We found natural products were quite effective and didn t have the side effects of some of the prescription medications she had taken in the past. So the recent headline on the Canadian website Medical News Bulletin Approximately 50 percent of the U.S. adult population particularly older adults use dietary supplements to enhance their health and well-being made sense to me. The article s author Natasha Tetlow PhD noted that financial social and health issues affecting older adults over 60 years old can lead to poor dietary habits which often coincides with worsening health conditions. For this reason many older adults use dietary supplements to help meet the recommended dietary intakes of various micronutrients. Yet this can present serious health issues. Supplementary nutrients can cause individuals to exceed the recommended tolerable upper intake levels as many foods are already fortified with micronutrients. In addition when taken in combination with prescription medications the resulting drug supplement interactions can cause adverse health effects. It is for these reasons that dietary supplement use should be monitored however information regarding dietary supplement use is typically not shared with health care providers. Tetlow pointed to a recent study published in The Journal of Nutrition in which researchers looked at why dietary supplement use is so high in older U.S. adults and whether particular subgroups were more likely to consume these. The researchers considered how various demographics (sex age race education income) lifestyle (physical A activity smoking status alcohol consumption) and health factors (health and insurance status prescription medications taken in the past 30 days) influenced dietary supplement use in a cohort of 3 469 adults aged 60 years and above. The frequency and motivation for using dietary supplements in a 30-day period were also assessed. Those who were more likely to use supplements individuals over 70 years old from non-Hispanic white and Asian backgrounds tended to use the supplements to improve and maintain health rather than supplement their diets. However an issue associated with dietary supplement use is that many individuals taking prescription medications do not seek guidance from their health care physicians Tetlow noted. This leads back to an Off the Cuff article I wrote in September titled Poison Control in response to a study that found calls to poison control centers in the U.S. due to exposures to dietary supplements rose by nearly 50 percent between 2005 and 2012. The research found that a majority of those calls involved children being exposed to supplements. The report published in July in the Journal of Medical Toxicology called for an increase in regulation by the U.S. Food and Drug Administration for certain supplements that were associated with high amounts of toxicity. Whether or not more regulation is the answer remains to be seen. In the meantime the Council for Responsible Nutrition (CRN) recommended that consumers store dietary supplement products in safe places out of a child s reach. CRN also recommends that consumers talk with their doctor or pediatrician about their family s supplement use. I believe this holds true for everyone--no matter their age. I think it is wise for retailers to remind all customers young and old to let their health care professionals--doctor pharmacist nutritionist etc.--know they are taking dietary supplements. It does not matter that you may only be a small company or even a retailer with a single location. In this day and age virtually every business is an e-business. With this in mind recent hacking attempts have shown that no company whatever its size can consider itself immune from attack. An example of how small of a target can be vulnerable to hacking consider that internet security firms have confirmed the existence of ransomware posing as a Pokemon Go related app. This malware inserts itself into a user s cell phone encrypts all data stored therein and presents a demand for payment of 0.01 bitcoin (about 57). One variation of this type of malware also creates a backdoor into a I phone s windows operating system potentially allowing a third party to take control of the phone. Internet security experts have not determined why this is being done. It is estimated that more than two million users have downloaded malware directly from the Google Play Store into their androidbased devices. As to the potential risk to your business consider first does your company have an online website where it sells products and obtains customer payment order history and contact information Second even if your company does not maintain a website does it use a computer to maintain data about its employees such as health information salary history or social security numbers Third does your company maintain its product information inventory purchasing and sales and financial data on computer Fourth does your company use internal email or texting Fifth can employees bring their own devices to work and or can they use company computers for personal email shopping or merely searching the web Risk Reduction Any one of these factors alone can make your company vulnerable to a variety of hacking attacks but there are steps that can be taken which can help to reduce the risk and to protect your valuable computer information. According to the Second Annual State of Ransomware Report United States published in July 2017 by Malwarebytes an anti-malware software company [a]mong small to midsized organizations that have experienced a successful infiltration of the corporate network by ransomware 20 percent reported that they had to cease business operations immediately and 12 percent lost revenue both slightly lower than the global average. The problem for these companies is not only making the decision whether to pay the ransom it is also the significant amount of time that the company may be without its computers. This results in loss of business and damage to customer confidence. For example the last thing that any company wants is to have to inform its customers or employees that their personal information was hacked which is exactly what many state laws require them to do. Ransomware is a unique type of malware that does not seek to steal or destroy data from a computer system. Instead once activated it locks the computer blocking users from accessing information stored within. Thus any business that stores data on a computer or that needs a computer for its daily operations is vulnerable to ransomware. In the case of a ransomware attack it does not matter that a business does not store any personal financial information or health data on its systems--all that matters is that computers are needed to keep things up and running. Ransomware can be unknowingly downloaded when visiting malicious or infected compromised websites. It can also get into a system through other malware inserted onto a single user s computer (for example a thumb drive ) or more commonly by an unwitting user opening an attachment to spam email. Each one of these routes of attack relies on lack of discipline by curious computer users and highlights the need for organizations to educate employees to understand the need to exercise care in visiting websites and opening suspicious email attachments and avoiding anything that raises suspicions or just does not seem right. There are generally two types of ransomware one locks a system s computer screens and the other seeks out and ties up files that are likely to be important such as documents data bases and spreadsheets. In either case all known attacks have been accompanied by a demand for payment usually in electronic currency such as bitcoins. A second form of hack is even more insidious. This is when the hackers actually breach company computers for the purpose of obtaining sensitive customer business or employee data. One of the first big breaches of this type occurred in 2005 when shoe discounter DSW Inc. was hacked. DSW had used computer networks to obtain authorization for credit and debit card as well as check purchases at its stores and to track inventory. When the company notified consumers of the breach the Federal Trade Commission (FTC) also learned of the breach and opened an investigation. The FTC alleged that DSW s failure to secure customers sensitive information was an unfair practice because it caused substantial injury that was not reasonably avoidable by consumers and not outweighed by offsetting benefits to consumers or competition ( news-events press-releases 2005 12 dsw-inc-settles-ftc-charges). The company s settlement with FTC required it among other things to maintain a comprehensive information security program that includes administrative technical and physical safeguards and to have third party audits of its systems performed every two years for the 20 years as well as reporting regularly to the FTC. So if you fail to adequately protect your business computers you not only have to be concerned with the loss to your business and the loss to your customers (and possible lawsuits or other compensation)--you may find yourself having to deal with the FTC in addition to state or local consumer protection authorities. The most recent data breach in the news involves Equifax one of the three major credit reporting firms in the United States. The breach occurred from mid-May through July of 2017 during which the hackers accessed names Social Security numbers birth dates addresses and in some instances driver s license numbers of approximately 143 million people. It is reported that also stolen was credit card numbers for about 209 000 people and dispute documents with personal identifying information for about 182 000 people. Obviously the fallout from this data breach will continue for many months if not years to come. establish a robust cyber-security protocol. However the ultimate cost of the failure to do so can be devastating. A single attack has the potential to put a small business out of operation for a long period of time potentially causing fatal damage to its operations. Compounding the problem is that many small businesses do not have access to skilled IT personnel on staff meaning that the initial response to a ransomeware (or any kind of cyber) attack is often handled by someone without the expertise to deal with the situation effectively. unsecured guest Wi-Fi for visitors but personal and company data should only be sent on Wi-Fi with an encrypted password. Another real protection is to practice safe computer use. All employees should be trained in basic internet security precautions. This includes not opening emails from unknown sources and especially not clicking on unknown links or opening unknown attachments. Employees should not bring unauthorized memory sticks into the business nor should they click on links to unknown websites. The use of strong and unique passwords for both personal and business accounts and devices that change periodically are highly recommended. Training and reminding staff of these very basic precautions can greatly reduce a company s risk of learning about ransomware and data stealing the hard way. Finally everyone should maintain an external backup to their computer systems that backs up data on a frequent and regular basis. As frustrating a ransomware may be its devastating affect can be greatly blunted if there is a complete backup to the locked data. Grand opening FTC Complaint Against Quincy Bioscience Dismissed n September 29 Judge Louis Stanton of the U.S. District Court in the Southern District of New York dismissed the Federal Trade Commission (FTC) and the New York Attorney General s (NYAG) complaint against Wisconsin-based Quincy Bioscience the maker of Prevagen. In January the FTC and the NYAG charged the marketers of the dietary supplement Prevagen with making false and unsubstantiated claims that the product improves memory provides cognitive benefits and is clinically shown to work. The federal court complaint alleged that the defendants enticed consumers to spend anywhere from 24 to 68 for bottles of 30 supplement pills by touting the product s active ingredient--a protein derived from jellyfish--to improve memory and reduce memory problems associated with aging. The complaint noted that the supplement is widely available at major retailers such as Amazon CVS the Vitamin Shoppe and Walgreens and is also sold directly on Quincy Bioscience s websites. The company s product sales have topped 165 million according to the complaint. A study of 766 otherwise healthy adolescents showed that those who consumed the least vitamin K1--found in spinach cabbage iceberg lettuce and olive oil--were at 3.3 times greater risk for an unhealthy enlargement of the major pumping chamber of their heart according to the study published in The Journal of Nutrition. Vitamin K1 or phylloquinone is the predominant form of vitamin K in the U.S. diet. Those who consumed less had more risk said Dr. Norman Pollock bone biologist at the Georgia Prevention Institute at the Medical College of Georgia at Augusta University and the study s corresponding author. Overall approximately 10 percent of the teens had some degree of this left ventricular hypertrophy Pollock and his colleagues reported. Left ventricular changes are more typically associated with adults whose hearts have been working too hard too long to get blood out to the body because of sustained elevated blood pressure. Unlike other muscles a larger heart can become inefficient and ineffective. The scientists said they believe theirs is the first study exploring associations between vitamin K and heart structure and function in young people. While more work is needed their findings suggest that early interventions to ensure young people are getting adequate vitamin K1 could improve cardiovascular development and reduce future disease risk they wrote. In the 14-18 year olds who consumed the least vitamin K1 the study found the overall size and wall thickness of the left ventricle were already significantly greater and the amount of blood the heart pumped out significantly lower Pollock said. Like matrix Gla protein vitamin K is essential to increased production of osteocalcin a protein hormone important to bone metabolism and insulin sensitivity. Those parallels--and the fact that osteocalcin can t currently be given directly--led Pollock to pursue his ongoing clinical trial in obese children with higher-fasting glucose levels. Pollock s lab and other investigators in the United States and Europe are also looking at the impact of vitamin K supplements on adults with heart disease but adult findings to date have been inconclusive. Further study is needed to clarify the importance of vitamin K1 intake to cardiovascular development and to better understand how vitamin K dependent proteins like matrix Gla protein aid cardiovascular development and health the scientists noted. Participants wore activity monitors for seven days and completed between three and seven 24-hour periods of self-reports about what they ate. Approximately 70 percent had a least six days of food records which increases the accuracy of self-reports Pollock said. Substitutions Butter Try to use butter substitutes that come as a stick rather than in a tub as these will contain a similar amount of moisture than regular butter. Shortening coconut oil olive oil and silken tofu are all suitable substitutes. Using combinations of these ingredients will work best. Eggs Store-bought egg replacer is a good option for substitution. Alternative ingredients include milled flax seeds silken tofu mashed bananas or figs. Milk can also work if boosting with additional baking powder. If using flax seed it must be milled and combined with hot water before using. Gluten is defined as the protein fractions from grains like wheat rye barley triticale and hybrids thereof. For celiac and gluten sensitive individuals a gluten-free diet is their only medical treatment. These people get sick when there is a failure. For more information call (503) 646-8138 or visit 48 VITAMIN RETAILER WWW.VITAMINRETAILER.COM NOVEMBER 2017 SUPPLEMENTSCIENCE Curcumin & Cardiovascular Health By Gene Bruno MS MHS RH(AHG)-- Huntington College of Health Sciences urmeric and more specifically its primary active constituent known as curcumin has been used extensively in ancient and modern times for its medicinal qualities.1 2 It was used as a traditional remedy in Chinese and Indian ayurvedic medicine for more than 2 000 years 3 and the authors of a textbook on bioactive foods indicate that the use of turmeric in Indian folk medicine is one of a veritable panacea apparently efficacious for conditions that we would nowadays classify in the realm of infectious inflammatory metabolic and immunological diseases. 4 Furthermore research on the modern evidence-based uses of curcumin is extensive and includes studies demonstrating a wide spectrum of biological actions. These include anti-inflammatory antioxidant anti-carcinogenic anti-mutagenic anticoagulant anti-fertility anti-diabetic antibacterial antifungal antiprotozoal antiviral anti-fibrotic antivenom antiulcer hypotensive and cholesterol-lowering activities.5 The focus of this article is on curcumin s benefits for cardiovascular health--and specifically for T endothelial dysfunction. Endothelial Dysfunction The endothelium is the tissue that lines the interior surface of blood vessels (and lymphatic vessels). Normal functions of the endothelium include mediation of coagulation platelet adhesion immune function and control of volume and electrolyte content of the intravascular and extravascular spaces. In vascular diseases endothelial dysfunction is a disorder broadly defined as an imbalance between vasodilating and vasoconstricting substances produced by (or acting on) the endothelium.6 Endothelial dysfunction can result from and or contribute to several disease processes including hypertension (high blood pressure) hypercholesterolaemia diabetes septic shock and Behcet s disease (a rare autoimmune disorder causing blood vessel inflammation).7 Maintaining healthy endothelial function is of vital importance for supporting cardiovascular health so measuring endothelial function is a good reflection of cardiovascular health. When blood flow increases through a vessel the vessel dilates. This phenomenon is referred to as flow-mediated dilatation (FMD). FMD is a sensitive measure of endothelial function and serves as a predictive marker of cardiovascular risk. Therefore improvements in endothelial function as measured by FMD are indicative of a reduced cardiovascular risk.8 Supplementation with curcumin is one way to help promote healthy endothelial function via FMD. Preliminary Research With Curcumin Excess fructose consumption is a risk factor for metabolic syndrome causing hyperuricemia (excess of uric acid) and endothelial dysfunction in the kidneys. In a study with fructose-fed rats curcumin lowered uric acid levels and appeared to ameliorate endothelial dysfunction in the kidneys.9 Likewise in a randomized controlled crossover study 10 14 healthy male subjects were given a single serving of curry 50 VITAMIN RETAILER WWW.VITAMINRETAILER.COM NOVEMBER 2017 meal (a natural source of curcumin) or spice-free control meal to determine if it would improve endothelial function. Before and one hour after consumption fasting and postprandial FMD responses were measured. According to the results curry meal increased FMD from 5.2 2.5 percent to 6.6 2.0 percent (P 0.001) whereas the control meal decreased FMD from 5.8 2.4 percent to 5.1 2.3 percent (P 0.039). The postprandial FMD after the curry meal was significantly higher than after the control meal (P 0.002). The researchers concluded that consumption of curry ameliorates postprandial endothelial function in healthy male subjects and may be beneficial for improving cardiovascular health. Of course a study in rats and a study on a single serving of curry is not enough to definitively demonstrate that curcumin is effective for improving endothelial function. However a well-designed and executed human study on curcumin has shown just that. Curcumin Bioavailability Before proceeding to a discussion of the human study on curcumin and endothelial function it is important and relevant to first address the fact that curcumin has relatively poor biovailability--with 40 to 75 percent of curcumin passing through the digestive tract unchanged in animal research.11 Also blood concentrations of curcumin are low and tissue distribution is limited following oral dosing due to its fast metabolic turnover in the liver and intestinal wall.12-20 Even upon intake of doses as high as 10 or 12 g curcumin maximum plasma curcumin concentrations in humans remain in the low nanomolar range ( 160 nmol L).21 There are however a few different commercial curcumin extracts that are supported by research showing better bioavailability vs. regular curcumin. One such extract whose effects on endothelial dysfunction will be discussed below is CurcuWIN a novel curcumin formulation from OmniActive Health Technologies containing 20 percent curcuminoids. A recent human study22 showed that the absorption of total curcuminoids in the blood of this water-dispersible form of curcumin was 45.9-fold higher than normal curcumin. Curcumin s Effect on Endothelial Dysfunction A double-blind randomized placebo controlled parallel study23 was conducted to examine the effect of differing doses of curcumin on FMD and consequently on endothelial dysfunction. Researchers from Texas Christian University (USA) Massey University (New Zealand) and Increnovo LLC (USA) recruited 59 moderately trained men and women to participate. Subjects were assigned to placebo 50 mg or 200 mg curcumin (from 250 and 1 000 mg CurcuWIN respectively) for eight weeks. The results of this study were originally presented at the Experimental Biology 2016 Meeting in San Diego and subsequently published in the Journal of Nutrition and Metabolism. Results showed that 200 mg of curcumin produced a dose-mediated improvement in endothelial function as measured by FMD. The outcome was a clinically significant 3.0 percent increase relative to placebo (p 0.020). Those in the placebo group had a decrease in FMD. Consequently the improvement in FMD with the 200 mg dose of curcumin represented a 37 percent increase over placebo. The researchers concluded that eight weeks of 200 mg oral curcumin supplementation resulted in a clinically significant improvement in endothelial function as measured by FMD in apparently healthy adults. Not only does this suggest that oral curcumin supplementation may present a simple lifestyle strategy for decreasing the risk of cardiovascular diseases but other research suggests that for every 1 percent increase in FMD there is a 9 to 17 percent decrease in cardiovascular disease.24 Therefore daily supplementation with 1 000 mg CurcuWIN (providing 200 mg of curcumin) had a clinically meaningful impact on potentially reducing cardiovascular risk by 15 to 50 percent in healthy individuals. Conclusion Curcumin has been used extensively in ancient and modern times for its medicinal qualities with research demonstrating a wide range of biological effects. A particular benefit of curcumin supplementation is protection against and improvement of endothelial dysfunction through an increase in FMD. CurcuWIN which has been shown to have 45.9-fold higher bioavailability vs. standard curcumin has also demonstrated a significant increase in FMD vs. placebo in clinical study. One would think that is good news. If we know some of the factors that contribute to cancer etiology why aren t we doing a better job with prevention A part of the problem is finding out something is a risk factor for cancer after the fact. For example we all know smoking is a risk factor not just for lung cancer but for thyroid cancer head and neck cancer and more. So why doesn t everyone stop smoking I grew up in a time when cigarette commercials were on TV airplanes had smoking sections and patients (and doctors ) were allowed to light up in hospitals. Many people were addicted before the risks were fully understood. And even today there is a great deal of denial among smokers that they will lose the lottery and get a smoking related cancer. Overweight and obesity also causes 52 VITAMIN RETAILER cancer. Excess adipose (fat) tissue upsets normal hormone metabolism and also puts you into a pro-inflammatory state. Chronic inflammation is one of the most powerful root causes of genetic changes that lead to cancer. Environmental toxins and food additives cause cancer. Industrialized countries have far higher cancer rates than non-industrialized. So why don t we just stop Mobilizing the U.S. population to make a dramatic change is like trying to turn a tsunami. Cancer is not an invader it is the body s own damaged cells malfunctioning in such a way that they cause death and destruction. Any antioxidant plays a role in cancer prevention to some extent. Will eating spinach help prevent cancer Yes. Oranges Yes. Blueberries Yes. There is not a one-size-fits-all antioxidant so humans need to eat a variety of antioxidant foods to stay healthy. Each of these supplements has an array of tools to prevent certain types of oxidative damage and subsequently help prevent cancer. One way to measure antioxidant strength is using an oxygen radical absorbance capacity (ORAC) value measured in units per 100 grams. In food a good ORAC value can be between 6 000 and 36 000. However in the case of a clinically studied curcumin with turmeric essential oils testing showed the ORAC value as nearly 1.5 million and in the case of a scientifically studied grape seed extract called VX1 that testing showed a value of over two million. That is an example of very high levels of antioxidant activity and contributes to the science showing these botanicals can help prevent cancer and cancer recurrence. Grape seed VX1 has also been shown to modulate a tumor promoting pathway in the body called hippo-yap. It is the first compound to show significant activity on this pathway which holds great promise for cancer prevention. And pomegranate is not only an excellent antioxidant it provides unique plant compounds that are especially active against hormonally related cancers. without fully inhibiting any of them. This is why it can so effectively reduce inflammation without the side effects of non-steroidal anti-inflammatory drugs (NSAIDs) which block important enzyme pathways in the body resulting in gastrointestinal mucosal erosion reduced kidney function and increased risk of heart attack and stroke. Curcumin with turmeric essential oil has been shown to help alleviate major depressive disorder and support cognitive function in older adults. Curcumin has also shown benefits for every type of cancer for which it has thus far been tested. This is due in part to its unique role as antioxidant anti-inflammatory and epigenetic modulator. It deserves its reputation as the ultimate anti-cancer herb. cannabinoid receptors can have a significant influence on health and hemp oil contains some of those compounds. CBD has been found to reduce inflammation via unique pathways and provide pain relief as well. It is also active in selectively inhibiting cancer cell proliferation and tumor growth. Promote Detoxification Many of the carcinogens to which we are exposed are detoxified by the liver and prepared for excretion from the body. The better your liver works and the more quickly it takes carcinogens out of circulation the lower your risk of cancer related to toxic exposure. Not all the toxins in your body are environmental. Some are naturally occurring. For example as estrogen is metabolized it becomes broken down and can convert into forms that are more likely to be tumor promoting. Your body strives to eliminate used-up estrogen via the liver and creates new estrogen to take its place. One of the easiest ways you can prevent cancer is to increase the fiber in your diet. This is because fiber is a super sponge. The toxins are dumped into the gut from the liver and if your last meal was ice cream white rice and cheese there will be virtually no sponges available to bind up the toxins. Toxins that are not bound are reabsorbed which circulates the toxin all over again doubling or tripling your carcinogenic exposure. That is why increased fiber intake can help with hormonal issues too like premenstrual syndrome or perimenopause symptoms. If you cannot eat a diet high in fiber you can use supplemental fiber. The recommendation for optimal intake is 25 to 30 grams a day and most Americans get less than half of that amount. And once again curcumin is an effective botanical. It reduces inflammation in the liver and promotes liver cell activity. It has been shown to have benefits even in fatty liver disease. In addition to enhancing liver detoxification processes milk thistle may also be anti-angiogenic meaning it helps suppress the growth of blood vessels to feed cancer tumors. While hemp oil is in a bit of a regulatory purgatory at present regarding the notation of important compounds like cannabidiol (CBD) it remains an effective anti-inflammatory and anti-cancer botanical. Some of the best preventative activities seem simple on paper but can be very difficult to institute. Don t smoke keep your body weight in check eat nutritious meals high in fiber and low in additives. Don t breathe polluted air or expose yourself to excessive radiation. Even though we cannot fully control our risk for cancer there are certainly steps that can be taken to reduce the probability that we will have to face this challenging illness. If you can effectively reduce both inflammation and oxidative stress and increase the liver s ability to detoxify problematic substances you are well on your way to a healthy prevention plan. Yet I noticed that many of the patients I was seeing had these symptoms but no evidence of candida being out of control. Something else had to be responsible for their symptoms and an overgrowth of bacteria in the small intestine seemed like a very good explanation. In the last 10 years or so SIBO has become a big buzzword in research and on the internet. Many customers entering into health food stores are looking for help. S SIBO Explained The stomach and small intestine is designed to be relatively free of microorganisms. The reason should be obvious--the presence of microorgan56 VITAMIN RETAILER isms overgrowing in the small intestine would set up a scenario where they start utilizing various nutrients before they have had a chance to be absorbed. The result is fermentation of carbohydrates and putrefaction of proteins. This can produce a lot of gas as well as digestive symptoms that resemble the irritable bowel syndrome (IBS) as well as some additional accompanying symptoms. Normally these gases are not expired in high amounts but with SIBO their levels can be quite high. Of the two sugars for testing glucose usually gives more meaningful results but only represents bacterial overgrowth in the first portion of the small intestine. Lactulose is more helpful if the overgrowth occurs in the ileum the last part of the small intestine. The diagnosis of SIBO via breath testing is given if the patient expires greater than normal levels of either hydrogen or methane. SIBO can be hydrogen dominant methane dominant or mixed. Hydrogen dominant SIBO is WWW.VITAMINRETAILER.COM NOVEMBER 2017 most associated with diarrhea while methane dominant is most associated with constipation. This difference reflects an overgrowth of different types of gut bacteria. Absorption and the Small Intestine he small intestine is usually more than 21 feet long and is divided into three segments the duodenum is the first 10 to 12 inches the jejunum is the middle portion and is about 8 feet long and the ileum which is about 12 feet long. The small intestine participates in all aspects of digestion absorption and transport of ingested materials. It secretes a variety of digestive and protective substances as well as receives the secretions of the pancreas liver and gallbladder. Absorption of minerals occurs predominately in the duodenum absorption of water-soluble vitamins carbohydrates and protein occurs primarily in the jejunum and the ileum absorbs fatsoluble vitamins fat cholesterol and bile salts. T bohydrates (oligosaccharides) and sugars that are fermented by intestinal bacteria yielding large amount of gases like hydrogen or carbon dioxide thus causing abdominal bloating. Sources of FODMAPs include most legumes vegetables fruit and grains. So the diet is extremely limiting and not sustainable. Fortunately recent studies indicate that use of digestive enzyme supplements especially those designed to digest the various offending oligosaccharides and sugars may be as effective in improving digestive symptoms without having to forego so many health-promoting foods. Digestive Enzymes in SIBO Digestive enzymes especially proteases and lipases are an important protective factor against SIBO. Insufficient output of digestive enzymes from the pancreas is associated with many symptoms associated with SIBO and may represent a key underlying factor in many cases. Digestive enzymes are also the likely host defense mechanisms within the gut that prevents the formation of biofilm--a collection of bacteria closely packed together that adhere to the lining of the small intestine within a slimy gluey matrix. In general methane-producing bacteria are more likely to produce biofilm and are often more difficult to clear. Digestive enzymes are capable of eating away at the biofilm matrix as well as acting as a deterrent for bacterial overgrowth in the small intestine. In general for SIBO I recommend a high potency digestive enzyme preparation be taken just before a meal. What Causes SIBO SIBO often represents a breakdown in protective mechanisms. There are several built-in factors that prevent bacterial overgrowth in the small intestine foremost are digestive secretions of bile and digestive enzymes along with the peristaltic contractions that move the food bolus through the small intestines. A lack of enzymes bile or other digestive secretions as well as reduced peristalsis greatly increases an individual s risk of having bacterial or candida overgrowth as well as an intestinal infection including chronic candida infections of the gastrointestinal tract. Another key barrier to bacterial overgrowth in the small intestine is the ileocecal valve that separates the colon from the small intestine. Much like a door this mechanical barrier is designed to prevent too many of the bacteria residing in the colon to enter the small intestine. A lack of enzymes bile or other digestive secretions as well as reduced peristalsis greatly increases an individual s risk of having bacterial or candida overgrowth as well as an intestinal infection including chronic candida infections of the gastrointestinal tract. As far as dealing with the bacterial overgrowth the conventional medical treatment of SIBO relies primarily on the administration of antibiotics. However this approach ultimately creates additional problems due to further disturbance of the microbiome. In contrast the natural approach focuses on dealing with the bacterial overgrowth by restoring the proper functioning of the protective barriers to SIBO or supplementation strategies designed to produce similar effects. Foremost in this latter goal are using HCl replacement therapy digestive enzymes and herbal agents that promote biliary secretions and peristalsis (e.g. berberine ginger artichoke milk thistle and other choleretics). It is also helpful to use natural agents to reduce the bacterial overgrowth. Foremost among these agents in my opinion are digestive enzymes berberine and enteric-coated peppermint oil. Berberine in SIBO Plants that contain the alkaloid berberine such as goldenseal (Hydrastis canadensis) barberry (Berberis vulgaris) Oregon grape (Berberis aquifolium) and goldthread (Coptis chinensis) have a long history of use in infectious diarrhea. More recently there have been numerous studies with pure berberine that have shown significant success in the treatment of acute diarrhea. Berberine has been found effective against diarrheas caused by a number of different types of organisms including E. coli (traveler s diarrhea) Shigella dysenteriae (shigellosis) Salmonella paratyphi (food poisoning) B. Klebsiella Giardia lamblia (giardiasis) Entamoeba histolytica (amebiasis) and Vibrio cholerae (cholera). These results indicate berberine appears to be effective in treating the majority of common gastrointestinal infections with results comparable to standard antibiotics in most cases. In fact results were better in several studies. The advantage of berberine over conventional antibiotics is that it exerts selective antimicrobial action as it targets a wide range of disease causing organisms including Candida albicans yet exerts no action against health promoting bacterial species such as Lactobacilli and Bifidobacter species. Several other studies suggest berberine may be effective in SIBO. In animal models berberine improves intestinal motility. This action is another key goal VITAMIN RETAILER 57 Treatment of SIBO Most often the treatment of SIBO involves a low carbohydrate diet combined with strategies to deal with the bacterial overgrowth. A low FODMAP diet has emerged as the primary dietary adjunct. FODMAPs are short-chain car- NOVEMBER 2017 WWW.VITAMINRETAILER.COM NATURALMEDICINE in patients with SIBO. And while berberine has not been studied in SIBO it has been studied in irritable bowel syndrome (IBS) with very good results. In a 2015 double-blind study published in Phytotherapy Research 196 patients with diarrhea predominant IBS were randomized to receive either berberine (200 mg) or a placebo (vitamin C 200 mg) twice a day for eight weeks. The berberine group but not the placebo group reported significant improvement in diarrhea and less urgency and frequency in defecation. The berberine group also experienced a 64.6 percent reduction in abdominal pain compared with initial scores at the end of the score. Berberine significantly decreased the overall IBS symptom score anxiety score and depression score. Lastly and not surprisingly berberine was associated with an increased quality of life score in patients while no such change was seen in the placebo group. Berberine has been extensively studied in clinical trials for lowering blood sugar lipids and hypertension. In more than 25 double-blind studies berberine has shown efficacy on par with conventional medications in these applications. I bring this up because in these studies the dosage was typically 500 mg two to three times daily before meals. This dosage level may bring about quicker results than those found in the eightweek trial in IBS and is likely the dosage that would show more consistent results in SIBO. Berberine is my preferred herbal recommendation for SIBO especially when diarrhea is a common feature. tional benefit of these volatile oils is their efficacy against bacteria or Candida albicans that may be overgrowing in the small intestine. The typical dosage of ECPO is 200 mg twice daily between meals. Final Comments Once symptoms suggest that the overgrowth of bacteria have been reduced a repeat breath test or symptom evaluation will determine how successful treatment has been. If symptoms are 90 percent better the next step will be prevention of recurrent SIBO. If symptoms are still present after the appropriate time on antimicrobials a repeat breath test may be helpful to determine how much the levels have changed and then influence the prognosis and length of treatment. Peppermint oil and presumably similar volatile oils such as those found in oregano rosemary thyme and caraway seeds exhibit a number of beneficial effects helpful in SIBO. Like berberine these volatile compounds exert selective antimicrobial effects and ECPO has shown very good results in IBS. Enteric coating is believed to be necessary because menthol and other volatile compounds in peppermint oil are rapidly absorbed. This rapid absorption tends to limit its effects to the upper intestine and may lead to esophageal reflux and heartburn. Nearly a dozen double-blind studies have shown ECPO preparations to be extremely helpful in IBS. ECPO is thought to work by improving the rhythmic contractions of the intestinal tract and relieving intestinal spasm. That is in spite of their taking folic acid in various supplements and eating fortified with folic acid. Unfortunately up to 60 percent of the population does not convert folic acid optimally into the active form that the body actually uses--5-methyltetrahydrofolate (or 5-MTHF for short). Those with one of the 50 genetic variations convert almost no folic acid to the useable form. Folate is the form found naturally in foods such as dark green leafy vegetables and Brussels sprouts. Sometimes called vitamin B9 it also needs to be converted to 5-MTHF. The scientific literature usually refers to generic folate but there seems to be no controversy that what does the work in the body is the active form 5-MTHF or that it is needed to regulate cell growth and repair tissue from conception through old age. Textbooks tell us that active folate helps regulate DNA and key metabolic processes needed for healthy mood memory circulation immune function and overall well-being. Folate is also involved in the production of various neurotransmitters like serotonin dopamine and melatonin. Anti-depressant medication may work better if the patient is supplemented with folate.1 Homocysteine as we ve learned is implicated in heart disease and cancer. Folate is required for the methylation process that neutralizes homocysteine. There is also some evi- dence that it helps regulate cholesterol.2 Inadequate folate levels are associated with infertility low birthweight babies and ADD. In 1996 the U.S. Food and Drug Administration (FDA) mandated fortification of grain products (like bread and cereal). At that time it was believed that synthetic folic acid was naturally changed in the human gut into the natural form. However recent science has shown that this step actually takes place in the liver. In addition to the genetic variations that can reduce and individual s capacity for converting folic acid to 5MTHF apparently anyone s metabolic system can become overloaded if for example he or she eats too many foods fortified with folic acid. The overloaded system may then dump unchanged folic acid into circulation creating unwelcome effects ... especially for those being treated for leukemia and arthritis. But fortifying the body s immune response with aged garlic extract (AGE) can help protect against the plethora of diseasecausing viruses and bacteria that we are exposed to every day. Compared to placebo those taking the AGE supplement had significant increases in -T cell (a unique type of intestinal T cell that responds to pathogen-associated molecular patterns) and natural killer (NK) cells. Glutathione levels also increased due to the sulfur-containing compounds naturally present in AGE. This was shown by an uptick in thiol levels among the AGE group. Studies suggest that glutathione enhances immunity by optimizing macrophage function while protecting lymphocytes from oxidative damage and apoptosis.2 3 Higher glutathione levels also indicate that the sulfur compounds in AGE are readily absorbed and utilized by immune cells.4 By the end of the study those taking the AGE reported a 61 percent reduction in the duration of their illness as well as a 21 percent drop in the severity of symptoms. They also experienced a 58 percent reduction in the number of days missed work or school days.1 Aging garlic using a pro- M prietary 20-month process creates a number of beneficial compounds that can influence immune function. Primary among those are the organosulfer compounds S-allyl cysteine and S-allylmercaptocysteine.5 AGE also contains specific immune-stimulating compounds such as lectins fructooligosaccharides apigenin N (alpha) fructosyl arginine (the product of a non-enzymatic reaction between sugars and proteins known as a Maillard reaction) and tetrahydro-carbolines.6-8 AGE also acts as a prebiotic stimulating the growth of friendly bacteria in the digestive tract. When I first started a company in the 1980s I thought I was in a partnership with a like-minded individual. About six months into our working relationship I realized we were not a good fit. This person only wanted to sell our products in case lots. My idea was to sell whatever a store wanted to buy. I knew that some smaller stores couldn t afford cases and only wanted one two or three items. After six months of not changing our minds we parted ways. I never put myself in that position again. The lesson Do your homework thoroughly. Know everything about anyone with whom you choose to partner. And if there is any kind of contract involved you will be living with that for a long time. Go slowly and know yourself and your partner s philosophy very well before going forward. could achieve alone. A strong partner can even be someone with whom you can bounce off ideas--a sounding board for thoughts and brainstorming.
