And I do all of that--I try to eat a balance diet I go to the gym and work out with my personal trainer multiple times a week keep track of when I need to make appointments with my doctors and try to relax with a good book or the occasional vacation. But recently I ve noticed multiple print articles and TV news stories about self-care that go beyond what I mentioned above. As stress levels seem to be on the rise our overall health is affected. In fact stress increases the risk of heart disease stroke heart attacks and is linked to insomnia. In response some Americans are taking their health into their own hands. Often we put other people and things first before remembering to take care of ourselves. But the idea of selfcare may be as simple as what flight attendants remind us before every take off put on your own oxygen mask before you assist others. It is a reminder that if we don t take care of ourselves first we won t be able to help anyone else. And self-care essentially refers to the activities and practices we can engage in on a regular basis to reduce stress and maintain and enhance our short- and longer-term health and well-being according to a report for students on the University of F Buffalo s website. As for how I have been incorporating self-care into my daily life after a discussion about the topic with a friend we decided to try meditating each day even if we only had a few minutes. We each downloaded an app on our phones that offered guided meditations with various time limits and set a reminder to do it each day. After checking in after a week my friend and I both felt that it helped mellow us out a bit found it to be particularly beneficial in stressful situations and it helped us to sleep better. About a month after our dedication to daily meditation my friend called me with another idea for stress relief crystal bowl meditation class. After some thought I agreed because I wanted to try something new and different. During the class our instructor played crystal bowls. The singing of the bowls is said to produce a harmonization between their vibration and that of the person. After the 70-minute class (which honestly felt like 15 minutes) my body did indeed feel like it was vibrating and I was incredibly relaxed both mentally and physically. Using the app and taking the class have not only encouraged me to try new things but has also made me adamant about making self-care a part of my daily routine. Whatever advice or recommendations you can offer your patients on practicing self-care I m sure they would greatly appreciate but don t forget to make sure that you practice selfcare yourselves. Use high force invasive modalities (e.g. surgery radiation chemotherapy) Naturopathic doctors don t just identify N FDA Delays Rolling Out New Nutrition Facts Label he U.S. Food and Drug Administration (FDA) delayed indefinitely implementation of a new nutrition facts label that would enlarge calorie counts itemize added sugar and bring serving sizes in line with actual average portions according to Reuters. The rule which had been championed by former first lady Michelle Obama had been scheduled to go into effect on July 26 2018. In a statement on its website the FDA said additional time would provide manufacturers covered by the rule with the necessary guidance from FDA. The agency did not say how much additional time companies would be given or when it would announce the time frame saying only it would make an announcement at a later time. The resolution encourages physicians and health care providers to use Person-first language in all discussions resolutions and reports with regards to obesity. Person-first language places the person before the disability or disease when describing individuals affected by obesity. By approving the resolution the AMA commits to educating its members on the importance of person-first language for treating patients with obesity equipping their health care facilities with appropriate furniture medical equipment and gowns for patients with obesity and having patients weighed respectfully and without judgment. The AMA resolution augments previous efforts the OMA has undertaken to affect how various audiences communicate about patients coping with obesity. OMA partnered with several other organizations to publish the Guidelines for Media Portrayals of Individuals Affected by Obesity to ensure that all persons regardless of their body weight are represented equitably and accurately in journalistic reporting. Additionally OMA played a major role in the AMA s decision to recognize obesity as a disease in 2013. This resolution was written and introduced by Dr. Ethan Lazarus OMA s representative in the AMA House Delegates and an obesity medicine physician from Denver CO. The first step toward improving obesity care in America starts with treating patients affected by obesity with dignity and respect he said. Lazarus invited several other organizations to cosponsor the resolution including the Colorado Medical Society the Minority Affairs Section the Endocrine Society the American Academy of Clinical Endocrinologists and the American Society for Metabolic and Bariatric Surgery. Unfortunately despite medical evidence to the contrary the prevailing view is still that obesity is caused solely by poor eating and sedentary behavior Dr. Lazarus said. Patients with obesity are still referred to as obese and shamed for their condition Dr. Lazarus said. He testified about a patient he treated recently who told him her primary care doctor told her she was morbidly obese. She never returned to that doctor because he reinforced her own perception that she was fat rather than a successful woman mother and wife who was coping with a disease. Similarly telling a person they are morbidly obese and that they should simply eat less and exercise more is not only of no help but is psychologically damaging reinforcing the notion they already believe which is that their obesity is their own fault he said. Using person-first language is a critical step for us to move the ball forward and begin to treat obesity like a serious disease with serious health consequences and to stop blaming patients for their health problems he said. Dr. Lazarus emphasized that the resolution does not get rid of the term obesity. It does not change diagnosis codes or billing codes nor does it change your medical charts. It simply asks us to use the term it in a sensitive way at the AMA and remember that we are treating people not diseases he said. Sure there s the tough independent and impassive type who wouldn t even think about visiting a doctor. Then there are those who make health checks and staying well a part of their regular routine in life. Some time ago a male friend noticed blood in his urine. Knowing this was unnatural he contacted his doctor within 30 minutes of experiencing this symptom. Later that day he was examined and within 48 hours was diagnosed with two potentially cancerous tumors in his bladder. Surgery took place the following week and now 10 years later my friend is still living a happy and in general healthy life. This is typically not the norm that a man would contact his physician within the first hour of his health issue. But had he overlooked the problem he might not be with us today. Why don t most men call a doctor upon encountering an abnormal symptom Why don t they schedule annual check-ups I have to agree that it s still considerably part of the tough guy mentality out there said Eric Wood ND at the Physical Health Complex in Fort Lauderdale FL. If you re not bleeding profusely or you can t breathe or something else dramatic hasn t happened you can tough it out. A report by the Centers for Disease Control and Prevention (CDC) found men are 80 percent less likely than women to use a regular source of health care.1 Other research shows that half of men ages 18 to 50 don t even have a regular source of health care.1 In fact one national survey revealed that a third of men in that age group hadn t had a checkup in more than a year and 40 percent had never had their cholesterol checked.1 Men reduce their risk of disease and death by paying more attention to their health from an earlier age and that can make a big difference said Ray Arnold former National Football League player and popular personal trainer at GB3 in Fresno CA. The body tends to bounce back more easily for men in their 20s but it takes longer to recover from poor health habits N the older men become. Doing weight bearing and cardiovascular exercises and stretching minimizing alcohol consumption eliminating smoking and eating a healthy diet with adequate amount of proteins to meet their physical needs offer many keys to maintaining good health said Arnold. According to Arnold who is in sales with a leading dietary supplement company all men should begin with a regiment that includes a multivitamin omega-3 fats ubiquinol CoQ10 and probiotics. Additional supplements like pomegranate extract prostate formulas and hormone replacement may be necessary on a caseby-case basis. Men certainly have the opportunity take better care of their health. So why don t they Men as boys aren t taught or encouraged to the degree girls are to pay attention to their bodies and tune into what they re body is telling them said Dr. Wood. Hence they generally aren t as clued-in as to what feels off what might be wrong and when it might be time to get some professional counsel. What can we as NDs do to convince men to take better care of their health For Dr. Wood it s education education education. He pointed out that investing in your health is investing in your true wealth and your family s prosperity and increasing your odds is powerful. Health is the number one determinant for achieving happiness in one s life so when you consider that shouldn t we all be paying much much more attention to it if one of the main aims of life is happiness he explained. No one truly wants to get old debilitated and suffer but if we don t take the time to care for ourselves properly in the first 50 years of our life what can we truly expect later as we get into the latter halves of our lives Preventative health goes a lot further to take care of overall health noted Florida-based Judith Thompson ND and vice president of the Florida Naturopathic Association ND s can start to focus on diet and exercise to stay physically strong and mentally focused. Preventing illness is much easier than treating disease. With the mentality of if it s not broken don t fix it the possibility for illness to develop is higher added Dr. Thompson. Dr. Wood agreed that prevention can go a long way to staying healthy. Prevention is key no matter what your gender is he said. Everyone needs to eat well sleep well and get enough sleep manage and lessen stress get active and do some regular exercise avoid risky behaviors such as drugs excess alcohol and smoking and work on regular detoxification. Men should have regular health checks because the leading causes of death among males can often be prevented. It s important to see your doctor for regular medical check-ups to help you stay healthy and to discover early warning signs of disease or illness. Cardiovascular disease diabetes and some cancers can often be found in their early stages when treatment may be more successful. By Lisa Schofield T rying to populate your own country by giving birth to 19 children (The Duggars) or using artificial means to have eight babies in one birth (Nadya Suleman) may be extreme but many childless couples are mentally emotionally--and they believe physically--ready to have one of their own offspring but unfortunately have trouble conceiving. Sterility in some men and women cannot be unchanged but for those relatively healthy men and women who are experiencing conception issues there is hope. Infertility said Dr. Bradley Nelson author of The Emotion Code is defined as the inability to conceive after one year of unprotected intercourse for women 34 and younger and six months if the woman is over 35 or the inability to carry a pregnancy to life birth. One in eight couples or 12 percent of married women have trouble achieving or sustaining a pregnancy now according to the Centers for Disease Control and Prevention (CDC). It s important to note how things have changed. Fertility rates around the world but particularly in the Western countries have been dropping steadily since the Baby Boom years and 1950s. Experts disagree on the reasons but everyone agrees that there are more people struggling with infertility now than any time in memory. There are some prevalent reproductive fertility issues and challenges for women and men today. A woman s reproductive system is delicate and complex said Michelle Violi PharmD dispensing pharmacists manager Wisconsin-based Women s International Pharmacy Inc. It is important for a woman s hormones to be balanced and her organs and tissues healthy in order for conception to occur. Hormones such as estrogen and progesterone play leading roles however there are many other hormones that are important players in the intricate process that is the female reproductive system. Corina Dunlap ND MS of A Woman s Time in Oregon explained that for women the most challenging reproductive concerns are ovarian factor infertility caused by agerelated ovarian decline (lessened ovarian reserve) or ovulatory dysfunction primarily when ovaries are not producing and releasing eggs normally. And in today s world she said women and men are having babies at an older age than in previous times and this negatively impacts egg count quality and likelihood for ovulation. Further ovulatory health can be impacted by the increasing prevalence of cases of endocrine disorders such as polycystic ovarian syndrome (PCOS) thyroid conditions and hypothalamic amenorrhea which can impede normal ovulation and ability for an egg to grow into a healthy embryo. These problems are often correlated with environmental exposures to pollutions pesticides and plastics-- known as endocrine disruptors--which impede and pose challenges to hormone health and fertility. Chris D. Meletis ND Educational Consultant with Fairhaven Health and author of Enhancing Fertility A Couple s Guide to Natural Approaches sees several other issues that impact reproductive efficiency in women. Early use of birth control pills he said override the natural hormonal balance of a young female prior to the monthly cycle being fully established. There can be long history of non- 22 NATURAL PRACTITIONER WWW.NATURALPRACTITIONERMAG.COM AUGUST 2017 pituitary driven monthly cycle due to the external hormones used to prevent pregnancy. He also pointed to relative estrogen dominance and relative progesterone insufficiency. Aging causes progesterone levels to drop more quickly than estrogen levels creating more marked imbalances. This is a concern as adequate progesterone is needed for pro-gestation to help sustain and maintain the fertilized egg until the placenta is sufficiently established to take of the maintenance of the pregnancy he explained. Low progesterone levels can increase risk of early miscarriage. More recently and more controversially Dr. Meletis asserted that the HPV vaccine may also contribute to ovarian dysfunction or failure. While it makes perfect sense to wait until financial security and stability are in place before having children this achievement also comes later in life than it ever did before and as such conception attempts later in life are not as easily successful. At this point said Dr. Meletis the hormonal cycle is not as strong and eggs are not as qualitative having endured years of exposure to chemicals and stress these impact genetics and epigenetic considerations of both nuclear DNA (human DNA) but also mitochondrial DNA that fuels cellular potential of each of the cells of a developing embryo and human for a lifetime he detailed. Prudence Hall MD founder of The Hall Center in California agreed. Today women are waiting to have children later in life lessening their chances for getting pregnant easily. Life and career goals emotional maturity and fertility are not necessarily in sync. Plus stressrelated issues (like career and other forces) can also make it more difficult to achieve pregnancy. In men explained Dr. Hall the combination of increased stress fatigue and bad lifestyle habits often create a toxic environment rending them less potent. She cited one study of Desert Storm combat soldiers showing that the extreme stress they were under significantly depleted their sperm count because of a significant drop in testosterone levels. According to Dr. Dunlap sperm health--a state that includes sperm count morphology and DNA fragmentation--is impacted greatly by environmental factors diet age and frequency of ejaculation. We ve seen average sperm counts decrease over the past few decades she noted. There are more recent conditions that affect reproductive health too. Dr. Nelson believes the most common underlying causes of infertility are emotional baggage followed by nutritional deficiencies. Emotional baggage he explained is also known as trapped emotions a state where emotional energies remain trapped within the body during and after intensely emotional events. A trapped emotion consists of invisible emotional energy that can be very disruptive to the body since the body itself is a complex energy field he explained. In this case a trapped emotion in the reproductive system will interfere with the biochemical reactions and acupuncture energy flow which may interfere with a normal reproductive process. This is one of the most common underlying reasons for reproductive difficulties especially in women in my experience he noted. Today women are waiting to have children later in life lessening their chances for getting pregnant easily. Life and career goals emotional maturity and fertility are not necessarily in sync. --Dr. Prudence Hall Inadequate nutrition is the second most prominent factor interfering with fertility Dr. Nelson stated. As an example he cited a study by Dr. Francis Marion Pottenger that compared fertility ability in cats fed either a totally cooked diet or a totally raw diet and concluding that the cooked diet group was as Dr. Nelson summarized never able to get past the third generation without losing their ability to reproduce. Incorporating more raw food into the diet and supplementing any deficiencies that may have developed is critical for fertility. Promoting Fertility Clients or patients interested in preparing themselves (and their partners) for familyhood or clients or patients who express difficulty in conceiving can be guided to help ensure their next generation will be a reality. For Dr. Dunlap recommendations are individualized for every preconception fertility patient she sees but generally she recommends a healthy diet lifestyle moderate exercise stress-reducing techniques and decreasing exposure to environmental toxicity as much as possible. Patients should stop drinking alcohol and not smoke marijuana to relax instead they can reduce stress via yoga meditation or a similar exercise don t overdo it in the gym fitness reduce caffeine intake (no more than 200 mg daily) reduce sugar simple carb intake and eat organic whenever possible. Further men and women should ensure they reduce their exposure to endocrine disruptors in and around the home and office (if possible here). These include dry cleaning solvents heavy metals pesticides and plastics. Timing of intercourse can help increase pregnancy potential. Dr. Dunlap recommends frequently after day seven (when day one is first day of menses) to several days after ovulation as sperm can be viable up to five days in the womb. The best chances for pregnancy are the five days leading up to ovulation. Women should track ovulation with any ovulatory predictor kit she added. For her male patients Dr. Dunlap recommends avoiding overheating of sperm (hot tubs hot baths saunas) limit EMR-cell phone WiFi exposure (such as carrying cellphones in pockets) maintain optimal BMI (body mass index) and avoid vigorous exercise and any anabolic steroids or supplements. The goal overall said Dr. Meletis is to ensure that both male and female contributions (sperm and egg) are from optimal healthy and minimally stressed and minimally medicated parents. He recommends a clean diet that follows at the very least the avoidance of the dirty dozen foods and consumption of the 15 clean foods offered annually by the Environmental Working Group. This will lessen incoming toxins. Also avoid unnecessary prescriptions and OTC (over-the-counter) medications as there is an entire class of drugs that poison the mitochondria that can alter male and female fertility he asserted. There are quite a few supplements that may be recommended to client and patients wishing to increase their ability to create a family. Choosing the correct nutraceutical for a given joint health customer is really a function of the criteria you set up as parameters. For example if you only want to consider those nutraceuticals with human clinical research to validate their efficacy that narrows the field somewhat. Now if you only want to consider those with multiple human clinical studies that narrows the field considerably more--but there are still several good ones from which to choose. If however your selection criteria also include only those nutraceuticals that have also been shown to work in healthy exercising and osteoarthritic populations your choices are limited to a select few nutraceuticals. Finally if you want the nutraceutical to provide significant results in just a few days the pickings are indeed slim. Nevertheless research suggests that there is still an excellent option that meets all these criteria nat- W ural eggshell membrane (NEM). NEM Background NEM is the thin membrane just under the shell that you sometimes have difficulty removing when you re peeling a hard-boiled egg. NEM is a sustainable food-sourced ingredient composed of fibrous proteins such as collagen types I V and X as well as other bioactive components such as glycosaminoglycans (i.e. dermatan sulfate chondroitin sulfate and hyaluronic acid)1 and sulfated glycoproteins including hexosamines such as glucosamine.2 This combination of bioactive components may contribute toward NEM s multiple mechanisms of action which may include oral tolerance suppression of tumor necrosis factor-alpha (TNF-) and suppression of collagen type II Ctelopeptide (CTX-II). Mechanisms of Action Oral tolerance refers to the phenomenon of reducing a certain type of immune response (tolerance) that results from the repeated exposure to ingested protein antigens that would otherwise cause a reaction. In-vitro research indicates that in the case of NEM this seems to occur via modulation of a transcription factor called NFB. Oral tolerance therapy has been shown to be effective in a variety of autoimmune diseases including arthritis diabetes colitis and multiple sclerosis.1 Further research on NEM s mechanism of action shows its ability to suppress TNF- a cell-signaling protein that plays a significant role in the inflammatory process.2 In-vitro research has shown that by suppressing TNF- NEM elicits an anti-inflammatory effect. CTX-II is a biomarker of cartilage turnover. In human3 dog4 and rat5 studies NEM has been shown to reduce CTX-II. This suggests that NEM helps reduce cartilage damage. Individually or collectively these mechanisms of action have demonstrated meaningful joint benefits in healthy and osteoarthritic populations. 30 NATURAL PRACTITIONER WWW.NATURALPRACTITIONERMAG.COM AUGUST 2017 Human Clinical Research in a Healthy Exercising Population A just-completed two-week randomized double-blind placebo-controlled clinical trial6 evaluated if 500 mg day NEM (NEM Stratum Nutrition) would reduce cartilage turnover as well as reduce joint pain or stiffness both directly following exercise and 12 hours post-exercise versus placebo in 60 healthy post-menopausal women while performing an exercise regimen on alternating days. Results showed that supplementation with NEM significantly reduced CTX-II versus placebo after both one week and two weeks of exercise. In addition NEM showed a significant reduction in joint stiffness versus placebo within four days. Likewise rapid treatment responses were observed at day 14 for recovery pain and stiffness compared to placebo. The treatment was well tolerated by study participants. pain and stiffness associated with osteoarthritis of the knee in 67 patients. WOMAC Osteoarthritis Index as well as pain stiffness and function WOMAC subscales were used to measure efficacy at 10 30 and 60 days. Results showed that supplementation with NEM resulted in statistically significant improvements (up to 26.6 percent) versus placebo at all time points for both pain and stiffness based upon WOMAC pain and stiffness subscales. Rapid responses were seen for mean pain sub-scores (15.9 percent reduction P 0.036) and mean stiffness sub-scores (12.8 percent reduction P 0.024) occurring after A four-week randomized controlled trial7 was conducted to evaluate the effects of 500 mg day of Stratum Nutrition s NEM or placebo on chronic joint pain in 60 physically active adults. Participants also completed a weekly exercise protocol designed to challenge their irritated joint and then rated their joint pain. Based upon VAS scores results showed that participants in the NEM group reported significantly less joint pain post-exercise following supplementation when compared to those in the placebo group (p 0.0171). Researchers concluded that NEM supplementation appeared to decrease post-exercise joint pain vs. placebo. Human Clinical Research in an Osteoarthritic Population A 60-day randomized multicenter double-blind placebo-controlled clinical study8 was conducted to evaluate the efficacy and safety of 500 mg day NEM (Stratum Nutrition) as a treatment for only 10 days of supplementation. Researchers concluded that supplementation with NEM 500 mg taken once daily significantly reduced both joint pain and stiffness compared to placebo at 10 30 and 60 days. Two open-label human clinical studies9 were conducted to evaluate the efficacy and safety of 500 mg day NEM brand as a treatment for pain and inflexibility associated with joint and connective tissue disorders. In the smaller study (which consisted of one group of 11 patients) results were that supplementation with NEM significantly improved flexibility at seven days (27.8 percent increase P 0.038) general pain at 30 days (72.5 percent reduction P 0.007) and flexibility (43.7 percent increase P 0.006) and range of motion-associated pain at 30 days (75.9 percent reduction P 0.021). In the larger study (which consisted of two group of 28 total patients) NEM pro- duced a significant improvement in pain at seven days for both groups (18.4 percent reduction P 0.021 and 31.3 percent reduction P 0.014). The significant treatment response continued through 30 days for pain (30.2 percent reduction P 0.0001) and NEM was well tolerated. A 60-day open label German clinical study10 was conducted to evaluate the efficacy and safety of 500 mg day NEM brand as a treatment for pain and inflexibility associated with moderate osteoarthritis of the knee and or hip in 44 patients. Measurements were taken at 10 30 and 60 days utilizing a 10question abbreviated questionnaire based on the WOMAC osteoarthritis questionnaire. Results showed that supplementation with NEM produced a significant improvement at 10 days (8.6 percent to 18.1 percent) and at 30 and 60 days (22.4 percent to 35.6 percent improvement) for all nine pain-related questions evaluated. In addition stiffness improved at 30 and 60 days (27.4 percent to 29.3 percent). More than 59 percent of patients rated the efficacy of NEM as good or very good following 60 days of supplementation. Treatment was well tolerated and researchers concluded that NEM significantly reduced pain both rapidly (10 days) and continuously (60 days) demonstrating its safety and efficacy as a therapeutic option for the treating knee and or hip osteoarthritis pain. Independently from the German study a four-week open-label Italian clinical study11 was also conducted to evaluate the efficacy and safety of 500 mg day NEM (Stratum Nutrition) in 25 subjects with moderate osteoarthritis of the knee. Measurements were taken at 10 and 30 days also using a 10-question abbreviated WOMAC osteoarthritis questionnaire. Supplementation with NEM resulted in reduction in pain at 10 days (40.6 percent p 0.001) and 30 days (66.4 percent p 0.001). There was also a statistically significant reduction in analgesic use during the 30-day study period. Additionally there was a significant treatment reduction in stiffness at 10 days (22.2 percent p 0.009) and 30 days 59.7 percent p 0.001). As with the other studies the treatment was well tolerated. What steps do you think should be taken for obese Americans to get to a healthy weight Q A Q Several years ago we developed a successful mathematical model for computer simulations that demonstrated how the body uses fuel in different energy consumption scenarios. It revealed body composition changes based on changes in diet. It validated that a calorie-is-a-calorie but even more importantly showed that what we choose to eat does have an effect on body composition. For example a calorierestricted high-protein low-carbohydrate diet resulted in more fat loss and less lean muscle loss. Note Kevin Hall working for the NIH (National Institutes of Health) recently introduced a similar computer model available at the USDA s (U.S. Department of Agriculture) Supertracker website ( bwp ml). A graduate student of mine used this model to observe how the changes in diet composition reported between 1979 and 2010 corresponded to the increase in average body weight for both men and women during the same time. It showed the obesity epidemic was caused by changing the types and amounts of food we eat. This student further simulated that if we were to return to the types and amounts of food we ate in the 1970s the obesity epidemic would reverse within five years. If the patient is committed to adopting a healthy lifestyle--diet exercise getting enough sleep and drinking enough water are all part of that. If there is an underlying cause of obesity the practitioner would need to identify it and treat it accordingly for there to be lasting success. When fat was declared the bad guy eating habits shifted to consuming more sugar. Americans need to increase fat intake such as avocados and olive oil back to 35-45 percent of their calories. Is there anything else you would like to add There is a lot of social pressure to fit a particular body type and the ideal body changes over time--just look at famous celebrities from each decade of the last century as an example. Not everyone is bred to be a Chihuahua just like not everyone is bred to be a Great Dane. Help patients learn to be comfortable with the body they have when they have a healthy lifestyle. The Health at Every Size (HAES) movement adopted that approach early on in the obesity epidemic. Fitness levels (physically and metabolically) are more important than fat levels when all is said and done. Simply losing fat does not increase fitness by any measure of fitness. The study suggests adequate nutrition in adolescence is important for the refinement of the adult brain and behavior. The structure and function of the brain continue to change throughout adolescence at the same time that teenagers gain increasing independence and begin to make their own food choices. Since high-calorie low-quality diets tend to be more affordable than healthy ones teenagers may opt for foods that lack key nutrients important for brain health such as omega-3 polyunsaturated fatty acids (n-3 PUFAs) which cannot be produced by the human body and must be obtained from foods such as fish and vegetables. Oliver Manzoni and colleagues fed mice a balanced diet until early adolescence when some mice were switched to a diet lacking n-3 PUFAs. Mice fed the poor diet during adolescence had reduced levels of n-3 PUFA in the medial prefrontal cortex and the nucleus accumbens in adulthood compared to control mice. The low-quality diet impaired the brain s ability to fine-tune connections between neurons in these regions. (Source The Journal of Neuroscience--June 2017) Increase in Use of High-dose Vitamin D Supplements rom 1999 through 2014 the number of U.S. adults taking daily vitamin D supplements above the recommended levels increased and 3 percent of the population exceeded the daily upper limit considered to possibly pose a risk of adverse effects according to a study published by JAMA. A 2011 report concluded that vitamin D was beneficial for bone health but noted possible harm (e.g. abnormally high levels of calcium in the blood soft tissue or vascular calcification) for intakes above the tolerable upper limit of 4 000 IU daily. The recommended dietary allowance for vitamin D is 600 IU d for adults 70 years or younger and 800 IU d for those older than 70 years. Using data from the nationally representative National Health and Nutrition Examination Survey (NHANES) Pamela L. F Lutsey PhD MPH of the University of Minnesota Minneapolis and colleagues assessed trends in daily supplemental vitamin D intake of 1 000 IU or more and 4 000 IU or more from 1999 through 2014. The analysis included 39 243 participants. The researchers found that the prevalence of daily supplemental vitamin D use of 1 000 IU or more in 2013-2014 was 18.2 percent in 1999-2000 it was 0.3 percent. In 2013-2014 prevalence of daily supplemental intake of 4 000 IU or more was 3.2 percent this figure was less than 0.1 percent prior to 2005-2006. Trends of increasing supplemental vitamin D use were found for most age groups race ethnicities and both sexes. In 2013-2014 intake of 4 000 IU or more daily was highest among women (4.2 percent) nonHispanic white individuals (3.9 percent) and those 70 years or older (6.6 percent). A limitation of the study is that data was self reported however participants were asked to bring supplement bottles to aid in reporting. Characterizing trends in vitamin D supplementation particularly at doses above the tolerable upper limit has important and complex public health and clinical implications the authors wrote. (Source JAMA--June 2017) Traditional Chinese Medicine May Benefit Some Heart Disease Patients raditional Chinese medicine (TCM) might be effective as a complement or alternative to traditional Western medicine for primary and secondary prevention of heart disease according to a state of the art review paper published in the Journal of the American College of Cardiology. Heart disease is the No. 1 cause of death worldwide and despite advances in Western medicine for treating and preventing heart disease unmet needs remain. As a result TCM is being increasingly looked at as a supplement to Western medicine but to date randomized controlled trials are overall of poor quality and flawed. Western scientists often reject Chinese medicine for specific reasons the formula consists of dozens of ingredients with many chemical molecules making it hard to clarify the therapeutic mechanism the medications available in China do not undergo the same rigorous approval T process as Western drugs to guarantee efficacy and safety and most trials were conducted in China by TCM physicians with medications largely unavailable in the United States. Researchers in this review looked at studies published over the past 10 years on randomized controlled trials of TCM used for patients with hypertension dyslipidemia diabetes pre-diabetes atherosclerotic cardiovascular disease and chronic heart failure to assess the efficacy and safety of TCM. In all certain Chinese medications showed suggested benefits for each of the cardiovascular health conditions studied. For example researchers looked at eight randomized controlled trials on TCM and hypertension. The evidence indicated that Tiankuijiangya Zhongfujiangya Qiqilian Jiangya and Jiangyabao have antihypertensive effects and a good safety profile making them a potential good alternative for patient intolerant of or who cannot afford Western medications. However whether those benefits transferred into long-term positive cardiovascular outcomes would have to be determined by long-term trials. Of note one should bear in mind that traditional Chinese medicine medications are usually prescribed as complex formulae which are often further manipulated by the practitioner on a personalized basis said Yuxia Zhao senior author of the review and a physician in the Department of Traditional Chinese Medicine at Shandong University Qilu Hospital in Jinan Shandong China. The pharmacological effects and the underlying mechanisms of some active ingredients of traditional Chinese medications have been elucidated. Thus some medications might be used as a complementary and alternative approach for primary and secondary prevention of cardiovascular disease. (Source Journal of the American College of Cardiology--June 2017) 48 NATURAL PRACTITIONER WWW.NATURALPRACTITIONERMAG.COM AUGUST 2017 Meditation and Yoga Can Reverse DNA Reactions That Cause Stress ind-body interventions (MBIs) such as meditation yoga and Tai Chi don t simply relax us they can reverse the molecular reactions in our DNA which cause ill-health and depression according to a study by the universities of Coventry and Radboud. The research published in the journal Frontiers in Immunology reviewed more than a decade of studies analyzing how the behavior of our genes is affected by different MBIs including mindfulness and yoga. Experts from the universities conclude that when examined together the 18 studies--featuring 846 participants over 11 years--reveal a pattern in the molecular changes which happen to the body as a result of MBIs and how those changes benefit our mental and physical health. The researchers focus on how gene expression is affected in other words the way that genes activate to produce proteins which influence the biological make-up of the body the brain and the immune system. When a person is exposed to a stressful event their sympathetic nervous system (SNS)-- the system responsible for the fightor-flight response--is triggered in turn increasing production of a molecule called M nuclear factor kappa B (NF-kB) which regulates how genes are expressed. NF-kB translates stress by activating genes to produce proteins called cytokines that cause inflammation at cellular level--a reaction that is useful as a short-lived fight-or-flight reaction but if persistent leads to a higher risk of cancer accelerated aging and psychiatric disorders like depression. According to the study however people who practice MBIs exhibit the opposite effect-- namely a decrease in production of NF-kB and cytokines leading to a reversal of the proinflammatory gene expression pattern and a reduction in the risk of inflammation-related diseases and conditions. The study s authors say the inflammatory effect of the fight-or-flight response which also serves to temporarily bolster the immune system would have played an important role in humankind s hunter-gatherer prehistory when there was a higher risk of infection from wounds. In today s society however where stress is increasingly psychological and often longerterm pro-inflammatory gene expression can be persistent and therefore more likely to cause psychiatric and medical problems. Lead investigator Ivana Buric from the Brain Belief and Behavior Lab in Coventry University s Centre for Psychology Behavior and Achievement said Millions of people around the world already enjoy the health benefits of mind-body interventions like yoga or meditation but what they perhaps don t realize is that these benefits begin at a molecular level and can change the way our genetic code goes about its business. These activities are leaving what we call a molecular signature in our cells which reverses the effect that stress or anxiety would have on the body by changing how our genes are expressed. Put simply MBIs cause the brain to steer our DNA processes along a path which improves our wellbeing. More needs to be done to understand these effects in greater depth for example how they compare with other healthy interventions like exercise or nutrition. But this is an important foundation to build on to help future researchers explore the benefits of increasingly popular mind-body activities. (Source Frontiers in Immunology-- June 2017) Acupuncture Relieves Pain in Emergency Patients he world s largest randomized controlled trial of the use of acupuncture in emergency departments has found the treatment is a safe and effective alternative to pain-relieving drugs for some patients. Led by RMIT University in Melbourne Australia the study found acupuncture was as effective as pain medicine in providing longterm relief for patients who came to emergency departments in considerable pain. But the trial conducted in the emergency departments of four Melbourne hospitals showed pain management remains a critical issue with neither treatment providing adequate immediate relief. Lead investigator Professor Marc Cohen from RMIT s School of Health and Biomedical Sciences said pain was the most common reason people came to emergency departments but was often inadequately managed. While acupuncture is widely used by practitioners in community settings for treating pain it is rarely used in hospital emergency departments Cohen said. Emergency nurses and doctors need a variety of pain-relieving options when treating patients given the con- T cerns around opioids such as morphine which carry the risk of addiction when used longterm. Our study has shown acupuncture is a viable alternative and would be especially beneficial for patients who are unable to take standard pain-relieving drugs because of other medical conditions Cohen continued. But it s clear we need more research overall to develop better medical approaches to pain management as the study also showed patients initially remained in some pain no matter what treatment they received. The study published in the Medical Journal of Australia and funded by a grant from the National Health and Medical Research Council involved 528 patients with acute low back pain migraine or ankle sprains who presented at the emergency departments of the Alfred Hospital Cabrini Malvern Epworth Hospital and Northern Hospital between January 2010 and December 2011. Patients who identified their level of pain as at least four on a 10-point scale randomly received one of three types of treatment acupuncture alone acupuncture plus pharma- cotherapy or pharmacotherapy alone. One hour after treatment less than 40 percent of patients across all three groups felt any significant pain reduction (two or more pain points) while more than 80 percent continued to have a pain rating of at least four. But 48 hours later the vast majority found their treatment acceptable with 82.8 percent of acupuncture-only patients saying they would probably or definitely repeat their treatment compared with 80.8 percent in the combined group and 78.2 percent in the pharmacotherapy-only group. Some Australian emergency departments already offer acupuncture when trained staff are available but further studies are needed on ways to improve pain management overall in emergency departments and the potential role for acupuncture in this Cohen said. We need to determine the conditions that are most responsive to acupuncture the feasibility of including the treatment in emergency settings and the training needed for doctors or allied health personnel. (Source Medical Journal of Australia--June 2017) AUGUST 2017 WWW.NATURALPRACTITIONERMAG.COM NATURAL PRACTITIONER 49 NATURAL HEALTH STUDIES Common Household Chemicals Lead to Birth Defects in Mice Research Finds new study at the Edward Via College of Osteopathic Medicine (VCOM) and the Virginia-Maryland College of Veterinary Medicine at Virginia Tech has found a connection between common household chemicals and birth defects. Known as quaternary ammonium compounds or quats the chemicals are often used as disinfectants and preservatives in household and personal products such as cleaners laundry detergent fabric softener shampoo and conditioner and eye drops. The research demonstrated a link between quats and neural tube birth defects in both mice and rats. These chemicals are regularly used in the home hospital public spaces and swimming pools said Dr. Terry Hrubec associate professor of anatomy at the VCOMVirginia campus and research assistant professor in the veterinary college s Department of Biomedical Sciences and Pathobiology. Most people are exposed on a regular basis. Dr. Hrubec investigated the effect of two commonly used quats alkyl dimethyl benzyl ammonium chloride and didecyl dimethyl ammonium chloride. These are often listed on ingredient lists as ADBAC and DDAC respectively and are valued for their antimicrobial and antistatic properties as well as their ability to lower surface tension. Dr. Hrubec found that exposure to these chemicals resulted in neural tube birth defects--the same birth defect as A spina bifida and anencephaly in humans. Birth defects were seen when both males and females were exposed as well as when only one parent was exposed said Dr. Hrubec who is first author on the study and holds both a doctor of veterinary medicine degree and PhD from the VirginiaMaryland College of Veterinary Medicine. The fact that birth defects could be seen when only the father was exposed means that we need to expand our scope of prenatal care to include the father. Dr. Hrubec found that mice and rats did not even need to be dosed with the chemicals to see the effect. Her research shows that simply using quat-based cleaners in the same room as the mice was enough to cause birth defects. We also observed increased birth defects in rodents for two generations after stopping exposure Dr. Hrubec added. An earlier study in Dr. Hrubec s laboratory found that these chemicals led to reproductive declines in mice. Follow-up research found that quats were decreasing sperm counts in males and ovulation in females. The research raises the possibility of quats contributing to human infertility which has been on the rise in recent decades. We are asked all of the time You see your results in mice. How do you know that it s toxic in humans Dr. Hrubec said. Our research on mice and rats shows that these chemicals affect the embryonic development of these animals. Since rodent research is the gold standard in the biomedical sciences this raises a big red flag that these chemicals may be toxic to humans as well. Quaternary ammonium compounds were introduced in the 1950s and 1960s before the standardization of toxicity studies. Chemical manufacturers conducted some toxicity studies on the compounds during this period but they were never published. Today the chemicals are regulated by the U.S. Environmental Protection Agency. Dr. Hrubec noted that an epidemiological study could determine whether people who have a high rate of exposure such as health care workers or restaurant servers have a more difficult time becoming pregnant or have a greater likelihood of having children with neural tube birth defects but no such study has been conducted to date.
