Very likely there are many Vitamin Retailer readers who have been affected themselves and or have a friend or loved one who has experienced the consequences of this awful disease. The issue is so severe the World Health Organization (WHO) recently highlighted its stark statistics The number of people living with diabetes has almost quadrupled since 1980 to 422 million adults with most living in developing countries. Factors driving this dramatic rise include overweight and obesity. WHO marked its annual World Health Day (April 7) by issuing a call for action on diabetes. D Health-promoting Environments Reduce Risk Factors Measures needed according to WHO include expanding health-promoting environments to reduce diabetes risk factors like physical inactivity and unhealthy diets and strengthening national capacities to help people with diabetes receive the treatment and care they need to manage their conditions. If we are to make any headway in halting the rise in diabetes we need to rethink our daily lives to eat healthily be physically active and avoid excessive weight gain said Dr. Margaret Chan WHO director-general. Commitments to Reduce Diabetes Many cases of diabetes can be prevented and measures exist to detect and manage the condition improving the odds that people with diabetes live long and healthy lives said Dr. Oleg Chestnov WHO s assistant director-gener- al for NCDs (non-communicable diseases) and mental health. But change greatly depends on governments doing more including by implementing global commitments to address diabetes and other NCDs. In March the Obama administration proposed expanding Medicare to cover programs to prevent diabetes among millions of people in the U.S. at high risk of developing the disease marking the sixth anniversary of the Affordable Care Act with the prospect of a new benefit according to a report in the New York Times. Sylvia Mathews Burwell the secretary of health and human services announced the proposal at a Washington D.C. YMCA. Under the plan Medicare would pay for certain lifestyle change programs in which trained counselors would coach consumers on healthier eating habits and increased physical activity as ways to prevent type 2 diabetes. Such programs have been found effective in people with prediabetes meaning that their blood sugar levels are higher than normal but not high enough to be considered diabetes. But who will deliver these preventive services and how they will be paid for are still questions to be answered. While the government supports expansion of the diabetes prevention program it must now go through a period of rule making and public comment beginning this summer American Medical Association (AMA) President-Elect Andrew W. Gurman said in an interview. So while government and medical leaders determine appropriate responses it is the natural product retailers and industry members place to lead the charge of WHO s call to action by keeping this health crisis top-of-mind. What makes them unique All three locations-- Barboursville Charleston and Kanawha--are inside or adjacent to a Drug Emporium store. We are basically a health food store within a drugstore said Travis Lemon manager and herbalist of the chain s Barboursville location. That often proves beneficial when a customer has concerns about possible interactions between their medications and supplements Lemon pointed out adding that there are pharmacists available to address those concerns including questions about supplement quality or dietary restrictions. And they remain unique in their community. We do not have many competitors in our area but some of the mass-market retailers are starting to carry more organic and natural foods Lemon said. Healthy Life Market tries to carry only brands that assure quality and purity standards. HEALTHY LIFE MARKET (Barboursville Charleston and Kanawha WV) dedicated to organic and natural foods with one of the aisles split between refrigerated frozen foods and glutenfree dry foods. According to Lemon Healthy Life Market has always tried to keep the brunt of its gluten-free and low-allergen items in a separate section for the convenience of customers that may be newly diagnosed with celiac or food allergies. Over the years the stores have worked very closely with local celiac groups to make it easier and more convenient to make the transition to a gluten free lifestyle. In addition Lemon noted We donate certain unsold food items to a local soup kitchen and pet items to a local no-kill animal shelter. The rest of the store consists of half an aisle of bulk herbs and spices and half an aisle to natural HABA. We [also] have two aisles dedicated to natural supplements arranged mainly by brand and our back wall is compiled of sports nutrition products. We have floor space for anywhere between six and nine full pallets of large quantity sale items Lemon said. We always do very well with NOW and our private label brand Lemon shared. The Terry Naturally brand is also selling very well and has been for the last year or so. I hear a lot back from our customers and they seem to be noticing great results with their turmeric-based products. Other brands that Healthy Life Market sees doing well in their store include Garden of Life particularly the new Dr. Formulated line of probiotics as well as Gaia Herbs and Oregon s Wild Harvest in the herbal category. H Community Activity Healthy Life Market strives to do as many community health fairs as they can. We donate gift baskets and gift cards to local events when we can and we try to have in-store events when time allows. Out last in-store event was a donation based class led by a local yoga studio with all proceeds going to a local no-kill animal shelter. I also think that customers trust our staff to be knowledgeable of supplements. Thankfully we work closely with our supplement brands and I as an herbalist spread my time between our three locations and work directly with our employees to help keep staff up-todate on product information Lemon said. The development and cultivation of green tea is thought to have begun during the Tang Dynasty (7 to 10th century). Matcha was extremely precious and was produced only in small quantities so that the Shogun (warriors) and nobility were able to drink it. The rule will require those involved in transporting human and animal food by motor or rail vehicle to follow recognized best practices for sanitary transportation such as properly refrigerating food adequately cleaning vehicles between loads and properly protecting food during transportation. The action is part of a larger effort to focus on prevention of food safety problems throughout the food chain and the rule implements the Sanitary Food Transportation Act of 2005 (SFTA) as well as the requirement in section 111 of FSMA that instructed FDA to issue SFTA regulations. The regulation will apply to food transported within the United States by motor or rail vehicle whether or not the food is offered for or enters interstate commerce. Shippers loaders carriers and receivers engaged in transportation operations of food imported by motor or rail vehicle and consumed or distributed in the United States are also subject to the final rule. Consumers deserve a safe food supply and this final rule will help to ensure that all those involved in the farm-to-fork continuum are doing their part to ensure that the food products that arrive in our grocery stores are safe to eat said Michael R. Children who drink at least three glasses of milk daily had a higher serum vitamin D level than their peers who drink milk in lesser amounts. The use of vitamin D supplements was also associated with a higher serum vitamin D level. Furthermore children who exercise more than two hours per day had a higher serum vitamin D level than children who exercise less than 1.5 hours per day. The article was published online in British Journal of Nutrition. Very few children had severely low serum levels of vitamin D. However 20 percent had a serum vitamin D level lower than 50 nmol l which is often considered as a sufficient level. Children whose blood samples were taken in the autumn had the highest serum vitamin D levels. This may be explained by the fact that in the summer vitamin D is formed on the skin due to sunlight exposure. In northern latitudes there is not enough sun exposure to maintain sufficient vitamin D levels in the winter season and vitamin D supplementation is often required. A sufficiently high serum vitamin D level is associated with better bone health and there is some evidence that it may also be associated with a lower risk of many chronic diseases. More than 80 percent of the children had a lower intake of vitamin D from food and vitamin supplements than recommended 10 g per day in Finland. Furthermore as many as 40 percent of the children did not use any vitamin D supplements at all. Milk was the most important source of vitamin D amounting to nearly 50 percent of the children s intake. Approximately 30 percent of the vitamin D intake was from dietary fats and approximately 10 percent from fish. High Serum Omega-6 PUFAs Linked to Lower Risk of Type 2 Diabetes new study from the University of Eastern Finland shows that high serum omega-6 polyunsaturated fatty acid concentrations are linked to a significantly reduced risk of type 2 diabetes. The findings were published in the American Journal of Clinical Nutrition. It has been speculated that a high intake of omega-6 polyunsaturated fatty acids may increase the risk of several chronic diseases as these fatty acids have been suggested to promote low-grade inflammation among other things. However studies conducted on humans have not established a link between even a high intake of omega6 fatty acids and inflammation. Furthermore omega-6 fatty acids have beneficial effects on for example glucose metabolism. Earlier research has systematically linked especially linoleic acid which is the most common omega-6 fatty acid to a reduced risk of cardiovascular diseases and type 2 diabetes. However scientific evidence relating to the health effects of other omega-6 polyunsaturated fatty acids is not as inconclusive. A The serum fatty acid concentrations of 2 189 men aged between 42 and 60 years and with no baseline type 2 diabetes diagnosis were analyzed at the onset of the Kuopio Ischaemic Heart Disease Risk Factor Study (KIHD) in 1984-1989 at the University of Eastern Finland. During a follow-up of 19 years 417 men were diagnosed with type 2 diabetes. The study found that high serum omega-6 polyunsaturated fatty acid concentrations were associated with a 46 percent lower risk of developing type 2 diabetes during the follow-up. When analyzing the independent associations of different omega-6 polyunsaturated fatty acids a similarly significant association was found between high serum linoleic and arachidonic acid concentrations and a lower risk of type 2 diabetes. However high serum gamma-linolenic and dihomo--linolenic acid concentrations were linked to a higher risk. The study indicated that high serum linoleic and arachidonic acid concentra- tions are linked to a lower risk of type 2 diabetes. The serum linoleic acid concentration is determined by the person s diet and the main sources of linoleic acid are vegetable oils nuts and seeds. Arachidonic acid is present in meat and eggs however the human body can also make arachidonic acid from linoleic acid. Gamma-linolenic acid and dihomo--linolenic acid are mainly formed in the human body from linoleic acid. Their concentrations in serum are very low in comparison to for example linoleic acid. The association of gammalinolenic acid and dihomo-linolenic acid with a higher risk of type 2 diabetes has been observed in some earlier studies but the underlying reason remains unknown. In fact in just the past year I ve personally had two scientific reviews on vitamin K2 published in peer-reviewed journals. The first one examined vitamin K2 s cardiovascular roles and the second reveals the prevalence of vitamin K2 deficiency insufficiency. Here I ll provide an overview of the research I presented in two peerreviewed articles. But before jumping right into this recently published research let s take a quick look at some background information regarding vitamin K. Research on New E min K1) and intestinal microbiota synthesizes a range of K2 forms collectively referred to as menaquinones.3 The menaquinones have a number designation according to the number of repeating 5-carbon units in its side chain. For example if there are seven repeating 5carbon units the designation will be menaquinone-7 or MK-7.4 Among the menaquinones the MK-7 form of vitamin K2 has had the most significant research behind it supporting is value in human health. bone Gla protein (synthesized by osteoblasts) and matrix Gla protein (MGP found in cartilage bone and soft tissue including blood vessel walls). Carboxylation and Vitamin K2 arboxylation is a process in which a carboxylic acid group is reacted with a substance such as a protein--and carboxylation is necessary for the formation of critical VKDPs that have a positive impact on bone strength and arterial stiffness. Since 10-40 percent of VKDPs remain undercarboxylated research was conducting showing that the MK-7 form of vitamin K2 was more effective than vitamin K2 and increasing the carboxylation of VKDPs. This was true when doses of 90 mcg or more if MK-7 were used daily. C Background Discovered in 1929 vitamin K was originally identified for its role as a blood-clotting (coagulation) cofactor.1 In the 1970s vitamin K-dependent proteins (VKDPs) were discovered which led to a more comprehensive understanding of vitamin K s role with respect to bone and cardiovascular health.2 Additionally two forms of this fat soluble vitamin exist vitamin K1 and K2. Plants synthesize phylloquinone (aka vita38 VITAMIN RETAILER The Cardiovascular Benefits of Vitamin K2 In the July August 2015 issue of the journal Agro Food Industry Hi Tech 5 my coauthor and I discussed research showing that the primary way in which vitamin K acts to promote cardiovascular health is by inhibiting calcification in the arteries-- which would otherwise lead to atherosclerosis. Specifically some of the seventeen VKDPs have been identified to date act as calcification inhibitors. These include osteocalcin (OC) also known as WWW.VITAMINRETAILER.COM MAY 2016 These proteins are local inhibitors of calcification in the tissues in which they exert their function. We also discussed the populationbased Rotterdam study 6 which included 4807 subjects and which provided the first evidence for a link between vitamin K and vascular health. The results showed the relative risk of coronary heart disease mortality and calcification of the aorta (an artery in the heart) was reduced in those with high intakes of vitamin K2 compared to those with a lower intake. The intake of vitamin K1 however was not associated with these same cardiovascular benefits. Another study 7 with 564 post-menopausal also showed a decreased risk of coronary calcification with higher intakes of vitamin K2 but not vitamin K1. In a three-year double-blind placebocontrolled trial 248 postmenopausal women received 180 mcg day of MK-7 (MenaQ7) or a placebo for three years to examine the effect on arterial stiffness (i.e. caused by calcification). The results were that supplementation with MK-7 significantly decreased measures of aortic stiffness and improved undercarboxylated VKDPs by 50 percent. Vitamin K2 Deficiency Insufficiency In the January 2016 issue of the Journal of Human Nutrition & Food Science 8 I discussed research suggesting that recommendations for vitamin K2 be increased to 180 mcg day for adults up from its current adequate intake levels of 90-120 g day for women and men respectively. The basis for this includes that facts that vitamin K Has poor bioavailability from food Has low circulation in the blood Is quickly metabolized and excreted Has low tissue stores Levels are often measured by an inadequate method The Food and Agriculture Organization of the United Nations estimates that the absorption of vitamin K from boiled spinach (a good source of vitamin K) is no greater than 10 percent. Furthermore relatively small amounts of vitamin K circulate in the blood compared to the other fat-soluble vitamins with about 70 percent being excreted in the feces and urine from an oral dose of vitamin K1. Due to its rapid metabolism circulating vitamin K levels is not dependable as measures of vitamin K status. Currently prothrombin time (a measure of how long it takes blood to clot) is typically used to assess vitamin K status but this is not routinely done except in individuals who MAY 2016 WWW.VITAMINRETAILER.COM take anticoagulants or have bleeding disorders. The problem is that changes in vitamin K intakes have rarely been shown to alter prothrombin time. Hence prothrombin time is not really an effective way to measure vitamin K status. Rather a much better method is to measure circulating levels of specific undercarboxylated VKDPs such as undercarboxylated MGP and undercarboxylated OC. Higher levels reflect of either of these reflect lower levels of vitamin K. In a study of 452 men and women (age range 60 80 years) undercarboxylated MGP was measured. Results showed that 97 percent had notable measures of undercarboxylated MGP suggesting vitamin K deficiency or insufficiency. When supplemented with vitamin K there was a significant reduction in undercarboxylated MPG. Furthermore research in healthy adults show that undercarboxylated MPG increase with age with a significant rise seen above age 40. This suggests that vitamin K levels decrease with age. The U.S. Food and Nutrition Board established the adequate intake (AI) levels of vitamin K for adults 19 years and older of 120 mcg day for men and 90 mcg day for women--but this may be inadequate. Research has shown that vitamin K2 (especially MK-7) has advantages of vitamin K1 and that MK-7 is well absorbed. Furthermore research has shown that 180 mcg day of vitamin K2 as MK-7 reduced arterial stiffness (caused by calcification) and decreased the age-related decline in bone mineral content and bone mineral density. Consequently it seems reasonable to suggest that recommendations for vitamin K2 (as MK-7) be increased to 180 mcg day for adults. with 45 mg (that s milligrams not micrograms) vitamin K2 (MK-4) daily for three years in 2185 postmenopausal osteoporotic women. Currently vitamin K antagonist medications (i.e. coumarins) are the only contraindications for patients using vitamin K2 although novel anticoagulant (NOAC) drugs may be used concomitantly. Consequently the recommendation of 180 mcg day vitamin K2 as MK-7 is well within safety ranges. Many of the super foods like cacao acai goji blueberries etc. owe their benefits to their flavonoid content. While different flavonoids have different effects in the body the key factor may not be a high intake of any one particular flavonoid but rather a high total flavonoid intake that also provides a high variety of flavonoids rather than any one particular flavonoid class. There are more than 8 000 different types of flavonoids out there in nature. The caveat is that the proanthocyanidin flavonoids must be a major part of the flavonoid intake. Good dietary sources of these compounds are found in red or black grapes (especially the seeds) apples cacao cocoa dark chocolate berries (especially blueberries cranberries and black currants) certain nuts (e.g. hazelnuts pecans and pistachios) and red wine. So with this caveat on the importance of proanthocyanidins in mind what is an effective dosage of flavonoids The best evidence on determining an effective dosage of total flavonoid intake is clinical trials with either well-defined sources of flavonoids from food and beverages or from flavonoid-rich extracts. Fortunately there has been an explosion of good scientific studies on a wide variety of flavonoid sources. For example there are fantastic studies with proanthocyanidin-rich extracts from grape seed cocoa pomegranate and pine bark as well as flavonoid rich extracts from citrus soy and green tea extract all showing significant clinical benefits including in the following health conditions Asthma Atherosclerosis (hardening of the arteries) Attention deficit disorder High blood pressure High cholesterol levels Male infertility Mild cognitive impairment Menopausal symptoms Osteoarthritis Periodontal disease Varicose veins venous insufficiency and capillary fragility Visual function retinopathy and macular degeneration There have also been a large number of studies with flavonoid-rich sources looking at more immediate effects such as their effect on blood vessel function blood flow or antioxidant capacity. Most of the studies with the aforementioned flavonoid-rich extracts have used an average dosage of about 300 mg per day to show an effect. Longer-term studies show even more benefit. For example one of the best examples of the practical effect seen by normalizing endothelial function is with grape seed extract (standardized to contain 95 percent procyanidolic oligomers) in people with high blood pressure. There are now two very well designed doubleblind clinical trials that show that within the first month of supplementation with 300 mg of grape seed extract it is able to normalize high blood pressure in most patients with pre-hypertension to moderate high blood pressure. Similar studies exist with pine bark (Pycnogenol) pomegranate hibiscus and hawthorn extracts--all natural products rich in procyanidins. And all used a dosage close to 300 mg per day to see a clinical effect. Methylenetetrahydrofolate reductase (MTHFR) is an enzyme that catalyzes the rate-limiting step in the final conversion of dietary folates into MTHF.1 There are several common genetic polymorphisms (i.e. two or more different forms of the gene) that impair the activity of this key enzyme.6 Because MTHF is required for the methylation of homocysteine (Hcy) to form methionine MTHFR polymorphisms inhibit the detoxification of Hcy causing it to build up in the plasma.1 4-6 High serum Hcy is considered to be a significant risk factor in cardiovascular diseases 7 neuropsychiatric diseases 8 ocular diseases 9 and others. It is well known that MTHFR polymorphisms are widely distributed within global populations.5 Pooled U.S. data indicate that up to 35 percent of Americans are carriers of at least one copy of one these polymorphisms.6 While heterozygotes with these polymorphisms typically exhibit only mild elevations in Hcy levels homozygotes tend to have much higher serum Hcy as homozygote status can decrease MTHFR activity by 50-70 percent.5 6 Because MTHFR is needed to convert dietary folate to MTHF these individuals tend to be unresponsive to FA supplementation.1 MTHF is a metabolically active form of folate that is also naturally occurring. It offers several advantages over FA and represents a superior form for supplementation. That is in spite of their taking folic acid in various supplements and eating fortified with folic acid. Unfortunately up to 60 percent of the population does not convert folic acid optimally into the active form that the body actually uses--5-methyltetrahydrofolate (or 5-MTHF for short). Those with one of the 50 genetic variations convert almost no folic acid to the useable form. Folate is the form found naturally in foods such as dark green leafy vegetables and Brussels sprouts. Sometimes called vitamin B9 it also needs to be converted to 5-MTHF. The scientific literature usually refers to generic folate but there seems to be no controversy that what does the work in the body is the active form 5-MTHF or that it is needed to regulate cell C growth and repair tissue from conception through old age. Textbooks tell us that active folate helps regulate DNA and key metabolic processes needed for healthy mood memory circulation immune function and overall well-being. Folate is also involved in the production of various neurotransmitters like serotonin dopamine and melatonin. This new trial builds on earlier research in the European Journal of Clinical Nutrition suggesting that taking just two capsules--or 480 mg--of AGE each day can effectively lower systolic blood pressure by up to 12 mgHg. In that trial 79 patients with uncontrolled hypertension above 140 mmHg were assigned to one of four groups. Three of the groups received AGE in doses of 240 480 or 960 mg. The fourth group received a placebo. After 12 weeks the researchers found that all of the patients taking AGE saw some reduction in their systolic blood pressure compared to placebo.
