1930. Bach s interest in developing this system of medicine was grounded in his experience of both conventional medicine and of homeopathy. He described 38 remedies elaborated with flowers and shoots each one corresponding to an emotional pattern or a typical personality. He also developed remedies for emergency situations.1 Flower essence remedies are designed to be a system of emotional healing. Put succinctly flower essences are selected by or for a patient with the intent of addressing issues associated with that person s psyche. The participant was a 53-year-old woman at the postmenopausal stage who had been diagnosed with insomnia according to the criteria of the American Academy of Sleep Medicine and complained of climacteric symptoms. The patient underwent treatment with a flower essence blend for four months. Based upon sleep questionnaires and polysomnography measurements results showed that both sleep perception and objective sleep were improved. The patient s anxiety and menopausal symptoms were also reduced after the treatment. Another published report9 described the case of a 78-year-old man who suffered from multiple disorders including herpes zoster. The patient was treated with flower essence therapy. The result of the treatment was a short recovery time for the different disorders and a reduction in anxiety for the patient. A case-study10 analysis was conducted to explore the potential of flower essence remedies as a means of pain relief. The results were that of 384 subjects 41 suffered pain. Of these 46 percent felt treatment had relieved their pain in 49 percent the physical outcome was unknown. About 88 percent of all subjects reported an improvement in their emotional outlook. Researchers concluded that flower essence remedies brought about positive emotional changes in the majority of subjects in this study. group of patients who received flower essence treatment for various physical and emotional disorders in a primary health care center from a nurse trained in this therapy. Results were that 87.4 percent of those patients who received treatment reported good or very good results. Depression five studies Five independent clinical studies12 were conducted in which flower essences were used in the treatment of depression. The results of these studies were measured using the Beck Depression Inventory (BDI) and the Hamilton Depression Scale (HAM-D). Therapists in Cuba under the Cuban Ministry of Public Health conducted four of the studies. The first study tracked about 50 patients who completed therapy over a period of five months. The results were a significant reduction in depressive symptoms. Twenty patients participated in the second and third studies over a two-month and three-month period. Results were that significant drops in Anxiety and depression In an open study 13 91 patients suffering either anxiety (including stress) or depression were treated with flower essences. They were followed up with regular assessments over several months (up to 16 for a proportion of patients) by therapists and outcomes were reported as either nil partial or complete recovery. Although the natural history of minor psychiatric disorders is that they are often transitory the results were nonetheless interesting 89 percent of this group of patients made a partial to complete recovery. Results were that those patients using the flower essence cream experienced significant self-reported improvements in symptom severity and pain intensity. In addition all signs observed during the clinical exam showed significant improvements among the groups as well as symptoms of pain night pain and tingling. In conclusion the flower essence cream was an effective intervention in the management of mild and moderate carpal tunnel syndrome reducing the severity symptoms and providing pain relief. Overdue pregnancy In a randomized controlled study16 of 24 pregnant women who were at least five days overdue subjects received flower essences attention or no intervention (control). Results were that medicines usage (i.e. orthodox medicines to control pain and nausea) was significantly less (p 0.032) for the group receiving flower essences when compared with the other two groups. Indeed seven of the eight subjects in the flower essence group used no medication. Furthermore the investigator reports that these mothers tended to deliver with less assistance suggesting less anxiety. Tantrums in children Fifty-eight children aged 2-5 years whose parents reported frequent tantrums participated in a study17 in which flower essences were tested for efficacy. Tantrum frequency tantrum severity and parental mood were measured on five occasions over eight days before treatment and on a further five occasions over 10 days after the start of treatment. There was a good response. The patient was symptomfree at three-year follow-up. This suggests that tissue salts such as ferrumphos prescribed on constitutional grounds may play a useful role in supportive and palliative for patients with malignant disease. In group B 56.5 percent were free of recurrence and 43.5 percent had a maximum of six recurrences. In group A only five subsequently received antibiotics though homoeopathic treatment was carried through to the healing stage in the remaining children. Mixing Flower Essences and Cell Salts Flower essences and cell salts are natural medicines with a history of safe use. Furthermore research suggests that these nature-based remedies have a broad range of effective applications for human health. Given that flower essences correspond to emotional patterns while cell salts are more specific to metabolic deficiencies there is no redundancy to their respective mechanisms of action. Furthermore since they both have relationships with homeopathy there may be value in their concurrent use. In fact a market trend seems to be developing toward mixing flower essences and cell salts together which may offer a safe and effective addition to the natural medicine repertoire. VR Clinical Studies Published clinical studies on cell salts include their use for chronic sinusitis and ear infection. Chronic sinusitis A multicenter observational study23 was conducted with 550 patients to ascertain the therapeutic usefulness of homeopathic medicine as tissue salts in the management of chronic sinusitis (CS). Symptoms were assessed using the chronic sinusitis assessment score (CSAS). Regimes and adjustment of regimes in the event of a change of symptoms were pre-defined. Results were that there was a statistically significant reduction in CSAS (P 0.0001) after three and six months of treatment. Radiological appearances also improved. The cell salts found to be most useful in promoting marked improvement included Sil. (55.2 percent of 210) Calc. (62.5 percent of 98) Phos. (66.7 percent of 45) and Kali iod. (65 percent of 40). No complications were observed during treatment. In conclusion treatment with cell salts may be effective for CS patients. Ear infection In a prospective observational study24 carried out by one homoeopathic and four conventional ENT practitioners the two methods of treating acute pediatric otitis media (ear infection) were compared. Group A received treatment with homoeopathic single remedies (including silicea) whereas group B received nasal drops antibiotics secretolytics and or antipyretics. The study involved 133 children.
